Ineraid cochlear implant in the ossified cochlea: surgical techniques and results.

Extensive ossification of the cochlea is a common finding in patients with total deafness caused by meningitis, labyrinthitis, or otosclerosis. When the cochlea is totally ossified, the prognosis for achieving free-running speech without lip reading is poor. However, subtotal ossification with residual cochlear patency in the upper basal turn, the middle turn, or the apical turn can be maximally exploited by selective insertion of electrodes. Results can be surprisingly adequate in spite of the poor anatomic conditions. Computed tomography and exploratory cochleotomy, in which electrically evo

Dynamic range of the contralateral stapedius reflex in cochlear implant patients.

The dynamic range of the contralateral Stapedius reflex elicited by analog electrostimulation via cochlear implant was investigated in 8 subjects supplied with a Vienna prosthesis. The study comprised patients fitted with intracochlear devices as well as patients with extracochlear devices. Sinusoidal bursts of 125, 500, 1,000 and 2,000 Hz were used as stimulation signals. Only in one patient was saturation of reflex amplitude observed. The dynamic range of the Stapedius reflex was defined either by the difference in stimulus level for reflex saturation or individual uncomfortable loudness lev

A young meningitically deaf child with a cochlear implant: a case study.

This study describes the development of the auditory, cognitive, linguistic, and social domains of a child deafened by meningitis at 20 mont … ation. The child regressed in all areas evaluated following his illness and subsequent deafness and cochlear implantation. At about one year after implantation, his social and cognitive skills began to improve. By two years after implantation (chronological age (CA) = 4:6), his abilities in all areas except auditory reception and speech had progressed to about the 4:0 …

Galvanic disruption of vestibulospinal postural control by cochlear implant devices.

All subjects with implanted cochlear stimulators demonstrated evidence of abnormal postural stability without their stimulators activated. Instability increased when they were tested with cochlear stimulation units turned on, and additional instability was demonstrated in four of these subjects when tested in noise. These findings suggest that the electrical stimulation delivered by the cochlear prosthesis is not limited to the auditory system. The precise characteristics of electrical stimulation devices designed for stimulations limited to the cochlea and their spurious effects upon motor pe

Optical cochlear implant: evaluation of insertion forces of optical fibres in a cochlear model and of traumata in human temporal bones.

Optical stimulation for hearing restoration is developing as an alternative therapy to electrical stimulation. For a more frequency-specific activation of the auditory system, light-guiding fibres need to be inserted into the coiled cochlea. To enable insertion with minimal trauma, glass fibres embedded in silicone were used as models. Thus, glass fibres of varying core/cladding diameter with and without silicon coating (single as well as in bundles) were inserted into a human scala tympani (ST) model. Insertion cochlear model force measurements were performed, and the thinner glass fibres tha

Cochlear Implant Training in Doctor of Audiology Programs: A Survey Study.

PURPOSE: As the population of cochlear implant (CI) candidates and recipients grows, there is a need for more audiologists to meet their clinical needs. Little information exists regarding the perception of recent doctorate of audiology (AuD) graduates and the training they received related to CI. METHOD: We surveyed audiologists who graduated from an accredited AuD program between 2017 and 2023 and asked them several questions about their level of satisfaction regarding their AuD studies. The survey included questions about current work setting, areas of rotation available, and instructor ch

Frequency-to-Place Mismatch and Cochlear Implant Outcomes by Electrode Type.

IMPORTANCE: Speech recognition outcomes with a cochlear implant (CI) are highly variable. One factor suggested to correlate with CI-aided speech recognition is frequency-to-place mismatch, or the discrepancy between the natural tonotopic organization of the cochlea and the electric frequency allocation of the CI electrodes within the patient's cochlea. OBJECTIVE: To evaluate the association between frequency-to-place mismatch and speech recognition outcomes in a large cohort of postlingually deafened adult CI users, while controlling for various clinical factors known to be associated with th

Is Pupil Response to Speech and Music in Toddlers with Cochlear Implants Asymmetric?

Background: Ear advantage (EA) reflects hemispheric asymmetries in auditory processing. While a right-ear advantage (REA) for speech and a left-ear advantage (LEA) for music are well documented in typically developing individuals, it is unclear how these patterns manifest in young children with cochlear implants (CIs). This study investigated whether pupillometry could reveal asymmetric listening efforts in toddlers with bilateral CIs when listening to speech and music under monaural stimulation. Methods: Thirteen toddlers (mean age = 36.2 months) with early bilateral CIs participated. Pupilla
