OBJECTIVE: Takayasu arteritis (TAK) and giant cell arteritis (GCA) are 2 major variants of large vessel vasculitis (LVV). The frequent involvement of large vessels in GCA has raised the possibility that TAK and GCA should be regarded as 1 disease. By detailed phenotyping of a single-center cohort, we aimed to define the differences between TAK and GCA.
METHODS: Forty-five patients (23 TAK, 22 GCA) were identified. Baseline characteristics, clinical symptoms, laboratory data, enhanced computed tomography/magnetic resonance imaging, treatments, and clinical courses were retrospectively assessed…

A young woman (G4P2L0A1) diagnosed with a case of Takayasu arteritis (Type IV) and a history of recurrent pregnancy loss presented at 7 weeks of gestation with chronic hypertension. She was on immunosuppressive and antihypertensive therapy preconceptionally. At 29 weeks, she developed a hypertensive emergency with stage 2 fetal growth restriction. Despite aggressive management, blood pressure (BP) remained elevated (up to 200/110 mm Hg). A preterm caesarean section was performed at 31 weeks under steroid cover, resulting in the delivery of a 1.45 kg male neonate. Both mother and baby had uneve…

OBJECTIVES: Cutaneous arteritis (CA) is a skin-limited medium-vessel vasculitis. Some patients with CA also exhibit extracutaneous manifestations, such as vasculitic peripheral neuropathy (VPN). VPN is likely underdiagnosed because nerve biopsies are invasive and often impractical. This study proposed the concept of provisional CA to better identify patients with neuropathic involvement and to evaluate the prevalence and electrophysiological features of VPN.
METHODS: We retrospectively analysed patients with provisional CA treated at the Severance Hospital between 2011 and 2024. Provisional C…

Giant cell arteritis (GCA), also known as temporal arteritis, is the most common systemic vasculitis in individuals over 50 and presents diagnostic challenges due to its nonspecific symptoms such as fever, headache, and fatigue. This case report describes the details of a male patient in his 70s who presented with recurrent intermittent fevers of unknown origin and was ultimately diagnosed with GCA after an extensive workup. His initial CT scans and lab tests were unremarkable. However, after a rheumatological workup displayed elevated erythrocyte sedimentation rate (ESR) and C-reactive protei…

BACKGROUND AND AIMS: Polymyalgia rheumatica (PMR) and giant cell arteritis (GCA) are inflammatory disorders predominantly affecting older adults. PMR is primarily characterized by proximal muscle pain and systemic inflammation, while GCA involves large-vessel inflammation, posing risks of vision loss and stroke. Due to overlapping clinical features and age-related prevalence, diagnosing these conditions remains challenging. While PMR is largely diagnosed based on clinical symptoms and laboratory markers, GCA often requires histopathological confirmation via temporal artery biopsy (TAB). This s…

BACKGROUND: Takayasu arteritis (TAK) is a granulomatous large-vessel vasculitis of unknown etiology that predominantly affects the aorta and its branches. Ocular involvement occurs due to ischemia from vascular stenosis or occlusion and may present as transient visual loss, Takayasu retinopathy (TR), or ischemic optic neuropathy. In rare cases, ocular symptoms can be the initial manifestation, underscoring the importance of early recognition.
CASE PRESENTATION: We report a 29-year-old male who presented with progressive blurring of vision, more marked in the left eye, for nine months. Systemi…

Crohn's disease (CD) is a chronic, recurrent, transmural inflammatory disease with lesions anywhere in the gastrointestinal (GI) tract. Takayasu arteritis (TA) is an idiopathic, chronic, granulomatous inflammatory panarteritis that involves the aorta and its branches, known as "pulseless disease." Crohn's disease and TA are both associated, as both are granulomatous disorders, and multiple cases of simultaneous occurrence of both diseases have been reported. Here we are reporting a rare case of simultaneous CD and TA in a young female who had a large enteroenteric fistula between the colon and…

OBJECTIVE: Accurate clinical assessment of disease activity in Takayasu arteritis (TAK) can be challenging. 18F-fluorodeoxyglucose-positron emission tomography (FDG-PET) can directly measure vascular inflammation. This study details the development of a new type of disease activity index called the Takayasu's Arteritis Integrated Disease Activity Index (TAIDAI).
METHODS: Clinical symptoms for TAIDAI were identified from a literature review. Each symptom was paired with FDG-PET findings in corresponding arterial territories. Constitutional symptoms were paired with acute phase reactant levels.…

Takayasu arteritis is a chronic, idiopathic, inflammatory disease mainly affecting medium and large vessels with a significant rate of morbidity and mortality. The vessels most frequently affected are the aorta and its branches; branches originating from the aortic arch include right brachiocephalic trunk and its branches, left common carotid artery, left subclavian artery, coronary arteries from the ascending aorta, celiac trunk, right and left renal arteries, superior and inferior mesenteric arteries from the descending aorta, and right and left iliofemoral arteries. Local and systemic infla…

PURPOSE OF REVIEW: There have been advances in the diagnosis and treatment of giant cell arteritis (GCA) and polymyalgia rheumatica (PMR).
RECENT FINDINGS: Themes in PMR and GCA include classification criteria, ultrasound imaging of temporal and axillary arteries replacing biopsies for diagnosis of GCA, faster diagnosis and treatment with rapid access clinics for suspected GCA, and expanding treatment options with the goal of rapid suppression of inflammation and sparing steroids.
SUMMARY: Treatment is aimed at suppressing inflammation quickly in both GCA and PMR. Randomized trials have demo…

OBJECTIVES: To investigate the clinicopathologic features of patients with giant cell arteritis (GCA) who had thoracic aorta aneurysm or dissection surgery.
METHODS: Patients who had thoracic aorta surgery between January 1, 2000, and December 31, 2021, at the Mayo Clinic, Rochester, Minnesota, were identified with current procedural terminology (CPT) codes. The identified patients were screened for a prior diagnosis of GCA with diagnostic codes and electronic text search. The available medical records of all the patients of interest were manually reviewed. Thoracic aorta tissues obtained dur…

BACKGROUND: Takayasu arteritis (TAK) is an autoimmune inflammatory disorder with an undefined etiology. This study aimed to characterize the glycosylation profiles of serum immunoglobulin G (IgG) in patients with TAK.
METHODS: Lectin microarrays containing 56 types of lectins were used to detect the glycan levels of serum IgG in 164 patients with TAK, 128 patients with atherosclerosis used as disease controls (DCs), and 100 healthy controls (HCs). Differentially altered glycosylation patterns between TAK and control groups as well as between TAK subgroups were identified and further validated…

INTRODUCTION: Renovascular disease and hyperthyroidism are secondary hypertension. Takayasu arteritis (TAK) is a chronic, progressive, nonspecific great vasculitis involving the aorta and its major branches. It is one of the causes of renal artery stenosis. Hyperthyroidism is an endocrine disease caused by improper continuous synthesis and secretion of excessive thyroid hormone by the thyroid gland. Both diseases can raise blood pressure (BP).
CASE PRESENTATION: we present a case of 18-year-old. Female, after exercise, fatigue palpitations. The maximum BP was 190/87 mm Hg, ankle-brachial inde…

Giant cell arteritis (GCA) usually presents with headache, scalp tenderness and raised inflammatory markers. GCA presenting with a clinically evident cranial nerve palsy is rare and may result in a delayed or missed diagnosis if not suspected. We present the rare case of a woman in her 70s with histologically confirmed GCA presenting with a unilateral sixth nerve palsy, which responded to treatment with high-dose oral prednisolone.
