This case involved a 76-year-old man. Total colonoscopy was performed as a second examination for colorectal cancer because of positive fecal occult blood results, revealing a neoplasm with ulceration in the sigmoid colon. We suspected type-3 colorectal cancer and performed a biopsy, but the biopsy diagnosis showed only an ulcer with active inflammation. BACKGROUND: Upon initial presentation, 25%-30% of patients with renal cell carcinoma have metastatic disease. Metastasis to the gastrointestinal tract is rare, but when it occurs, the large bowel is the least common site.
CASE REPORT: A 75-year-old white male presented with vague abdominal symptoms and worsening hemoglobin and hematocrit levels. Colonoscopy revealed a polyp in the distal portion of the sigmoid colon. Histologic and immunohistochemical analysis suggested metastatic papillary renal cell carcinoma.

BACKGROUND: Colonic obstruction is a relatively common condition in emergency care, with a mortality rate of up to 20%. In 90% of cases it results from colonic or rectal adenocarcinoma, volvulus, or stenosis secondary to diverticular disease. When fecal impaction is the underlying cause, the condition is usually managed conservatively, but may on occasion become complicated and even require surgical intervention. Based on the proven efficacy of Coca-Cola® to dissolve gastric phytobezoars, we report a case of colonic obstruction secondary to sigmoid fecaloma.

An 88-year-old man with unresectable sigmoid colon cancer due to retroperitoneal invasion and massive LN metastasis was treated with S-1 after colostomy. S-1 (80 mg/body/day) was administered for 2 weeks followed by a one week interval. Colonoscopy and abdominal CT revealed a partial response. After 5 courses, the patient underwent sigmoidectomy. S-1 was then administered for 15 courses. There was no side effect, and hospitalization was not required during the treatment.
