Study of naïve and switched memory B cell level in Egyptian patients with common variable immunodeficiency.

Common variable immunodeficiency (CVID) is one of the primary immunodeficiency disorders. The phenotype of peripheral blood memory B cells is a useful tool in the classification of patients into clinically and functionally relevant groups. This study aimed to assess the level of naïve and switched memory B cells level and their correlation with the clinical phenotypes and complications in patients with CVID. This case control study included 30 adult patients with CVID and 30 normal controls, matched for age and sex. Complete blood count, cluster of differentiation 3 (CD3)+, CD4+, CD8+ T cells …

Clinical and immunological differentiation of isolated IgG and combined IgG & IgM deficiencies from common variable immunodeficiency.

OBJECTIVE: To assess the clinical relevance of diagnosing and classifying isolated IgG deficiency and combined IgG/IgM deficiency separately from CVID. METHODS: In a retrospective cohort of patients with primary hypogammaglobulinemia, we evaluated and compared the clinical spectrum and immunological findings of patients with CVID, isolated IgG deficiency, and combined IgG/IgM deficiency. RESULTS: In comparison to CVID, respiratory tract infections and gastrointestinal infections were less common in isolated IgG or combined IgG/IgM deficiency, while recurrent mucocutaneous herpes simplex viru

Granulomatous-Lymphocytic Interstitial Lung Disease in Common Variable Immunodeficiency with a Rapid Progression and Refractory Infections.

A 34-year-old woman with common variable immunodeficiency (CVID) developed pulmonary abnormalities despite adequate subcutaneous immunoglobulin replacement and long-term low-dose erythromycin therapy. Her lung lesions and hypoxemia progressed, and a surgical biopsy confirmed granulomatous-lymphocytic interstitial lung disease (GLILD). Prednisolone and cyclosporine were administered. However, her condition worsened, and she died of infectious complications. Although patients with CVID-GLILD often respond to immunoglobulin replacement therapy or immunosuppression, this case was rapidly progressi…

Clinical and immunological factors associated with gastrointestinal involvement in common variable immunodeficiency: a real-world study.

BACKGROUND: Gastrointestinal (GI) involvement is a common and clinically significant manifestation of common variable immunodeficiency (CVID); however, its relationship with immunological parameters remains incompletely understood. OBJECTIVE: This study aimed to evaluate the association between GI involvement and immunological markers, particularly serum immunoglobulin levels and B-cell subsets, and to assess their discriminative performance. METHODS: In this single-center retrospective study, 109 adult patients with CVID were included and categorized based on GI involvement. Clinical, endos…

Monogenic Common Variable Immunodeficiency (Mo-CVID) Score for Optimizing the Genetic Diagnosis in Pediatric CVID Cohort.

Common variable immunodeficiency (CVID) represents an "umbrella" diagnosis due to its clinical and immunological heterogeneity. The primary objective of this study was to describe a cohort of CVID pediatric subjects from clinical, immunological, and genetic viewpoints. Secondary, we propose a model for prioritizing genetic investigations in these patients. Thirty-four patients with CVID followed at Meyer Children's Hospital, IRCSS, were enrolled. Whole exome sequencing was performed according to the latest International Union of Immunological Societies 2022 update. Genetic variants were identi…

Autoimmune encephalitis and delayed diagnosis in common variable immunodeficiency: A case report and review of literature.

Common Variable Immunodeficiency (CVID) is a primary immunodeficiency disorder that often presents with recurrent infections and autoimmune complications, leading to diagnostic delays. This case report presents two adult male patients, ages 44 and 42, with prolonged histories of recurrent pneumonia, sinusitis, meningitis, and autoimmune encephalitis. Both cases were initially mismanaged due to the isolated treatment of symptoms, delaying the diagnosis of CVID. Comprehensive diagnostic evaluations eventually revealed low immunoglobulin levels, confirming the diagnosis. These cases highlight the…

Comparative analysis of demographic and clinical features in common variable immunodeficiency and selective immunoglobulin G deficiency.

Background: Common variable immunodeficiency (CVID) and selective immunoglobulin G (IgG) deficiency (sIgGD) are primary antibody deficiencies with divergent clinical trajectories. CVID is characterized by recurrent infections and systemic noninfectious complications, whereas sIgGD typically follows a milder course. Despite sharing clinical features such as recurrent infections, comparative data on complications and mortality remain limited, which hinders risk-stratified management. Objective: This study compared the prevalence, clinical impact, and mortality associated with noninfectious compl…

A predictive model for identification of pediatric individuals with common variable immunodeficiency through electronic medical records.

