D-Cycloserine augmentation of cognitive behavior therapy for pediatric OCD: Predictors and moderators of outcome.

BACKGROUND: Over half of children receiving cognitive behavioral therapy (CBT) for obsessive compulsive disorder (OCD) do not fully remit. To improve response rates and enhance extinction learning, d-cycloserine (DCS) has been examined as an augmenting agent of CBT. To direct children with OCD towards treatments with the highest likelihood of success, the current study evaluated the conditions under which DCS augmentation works best (i.e., moderators) and the baseline characteristics associated with outcome, irrespective of treatment type (i.e., predictors). METHODS: Data came from a two-site…

The efficacy of cognitive behavior therapy for the treatment of perinatal anxiety symptoms: A preliminary meta-analysis.

Cognitive-behavior therapy (CBT) is considered the psychological treatment of choice for anxiety disorders in the general population. However the efficacy of CBT for anxiety that occurs during the perinatal period, defined as the period from conception to 12 months post birth, is less understood. Perinatal anxiety is a complex and multifaceted problem that can affect both the pregnant women and the unborn child, as well as the wider family unit. The aim of this study was to synthesize the current empirical status of CBT for perinatal anxiety using a meta-analytic approach. Relevant articles we

A meta-analysis of third wave mindfulness-based cognitive behavioral therapies for older people.

OBJECTIVES: The aim of this study is to review the effectiveness of third wave mindfulness-based cognitive behavioral therapies (CBTs) for depressive or anxiety symptomatology in older adults across a wide range of physical and psychological conditions. METHODS: Electronic literature databases were searched for articles, and random-effects meta-analysis was conducted. RESULTS: Ten studies met the inclusion criteria, of which nine reported the efficacy of interventions on depressive symptoms and seven on anxiety symptoms. Effect-size estimates suggested that mindfulness-based CBT is moderatel

[IDEM-depression: Characteristics and evaluation of an open group that combines psychoeducation and cognitive-behavior therapy].

INTRODUCTION: Depression is a highly prevalent mental illness that is associated with high rates of morbidity and functional impairment. At the psychiatric unit of the University Hospital of Strasbourg, France, we have developed an open group that combines psychoeducation and cognitive-behavior therapy (CBT), the information, discovery, exchange and mobilization for depression group (IDEM-depression). IDEM-depression is composed of 17 thematic, structured, and independent sessions, which address different aspects of depression (i.e., rumination, pharmacological treatments). Because of its flex

Effectiveness of Internet-based cognitive-behavior therapy for social anxiety disorder in clinical psychiatry.

OBJECTIVE: Internet-based cognitive-behavioral therapy (ICBT) has received increased attention as an innovative approach to improve access to evidence-based psychological treatments. Although the efficacy of ICBT for social anxiety disorder has been established in several studies, there is limited knowledge of its effectiveness and application in clinical psychiatric care. The purpose of this study was to evaluate the effectiveness of ICBT in the treatment of social anxiety disorder and to determine the significance of patient adherence and the clinic's years of experience in delivering ICBT.

The impact of cognitive-behavior therapy for anxiety disorders on concomitant sleep disturbances: a meta-analysis.

INTRODUCTION: Sleep disturbances are present in approximately 70% of individuals with an anxiety disorder (AD). Treatments for AD may alleviate associated sleep problems, but empirical support for this view is sparse. OBJECTIVE: To assess state of knowledge about the impact of CBT for AD on sleep disturbances. METHOD: Systematic search for clinical trials of CBT for any AD in PsycINFO, MedLine, and Proquest Dissertations and Theses. RESULTS: Of 1205 studies, only 25 (2.07%) reported sleep data. The combined ES of CBT for AD on sleep was 0.527 [95%CI 0.306-0.748], indicating a moderate effec

A randomized trial of sertraline, self-administered cognitive behavior therapy, and their combination for panic disorder.

BACKGROUND: Self-administered cognitive behavior therapy (SCBT) has been shown to be an effective alternative to therapist-delivered treatment for panic disorder (PD). However, it is unknown whether combining SCBT and antidepressants can improve treatment. This trial evaluated the efficacy of SCBT and sertraline, alone or in combination, in PD. METHOD: Patients (n=251) were randomized to 12 weeks of either placebo drug, placebo drug plus SCBT, sertraline, or sertraline plus SCBT. Those who improved after 12 weeks of acute treatment received treatment for an additional 12 weeks. Outcome measur

Virtual reality cognitive behavior therapy for public speaking anxiety: a randomized clinical trial.

Public speaking anxiety (PSA) is a common phobia. Although cognitive behavior therapy (CBT) is preferred, difficulties arise with the exposure component (lack of therapist control, patient's inability to imagine, self-flooding, loss of confidentiality resulting from public exposure). Virtual reality CBT (VRCBT) enables a high degree of therapist control, thus overcoming these difficulties. This study examined whether VRCBT is an alternative to CBT. Participants with PSA were randomly assigned to VRCBT (28 participants), CBT (30 participants), and wait list control (WLC; 30 participants). VRCBT

Cognitive behavior therapy for chronic insomnia occurring within the context of medical and psychiatric disorders.

