These evidence-based guidelines reflect the evolving surgical management of degenerative lumbar spondylolisthesis, grounded in high-quality data that help clarify longstanding topics of contention. Key topics discussed include operative versus nonoperative management, decompression alone versus decompression with fusion, instrumented versus noninstrumented fusion, fusion with reduction versus fusion in situ, minimally invasive and endoscopic surgery, the use of interbody devices and biologics for fusion, robot-assisted surgery, intraoperative CT-based navigation, single-position surgery, enhan

In this study, the authors summarize the current knowledge on epidemiology, demographics, risk factors, and prognostic factors that influence outcomes in patients with adult Chiari malformation type I (CM-I) who underwent posterior fossa decompression surgery with duraplasty. Furthermore, they describe the contribution of their research group to the field of adult CM-I treatment, including association of increased body mass index with severity of CM-I and syringomyelia, relevant surgical anatomy, and surgical technique of 270° microsurgical decompression of foramen magnum. The authors also rep

The authors present a case of a 45-year-old man with recurrent trigeminal neuralgia twelve years after microvascular decompression. The patient underwent surgical reexploration of the trigeminal nerve. A piece of hardened Teflon felt was found piercing a hole through the trigeminal nerve. No other compression of the nerve was found. Removal of the Teflon felt resolved the neuralgia. To the authors knowledge this is the first report of recurrent trigeminal neuralgia due to such a mechanism. This case emphasises the need for further improvement of the surgical technique for microvascular decompr

BACKGROUND: Periodontoid synovial cysts are rare lesions which may produce symptomatic cervico-medullary compression.
METHOD: We report such a patient, whose progressive neurological deterioration required surgical treatment by transoral odontoidectomy and decompression.
RESULTS: The diagnostic and theraputic interventions are described, including a lumbar puncture which precipitated a transient loss of consciousness and respiratory arrest. Surgery achieved clinical improvement, without complications or need for operative stablization. Detailed neuropathology is presented, as well as a liter

We describe three patients with acute chiasmal visual field loss caused by hemorrhage within an intrachiasmal arteriovenous malformation. One patient had five episodes; vision improved three times without treatment and twice after surgical decompression. Surgical intervention restored vision in two of the three patients. The role for surgery is indefinite, but may be indicated if the visual defect remains static for 1 week or progresses. Decompression is likely to be most effective if done early. The approach to the optic chiasm should be transcranial, not transsphenoidal. A radiologically nor

INTRODUCTION: Spinal epidural hematoma is a rare condition that most commonly occurs as a complication of spinal surgery. For patients with neurological deficits, surgical decompression can generally provide good outcome.
CASE: A 56-year-old, otherwise healthy, patient was admitted to the orthopedic emergency department with a pelvic ring fracture. Over the course of 4 days, a lumbar spinal epidural hematoma developed, with the patient complaining of pain radiating to the S1 dermatome and saddle paresthesia. The hematoma was surgically decompressed, and the patient had a complete recovery.

PURPOSE: Adding instrumented spondylodesis to decompression in symptomatic spinal stenosis with degenerative spondylolisthesis is subject of debate. The presence of spondylolisthesis due to degeneration is an indicator of severe facet joint and intervertebral disc degeneration, and this may fit increased instability of the spine. We aim to establish the incidence of degenerative spondylolisthesis in spinal stenosis surgical candidates and to evaluate the incidence of failure of decompressive surgery without concomitant spondylodesis as initial treatment.
METHODS: Medical files of all operated

PURPOSE: Different scleral decompression surgical procedures have been proposed for the treatment of idiopathic and nanophthalmic uveal effusion syndrome (UES). The aim of this review is to describe the different surgical approaches reported in the literature and compare the outcomes and complications between them.
METHODS: We searched PubMed/MEDLINE, Scopus and Web of Science for all articles that reported scleral decompressive surgical procedures for idiopathic and/or nanophthalmic UES treatment, as well as publications reporting prophylactic surgeries for uveal effusion in nanophthalmic ey

Degenerative spondylolisthesis is a common pathology, often causing lumbar canal stenosis. There is, however, no strong consensus regarding the various medical and surgical treatments available. Surgery is indicated mainly for perceived functional impairment; when the indication is accepted, several questions determine the choice of surgical strategy. Improvement in neurological symptoms is one of the main treatment objectives. For this, it is useful to perform radicular decompression. Some authors recommend indirect decompression by interbody fusion (ALIF, TLIF, XLIF), others by means of an i