INTRODUCTION: Common variable immunodeficiency (CVID) is characterized by recurrent sinopulmonary infections. However, in the pediatric population, recurrent sinopulmonary infections early in life are common, which can render key clinical features of CVID less distinctive. Accordingly, the diagnosis of CVID is often delayed owing to the heterogeneous nature of the presentation and the broad range of ages of onset. A 10-year lag in diagnosis has been found for CVID, and there is a critical need for improved time to diagnosis. OBJECTIVE: Our aim was to utilize machine learning techniques to ide…

Co-occurrence of Alpha-1 Antitrypsin Deficiency (AATD) and Common Variable Immunodeficiency (CVID): A Case Report.

Alpha-1 antitrypsin deficiency (AATD) and common variable immunodeficiency (CVID) are distinct disorders, and their co-occurrence is rare. We report a 47-year-old man with a history of smoking and chronic obstructive pulmonary disease (COPD), diagnosed with the PI*MZ genotype of AATD and treated with human alpha1-proteinase inhibitor (PROLASTIN-C®, Grifols Therapeutics, Clayton, NC, USA). Despite therapy, he experienced recurrent respiratory infections and pneumonia. Further evaluation revealed hypogammaglobulinemia and immune dysregulation consistent with CVID, leading to bronchiectasis. Comb…

Follow-up of immune response in patients with common variable immunodeficiency following SARS-CoV-2 vaccination.

The COVID-19 pandemic highlighted the importance of effective vaccination strategies in controlling the spread of infectious diseases. SARS-CoV-2 vaccine has demonstrated high efficacy in preventing COVID-19 infection in the general population. However, the efficacy of this vaccine in patients with predominantly antibody deficiencies, such as common variable immunodeficiency (CVID) and X-linked agammaglobulinemia (XLA), should be closely monitored. CVID and XLA are rare genetic disorders that impair the immune system's ability to produce antibodies, which are crucial for fighting infections. P…

Phenotype and oxidative burst of low-density neutrophil subpopulations are altered in common variable immunodeficiency patients.

Common variable immunodeficiency disorder (CVID) is the most common form of primary antibody immunodeficiency. Due to low antibody levels, CVID patients receive intravenous or subcutaneous immunoglobulin replacement therapy as treatment. CVID is associated with the chronic activation of granulocytes, including an increased percentage of low-density neutrophils (LDNs). In this study, we examined changes in the percentage of LDNs and the expression of their surface markers in 25 patients with CVID and 27 healthy donors (HD) after in vitro stimulation of whole blood using IVIg. An oxidative burst…

Case report of heart transplantation for giant cell myocarditis in a patient with common variable immunodeficiency.

BACKGROUND: Solid-organ transplantation in patients with common variable immunodeficiency (CVID) is controversial due to the risk for severe and recurrent infections. Determining transplantation candidacy in CVID patients is further complicated by the presence of CVID-related non-infectious complications that can reduce overall survival and also recur in the transplanted organ. Data regarding solid organ transplantation in patients with CVID are limited, particularly in heart transplantation. CASE SUMMARY: A 32-year-old female with CVID presented with new heart failure after 3 months of dyspn…

Common variable immunodeficiency with granulomatous-lymphocytic interstitial lung disease and preceding neurological involvement: a case-report.

BACKGROUND: Common variable immunodeficiency (CVID) is a group of heterogeneous primary immunodeficiencies characterised by a dysregulated and impaired immune response. In addition to an increased susceptibility to infection, it is also associated with noninfectious autoimmune and lymphoproliferative complications. CVID is rarely associated with neurological complications. Pulmonary involvement is more common, and patients can develop an interstitial lung disease known as granulomatous-lymphocytic interstitial lung disease (GLILD). CASE PRESENTATION: A 50-year-old Caucasian female with a hist…

Liver stiffness assessment by transient elastography suggests high prevalence of liver involvement in common variable immunodeficiency.

BACKGROUND: Up to 50% of patients with common variable immunodeficiency (CVID) present persistently increased serum levels of liver enzymes and/or mild hepatomegaly. Ultrasound-based transient elastography (TE) is largely used for early detection of the progression of chronic liver diseases, but has never been employed in CVID. We performed a cross-sectional study to evaluate TE values in a cohort of adult CVID-patients. METHODS: Full blood count, liver function test, liver and spleen sonogram and ultrasound-based TE were performed in 77 adult CVID patients. Demographic and clinical data were…

Fatal Enteroviral Encephalitis in a Patient with Common Variable Immunodeficiency Harbouring a Novel Mutation in NFKB2.

Common variable immunodeficiency is the most prevalent of the primary immunodeficiency diseases, yet its pathogenesis is largely poorly understood. Of the cases that are monogenic, many arise due to pathogenic variants in NFKB1 and NFKB2. Here, we report enteroviral encephalomyelitis as the cause of a fatal neurodegenerative condition in a patient with a novel heterozygous mutation in NFKB2 (c.2543insG, p.P850Sfs36*) that disrupts non-canonical NF-κB signaling. Investigations of primary and secondary lymphoid tissue demonstrated a complete absence of B cells and germinal centers. Despite multi…
