Citizenship and Community Mental Health Care.

Citizenship is an approach to supporting the social inclusion and participation in society of people with mental illnesses. It is receiving greater attention in community mental health discourse and literature in parallel with increased awareness of social determinants of health and concern over the continued marginalization of persons with mental illness in the United States. In this article, we review the definition and principles of our citizenship framework with attention to social participation and access to resources as well as rights and responsibilities that society confers on its memb

The social context of reproductive health in an Egyptian hamlet: a pilot study to identify female genital schistosomiasis.

This pilot study is the first to identify female genital schistosomiasis (FGS) in an Egyptian community setting. The year-long interdisciplinary study, in a small hamlet (ezba), combined clinical assessment with an in-depth study of the social context of reproductive health. Schistosoma haematobium ova were found in 16.7% of women in the study (21/126). Half of the women who agreed to a full gynecological examination (43 of 86) had evidence of reproductive morbidity due to schistosomiasis, either schistosome eggs in the cervix or sandy patches, tissue changes in the reproductive tract. Other r

Sobering thoughts: town Hall meetings on fetal alcohol spectrum disorders.

Prenatal exposure to alcohol is one of the leading causes of preventable birth defects and developmental disabilities. During the past 30 years, fetal alcohol spectrum disorders (FASD), including fetal alcohol syndrome, have gradually begun to attract attention. However, awareness and understanding of the disorders remain low, and people who are affected are seriously underserved. The FASD Center for Excellence held a series of town hall meetings in 2002 and 2003 to gauge the issues surrounding FASD nationwide. On the basis of its findings, the center proposed a series of recommendations to be

Effect of occupational therapy home visit discharge planning on participation after stroke: protocol for the HOME Rehab trial.

INTRODUCTION: After first stroke, the transition from rehabilitation to home can be confronting and fraught with challenges. Although stroke clinical practice guidelines recommend predischarge occupational therapy home visits to ensure safe discharge and provision of appropriate equipment, there is currently limited evidence to support this recommendation. METHODS AND ANALYSIS: The HOME Rehab trial is a national, multicentre, phase III randomised controlled trial with concealed allocation, blinded assessment and intention-to-treat analysis being conducted in Australia. The trial aim is to det

Community-based HIV testing services in an urban setting in western Kenya: a programme implementation study.

BACKGROUND: Some countries are struggling to reach the UNAIDS target of 90% of all individuals with HIV knowing their HIV status, especially among men and youth. To identify individuals who are unaware of their HIV-positive status and achieve testing saturation, we implemented a hybrid HIV testing approach in an urban informal settlement in western Kenya. In this study, we aimed to describe the uptake of HIV testing and linkage to care and treatment during this programme. METHODS: The Community Health Initiative involved community mapping, household census, multidisease community health campa

Utilization of mobile phones for accessing menstrual regulation services among low-income women in Bangladesh: a qualitative analysis.

BACKGROUND: As many as one-third of all pregnancies in Bangladesh are unplanned, with nearly one-half of these pregnancies ending in either menstrual regulation (MR) or illegal clandestine abortion. Although MR is provided free of charge, or at a nominal cost, through the public sector and various non-profits organizations, many women face barriers in accessing safe, affordable MR and post-MR care. Mobile health (mHealth) services present a promising platform for increasing access to MR among low-income women at risk for clandestine abortion. We sought to investigate the knowledge, attitudes a

Family planning for teens: strategies for improving outreach and service delivery in public health settings.

The persistent underuse of family planning services by inner-city, low-income, sexually active youth underscores the importance of testing innovative programs that maximize participation. Presented in this paper is an analysis of a Chicago public health clinic's special program for adolescents that originated from the staff's observations of the scheduling, educational, and support needs of teens seeking family planning services from a traditionally managed public health facility. Between December 1982, when the special program--the Teen Clinic--was implemented, and March 1985, more than 600 a

Public health preparedness and response synergies between institutional authorities and the community: a qualitative case study of emerging tick-borne diseases in Spain and the Netherlands.