PURPOSE: To report patient-rated outcomes after ulnar nerve decompression at the elbow and to compare the outcome after open in situ decompression with that after endoscopic in situ decompression.
METHODS: Patients having ulnar nerve decompression were evaluated using patient-rated outcome measures. Fifty-five patients were recruited; 3 were lost to follow-up, and 18 were excluded because they had anterior transposition. Of the thirty-four patients followed up for 12 months, 19 had endoscopic decompression and 15 had open in situ decompression. Patient demographics, presenting symptoms, range

BACKGROUND: Both thoracic ossification of the posterior longitudinal ligament (T-OPLL) and thoracic disc herniation (TDH) may cause thoracic spinal stenosis (TSS) resulting in thoracic myelopathy. Surgical decompression is the only effective treatment for symptomatic TSS. 360° Circumferential decompression, also called the "cave-in" technique, can safely and effectively treat T-OPLL; however, this procedure has not yet been performed endoscopically. Herein, we report the first case of massive T-OPLL combined with TDH that was treated endoscopically using the "cave-in" technique, with 36 months

BACKGROUND: Cubital tunnel syndrome (CuTS) is a lifestyle-altering peripheral neuropathy lacking a consensus for optimal surgical management. We describe creation of a fascial "V-sling" without ulnar nerve transposition, which is associated with increased surgical morbidity compared with decompression. The purpose of this study is to evaluate a novel technique with effective ulnar nerve decompression and subluxation prevention by creating a fascial sling in patients with CuTS and ulnar nerve subluxation.
METHODS: We reviewed records of 39 elbows in 35 patients who underwent in situ ulnar nerv

BACKGROUND: At present, few reports of percutaneous endoscopic transforaminal decompression surgery have been reported to solve central lumbar spinal stenosis (CLSS). Is endoscopic decompression through bilateral transforaminal approach decompression sufficient for degenerative CLSS?
METHODS: This retrospective study included 47 cases of CLSS patients who underwent percutaneous endoscopic decompression through bilateral transforaminal approach. Clinical outcomes such as ODI, back and leg VAS, the Macnab criteria were evaluated. Surgical results including operative time, postoperative hospital

BACKGROUND AND IMPORTANCE: Acute bilateral brachial plexus injury is rare and usually a result of traction injury. Immediate operative intervention is reserved for rare cases of ongoing compression of the plexus; the role for acute decompression of the brachial plexus secondary to compartment syndrome has not been previously described. In this report, we describe the technique and role for urgent brachial plexus decompression.
CLINICAL PRESENTATION: A 32-yr-old man presented with acute complete bilateral brachial plexus palsy due to focal rhabdomyolysis and brachial plexus compression after a

INTRODUCTION: Carpal tunnel decompression is the most commonly performed surgical procedure within a hand unit. Although very effective, the complications following the procedure can be significant. In an attempt to identify the causes of such complications, we assessed whether seniority of the surgeon impacts on outcome for open carpal tunnel decompression.
PATIENTS AND METHODS: Data were jointly and prospectively gathered from two units from either side of the Atlantic - the Pulvertaft Hand Center, UK and the Curtis Hand Center, USA. The aim of the study was to assess outcome following carp

Lumbar synovial cysts are fluid-filled lesions arising from the facet joints that can cause lateral recess stenosis and neurogenic claudication. While surgical decompression is the gold standard for symptomatic cysts, patients with significant comorbidities, such as chronic obstructive pulmonary disease (COPD) and emphysema, face elevated perioperative risks under general anesthesia and delayed wound healing. This case report describes the successful application of the minimally invasive lumbar decompression (MILD) procedure as a safe alternative for a high-risk surgical candidate. A 68-year-o

BACKGROUND: Involvement of the trigeminocerebellar artery (TCA) in trigeminal neuralgia (TN) is rare, and reports of decompression using an endoscopic retro sigmoid keyhole approach are limited. This study, the largest of its kind, examines TCA-related TN cases to highlight the anatomical and surgical importance of the TCA, discuss technical difficulties in decompression, and review strategies for managing complications, along with a summary of previous cases.
METHODS: Between April and September 2024, 56 endoscopic microvascular decompression (eMVD) procedures for TN were conducted using a 0
