Topiramate-induced weight loss depends on level of intellectual disability in patients with epilepsy.

OBJECTIVE: Intellectual disability (ID) is associated with weight gain caused by antiepileptic drugs such as valproic acid. The present study analyzed the relationship between ID and weight loss caused by topiramate (TPM). METHODS: Seventy-eight patients with epilepsy (35 women, aged 18 to 70years) were enrolled in this prospective study. Body weight was measured before and 1, 6, 12, and 18months after initiation of TPM treatment. Both patients and caregivers were provided information about TPM-related weight loss. The patients were divided into the group with no or mild ID (intelligence quot

Management of nutritional alterations and issues concerning quality of life.

Malnutrition is common in human immunodeficiency virus (HIV) infection and plays an important role in morbidity and mortality. Malnutrition can affect hospitalizations, disease complications, quality of life, and survival, and has adverse clinical consequences that may be independent of CD4 lymphocyte count. There have been recent advances in knowledge concerning the pathogenesis of malnutrition and the nature of weight loss in HIV patients. The onset of body cell mass depletion may occur early in the infection and predate significant immune deficiency, implying that the virus itself may be in

Association of Weight Changes With Changes in Histological Features and Blood Markers in Nonalcoholic Steatohepatitis.

BACKGROUND AND AIMS: Weight loss is recommended for patients with non-alcoholic steatohepatitis (NASH) but the impact of weight change on disease activity remains unclear. We examined the association between weight change (gain/loss) and changes in biochemical and histological features of NASH. METHODS: This was an analysis of the PIVENS and FLINT trials in adults with NASH who had liver biopsies at baseline and at either 1.5 years or 2 years. Multivariable regression models examined how weight change was associated with changes in (a) blood liver markers, (b) NASH resolution with no fibrosis

Race differences in the effect of obesity on inflammation among Black and White older adults who quit smoking: a longitudinal analysis of the Health and Retirement study.

IMPORTANCE: Race differences in inflammation have been attributed to higher body mass index (BMI); however, few studies have comprehensively examined whether changes in obesity influence changes in C-reactive protein (CRP) by race, particularly following smoking cessation. OBJECTIVE: As CRP declines after quitting smoking, the current study examined whether longitudinal changes in multiple measures of obesity were differentially associated with CRP change among non-Hispanic Black (NHB) and non-Hispanic White (NHW) older adults who formerly smoked. DESIGN: This is a longitudinal analysis of t

Effects of decreased masseter muscle function and its recovery on mandibular morphology in growing mice: condylar structural properties and transcriptome changes.

Reduced masticatory muscle function during the growth period is implicated in the development of craniofacial deformities; however, its effects on skeletal development remain unclear. This study investigated systemic and local skeletal responses to reduced masticatory muscle function using botulinum neurotoxin type A (BTX) in growing mice. Four-week-old male mice were divided into three groups: a saline-injected Sham group; a BTX-injected group (both injected into the right masseter muscle); and an untreated control group. Blood samples, body weight, and mandibular deviation were assessed 2, 4

Muscle Dysfunction and Bone Loss in a Woman With Cystic Fibrosis and Obesity Treated With Glucagon-Like Peptide 1 Agonist: A Case Report.

Cystic fibrosis (CF) modulator therapies can lead to rapid and excessive weight gain. Obesity in CF can lead to undesirable metabolic complications including type 2 diabetes. Therefore, glucagon-like peptide 1 (GLP-1) agonists which can facilitate weight loss and improve metabolic profiles are increasingly prescribed to people with CF. We present the first case report documenting serial changes in body composition and bone mineral density over a 3-year period in an adult with CF, obesity and diabetes treated with Semaglutide and Elexacaftor/Tezacaftor/Ivacaftor (ETI). Our case experienced subs

Central Modulation of Energy Homeostasis and Cognitive Performance After Bariatric Surgery.