BACKGROUND: Communities affected by infectious disease outbreaks are increasingly recognised as partners with a significant role to play during public health emergencies. This paper reports on a qualitative case study of the interactions between affected communities and public health institutions prior to, during, and after two emerging tick-borne disease events in 2016: Crimean-Congo Haemorrhagic Fever in Spain, and Tick-Borne Encephalitis in the Netherlands. The aim of the paper is to identify pre-existing and emergent synergies between communities and authorities, and to highlight areas whe

Multiple Sclerosis and COVID-19: Health and healthcare access, health information and consumer co-created strategies for future access at times of crisis.

BACKGROUND: Persons with multiple sclerosis (MS) need regular access to medical services for optimal health outcomes. During the COVID-19 crisis, evidence indicated some detrimental health changes in persons with MS. Maintaining access to healthcare providers and healthcare information may minimise detrimental health changes during times of crisis. In Australia, there is limited evidence of consultation with people who have chronic health conditions or disabilities regarding government decisions to restrict healthcare access and in the provision of health guidance during the COVID-19 crisis. Health care cost and access challenges persist: initial findings from HSC's 2007 site visits.

Little has changed in local health care markets since 2005 to break the cycle of rising costs, falling insurance coverage and widening access inequities, according to initial findings from the Center for Studying Health System Change's (HSC) 2007 site visits to 12 nationally representative metropolitan communities. As intense competition among hospitals and physicians for profitable specialty services continues, employers and health plans are looking to consumers to take more responsibility for medical costs, lifestyle choices and treatment decisions. While consumer-directed health plans have

Developing a community-prioritized research agenda on primary care strategies for minimizing the health impacts of heat and poor air quality events: protocol.

INTRODUCTION: Extreme heat / poor air quality events can adversely impact health, and low-income communities are especially vulnerable to these impacts. Evidence-based interventions that minimize these impacts are needed and ideally will be developed and led by front-line organizations such as primary care community health centers (CHCs). A community-driven and -prioritized research agenda is necessary to guide the generation of evidence that CHCs need to intervene effectively. We will generate this agenda via the following protocol. METHODS: We will first describe patterns of heat- and air q

The Perfect Storm: Perceptions of Influencing Adults Regarding Latino Teen Pregnancy in Rural Communities.

INTRODUCTION: Recognition of the importance of adolescents' environments in influencing their sexual and reproductive health (SRH) decision-making necessitates a deeper understanding of the role that community stakeholders play in shaping Adolescent and Young Adults (AYA) access to SRH education and care. We describe community stakeholders' knowledge, beliefs, and attitudes about AYA's SRH needs in three rural Latino communities in Kansas. METHODS: Key stakeholders completed a written survey incorporating the theory of Planned Behavior to assess attitudes, norms, and intentions to support AYA

Extreme heat preparedness and response implementation: a qualitative study of barriers, facilitators, and needs among local health jurisdictions in the United States.

Extreme heat events (EHEs) are the deadliest weather hazards in the United States (U.S.). Local health jurisdictions (LHJs) in the U.S. are frontline responders during EHEs and other public health emergencies. This study aims to clarify the factors influencing EHE preparedness and response implementation. From January to March 2023, we conducted and thematically analyzed four focus group discussions with 17 representatives from U.S. LHJs. The Consolidated Framework for Implementation Research was used to guide the discussion. Participants described barriers, facilitators, and needs surrounding

Long term care planning for cancer survivors: a health services research agenda.

INTRODUCTION: The recent Institute of Medicine report From Cancer Patient to Cancer Survivor: Lost in Transition recommended that 'survivorship care plans' be created for patients as they complete primary therapy for cancer in order to ensure clarity for all involved about patients' diagnoses, treatment received, and plan for surveillance. It should explicitly identify the providers responsible for each aspect of ongoing care, and provide information on resources available for psychosocial and other practical issues that may arise as a result of the prior cancer diagnosis. The IOM stated that
