A large animal model is needed to study artificial lung attachment in a setting simulating chronic lung disease with significant pulmonary hypertension (PH). Heart transplantation should follow the implantation of a left ventricular assist device (LVAD) only after optimal postoperative recovery of pulmonary function. We reviewed hospital records of 12 patients who underwent extended (greater than 30 days) left ventricular support before transplantation to determine the rate of return of pulmonary function. In an open prospective study, adrenaline administration in ten patients with eleven episodes of septic shock was studied. Appropriate supportive therapy (antibiotics, laparotomies, parenteral alimentation, ventilation) was given as needed. Direct assessment of glomerular hemodynamics revealed that elevation of loop of Henle flow induces a significant increase of afferent arteriolar resistance. Examination of glomerular poles of normo- and hyperperfused nephrons by light microscopy supports this result by demonstrating significantly narrowed luminal diameters of afferent arterioles in the hyperperfused tubules.