In moderately or morbidly obese patients, bariatric surgery has been proven to be an effective therapeutic approach to control body weight and comorbidities. Surgery-mediated modulation of brain function via modified postoperative secretion of gut peptides and vagal nerve stimulation was identified as an underlying mechanism in weight loss and improvement of weight-related diseases. Increased basal and postprandial plasma levels of gastrointestinal hormones like glucagon-like peptide 1 and peptide YY that act on specific areas of the hypothalamus to reduce food intake, either directly or media

Should obesity be a limiting factor for clozapine treatment?

OBJECTIVE: Clozapine is the first choice in drug-resistant schizophrenia but also causes important weight changes. This might discourage clinicians concerned about the risk of developing health problems. To assess this issue we measured change in body mass index (cBMI) induced by clozapine at 18 and 56 weeks. METHODS: Baseline body weight and height were measured and weight weekly thereafter during the first 18 weeks of treatment. After that, measurements were made monthly. Steady clozapine dose, clozapine and norclozapine blood concentrations, concomitant medication, gender and age were reco

Effect of the phosphodiesterase 4 inhibitor apremilast on cardiometabolic outcomes in psoriatic disease-results of the Immune Metabolic Associations in Psoriatic Arthritis study.

OBJECTIVES: Studies have suggested phosphodiesterase 4 (PDE4) inhibition may be associated with weight loss and other cardiometabolic benefits. We evaluated the effect of the PDE4 inhibitor apremilast on body weight and composition, glucose homeostasis, lipid profiles and vascular function in psoriatic disease and whether weight change correlated with therapeutic response. METHODS: We conducted a prospective, open-label study (Immune Metabolic Associations in Psoriatic Arthritis) of adults receiving apremilast 30 mg as part of routine care for PsA and/or psoriasis. Overweight and changes in weight throughout adult life in breast cancer etiology. A case-control study.

Numerous epidemiologic studies have found body size to be a significant risk factor in the etiology of breast cancer. In an Israeli study population of 1,065 breast cancer patients, 964 surgical controls, and 981 neighborhood controls, height and weight at three periods (age 18, "most of adult life," and recent) were ascertained. The authors analyzed these parameters and body mass index (weight/height) for each period, as well as body mass index changes throughout life, controlling for age, menstrual status, and ethnic origin. Odds ratios were determined for three body mass index categories: 1

Body weight and waist circumference are differentially associated with the response to L-thyroxine treatment in primary hypothyroidism.

BACKGROUND: Thyroid hormones (TH) and neurotransmitter orexin (ORX) are implicated in the regulation of metabolism. Abdominal weight gain is common in primary hypothyroidism (PH). METHODS: Our aim was to investigate whether TH affected peripheral weight gain, waist circumference (WC) and low-density lipoprotein cholesterol (LDL-C), before and 6 months after L-thyroxine substitution therapy. A secondary aim was to investigate the role of ORX. RESULTS: Weight gain was positively correlated with improvement in QoL (r = 0.72, p = 0.003) and with CSF ORX levels in the 15 included patients (r = 0.

Predictors of axial and peripheral bone mineral density in healthy children and adolescents, with special attention to the role of puberty.

The role of various physical and lifestyle factors in determining axial bone mineral density (BMD) at the lumbar segment of the spine, as measured by dual-photon absorptiometry, and peripheral BMD at the distal third of the radius, as measured by single-photon absorptiometry, was assessed in 299 healthy white children of both sexes, aged 6 to 18 years. The BMD measurements were correlated with age, height, weight, body mass index, and pubertal status. Peripheral and axial BMD were highly correlated with age, height, weight, and pubertal stage, and more weakly with body mass index. Approximatel

Managing adolescents with eating disorders.

Anorexia nervosa is a complex psychiatric disorder with endocrinologic manifestations primarily affecting adolescent females. The classic triad of presenting symptoms is weight loss in excess of 15% of ideal body weight, behavioral changes and amenorrhea (secondary or primary). The menstrual irregularities may cause the patient or family to seek gynecologic consultation before the diagnosis of primary psychiatric disorder has been made. Bulimia is a separate disease entity characterized by compulsive overeating binges followed by compensatory purging behavior to maintain a desired weight. Depe