Insomnia is a pervasive problem for many patients suffering from medical and psychiatric conditions. Even when the comorbid disorders are successfully treated, insomnia often fails to remit. In addition to compromising quality of life, untreated insomnia may also aggravate and complicate recovery from the comorbid disease. Cognitive behavior therapy for insomnia (CBT-I) has an established efficacy for primary insomnia, but less is known about its efficacy for insomnia occurring in the context of medical and psychiatric conditions. The purpose of this article is to present a rationale for using

Imagery rescripting within cognitive behavior therapy for bulimia nervosa: an illustrative case report.

OBJECTIVE: This case report describes the use of imagery rescripting as an adjunct to cognitive behavior therapy (CBT) for bulimia nervosa. METHODS: The patient was a 22-year-old woman, who had a 6-year history of diagnosable eating problems, with no comorbid diagnosis. She had a history of emotional abuse by her family. The single session of imagery rescripting was aimed at effecting change in the core beliefs that were hypothesized to result from that emotional abuse, and hence in the associated bulimic psychopathology. RESULTS: While eight sessions of conventional CBT reduced symptom beha

The role of expressive suppression and cognitive reappraisal in cognitive behavioral therapy for social anxiety disorder: A study of self-report, subjective, and electrocortical measures.

BACKGROUND: Contemporary models of cognitive behavioral therapy (CBT) for social anxiety disorder (SAD) emphasize emotion dysregulation as a core impairment whose reduction may play a causal role in psychotherapy. The current study examined changes in use of emotion regulation strategies as possible mechanisms of change in CBT for SAD. Specifically, we examined changes in expressive suppression and cognitive reappraisal during CBT and whether these changes predict treatment outcome. METHODS: Patients (n = 34; 13 females; Mean age = 28.36 (6.97)) were allocated to 16-20 sessions of CBT. An ele…

Effectiveness of internet-based cognitive behavioral therapy with virtual reality exposure therapy for social anxiety disorder: A randomized controlled trial in Hong Kong.

BACKGROUND: Social anxiety disorder (SAD) is a common but under-treated mental health condition. Internet-based cognitive behavioral therapy (iCBT) and virtual reality exposure therapy (VRET) are effective treatment approaches for SAD. However, few studies have integrated both, particularly in Chinese communities. This study examines the effectiveness of a 14-week iCBT program that includes VRET, called "Ease Anxiety in Social Event Online" (Ease Online), among Hong Kong adults with SAD. METHOD: 329 Hong Kong Chinese adults with SAD were randomized into web-based iCBT with VRET (n = 117), app…

Group schema therapy versus group cognitive behavioral therapy for patients with social anxiety disorder and comorbid avoidant personality disorder: A randomized controlled trial.

BACKGROUND: Patients with social anxiety (SAD) and comorbid avoidant personality disorder (AVPD) are severely impaired. Group cognitive behavioral therapy (GCBT) is considered an effective treatment for SAD. More knowledge on treatment of SAD with comorbid AVPD is needed. Schema therapy, developed for personality and chronic mental disorders, may be a promising treatment. METHODS: We conducted a randomized controlled trial in an outpatient population (n = 154) with both SAD and AVPD. Group Schema Therapy (GST) and GCBT were compared on SAD symptoms (Liebowitz Social Anxiety Scale) and manifes…

Internet-based cognitive behavioral therapy for anxiety in an outpatient specialized care setting: A qualitative study of the patients' experience of the therapy.

Knowledge about user experiences of internet-based cognitive behavioral therapy (iCBT) has mostly been drawn from non-clinical groups or with iCBT offered via self-referral. The present study therefore focused on patients who had undergone iCBT with minimal support while actively awaiting outpatient psychological treatment in the form of face-to-face CBT. To seek out barriers to adherence the study also included patients who had withdrawn from the iCBT treatment before completion. The study was performed in an outpatient clinic for anxiety disorders where twelve participants with a primary dia…

Matching adolescents with a cannabis use disorder to multidimensional family therapy or cognitive behavioral therapy: treatment effect moderators in a randomized controlled trial.

BACKGROUND: In a recent randomized controlled trial (Hendriks et al., 2011), multidimensional family therapy (MDFT) and cognitive behavioral therapy (CBT) were equally effective in reducing cannabis use in adolescents (13-18 years old) with a cannabis use disorder (n=109). In a secondary analysis of the trial data, we investigated which pretreatment patient characteristics differentially predicted treatment effect in MDFT and CBT, in order to generate hypotheses for future patient-treatment matching. METHODS: The predictive value of twenty patient characteristics, in the area of demographic b…
