Thoracoscopic repair of chronic post-traumatic diaphragmatic hernia: outcomes and selection criteria.

BACKGROUND: Chronic post-traumatic diaphragmatic hernia is a delayed consequence of missed diaphragmatic injury that presents months to years after trauma. It develops in a stable anatomical setting with mature adhesions and organ migration, creating operative conditions distinct from emergency management of acute rupture. OBJECTIVE: To evaluate the feasibility and perioperative outcomes of video-assisted thoracoscopic surgery for chronic diaphragmatic hernia and to identify anatomical factors that guide selection between minimally invasive and open repair. METHODS: From January 2010 to May

Innovative approach to open abdomen: converting an enteroatmospheric fistula into an easily manageable stoma.

A 52-year-old super morbidly obese patient underwent elective laparoscopic conversion of a failed silastic ring vertical gastroplasty to a Roux-en-Y gastric bypass. Following surgery, she developed an anastomotic leak which required emergent laparotomy. The patient then suffered from a complex postoperative course during which she developed an intestinal fistula which freely drained into the wound and gradually led to its complete dehiscence. Her course was further complicated by the surfacing of an enteroatmospheric fistula. This devastating complication was managed by employing the TopClosur

Robotic Colonic Surgery in Elderly and Octogenarian Patients: Chronological Age Alone Does Not Predict Severe Complications.

INTRODUCTION: The role of robotic colonic surgery in very elderly patients remains debated, and chronological age is often considered a limi … e between the groups. No differences were observed in operative time, length of stay, or mortality. Conversion to open surgery independently predicted severe complications; chronological age was not associated with increased risk. CONCLUSIONS: Robotic colonic surgery is safe in selected elderly patients. Age alone should not contraindicate robotic surgery.

Detection of evolving injury to the brachial plexus during transaxillary robotic thyroidectomy.

OBJECTIVES/HYPOTHESIS: Continuous intraoperative neuromonitoring (IONM) of transcranial electric motor evoked potentials (tceMEPs) and somatosensory evoked potentials (SSEPs) has gained universal acceptance as an efficacious method for detecting emerging positional brachial plexopathy or peripheral nerve compression during spinal and shoulder surgery. This has implications for transaxillary thyroid surgery. STUDY DESIGN: Case report with literature review. METHODS: The patient underwent robotic transaxillary thyroid surgery with continuous tceMEP and SSEP monitoring of brachial plexus functi

A Potential Role for Robotic Cholecystectomy in Patients with Advanced Liver Disease: Analysis of the NSQIP Database.

Robotic surgery has been widely adopted by many specialties, including hepatobiliary surgery. However, robotic procedures generally require longer operative times and are costlier than their laparoscopic counterparts. The role for robotic cholecystectomy (RC), particularly in patients with advanced liver disease, has not been established. A retrospective analysis of the NSQIP database was performed, focusing on patients with chronic liver disease who underwent cholecystectomy. Patients were categorized based on their model for end-stage liver disease (MELD) score and the type of surgical proce

Outcomes of Transcatheter Aortic Valve Replacement in Patients Treated With Systemic Steroids.

BACKGROUND: Chronic steroid therapy is associated with higher vascular complication rates in patients undergoing transcatheter aortic valve replacement (TAVR). The effect of corticosteroids on aortic annular complications has not been directly assessed in this population. METHODS: A retrospective analysis of 1095 patients undergoing transfemoral TAVR was performed. Patients treated with chronic steroids at the time of the procedure (n = 99) were compared with those who received no steroids (n = 992). The primary outcome included a composite of aortic annular complications, defined as a combin

Comparative outcomes of laparoscopic and open surgery in inflammatory bowel disease in pediatric and young adult patients: a systematic review and meta-analysis.

BACKGROUND: Pediatric inflammatory bowel disease (IBD), encompassing Crohn's disease, ulcerative colitis, and indeterminate colitis, often necessitates surgical intervention in cases of severe or refractory disease. Although biologic therapies have significantly reduced the need for surgery, operative management remains essential for certain patients. The choice between laparoscopic (laparoscopy group [LG]) and open conventional surgery (open group [OG]) continues to be a subject of debate. This meta-analysis aimed to compare the postoperative outcomes of LG and OG in pediatric patients with I
