[Planning dental care and orthodontic treatment for children afflicted with malignant hematological diseases].

THE PROBLEM: In France, cancer affects 1 to 3% of all children and represents the second most frequent cause of mortality among children younger than fifteen. Malignant blood diseases are the most frequently occurring childhood cancers. Although their mortality rate has been tending to decrease, they often seem to develop after "opportunistic" latent infections persist, undetected, in un-eradicated sites that may be of dental origin. This infectious risk, frequently undiagnosed in hospitals, should be of concern to both general dentists and orthodontists. OBJECTIVES: General dentists and orth…

Malpractice claims for permanent nerve injuries related to third molar removals.

On the basis of the register of the Finnish Patient Insurance Association, the aim of this study was to examine malpractice claims for nerve injuries associated with third molar removals and determine whether they are concentrated among specialists, among less experienced dentists, or in certain geographic areas. During 1987-93 there were 139 claims for permanent sensory or motor disturbances related to removal of lower third molars in Finland. The lingual nerve was injured in 54% and the inferior alveolar nerve in 41% of the claims. In 91% of the cases the injury occurred in relation to surgi…

The dental practitioner's role in emergency health services.

In a national disaster, the medical profession would lose physicians and auxiliary personnel and would need assistance. Canada's 22,000 physicians and 85,000 nurses are located for the most part in potential target areas. Survivors among Canada's 6396 dentists could supply 30% reinforcement. The dentist's training, his manual dexterity and experience acquired in the management of hemorrhage, shock, débridement, suturing, reduction and immobilization of fractures, and control of pain and infection would be valuable. Additional functions he could perform would be first-aid, including but not lim…

The Level of Self-Perceived Knowledge Regarding Temporo-mandibular Disorders among the Saudi Arabian Dental Professionals Working in Dental Institutes or Universities vs. Clinics or Hospitals.

Temporo-mandibular disorders (TMDs) are considered as one of the most common type of oro-facial pain. The basic knowledge regarding examining, diagnosing and treating the TMDs is part of the dental curriculum designed by the Saudi Dental Council. But till date it is not reported how the dentists working in dental institutes/universities and clinics/hospital of Saud Arabia make an attempt to diagnose and treat TMDs cases. Therefore, the main aim of the study was to evaluate the level of self-perceived TMD knowledge among the dentists working in Saudi Arabia and to compare the between those work…

Viral infections of oral cavity.

Viral infections of the oral cavity usually manifest as either ulceration or blistering presentation of oral tissues. Oral viral infections are encountered in dental practice but received less clinical interest due to the lesser frequency of patients and diagnostic challenges. The clinical presentation, pathogenic mechanism, investigations, and management of oral viral infections are integrated into the article which will enable general dentists to develop critical thinking processes on differential diagnosis and management through a multidisciplinary approach with specialist dentists.

Clinical anatomy of the maxillary sinus: application to sinus floor augmentation.

The anatomy of the maxillary sinus, especially its vascular anatomy, and its relationships with the teeth and alveolar processes have been well documented. The development of cone-beam computed tomography has resulted in dentists being more familiar with maxillary sinus floor augmentation procedures. This paper aims to revisit the classic anatomy of the maxillary sinus and review the newly published literature in order to help dentists diagnose in more detail and perform safer surgery of the maxillary sinus.

Selection of an optimal treatment method for acute periodontitis disease.

The present paper is devoted to selection of an optimal treatment method for acute periodontitis by using fuzzy Choquet integral-based approach. We consider application of different treatment methods depending on development stages and symptoms of the disease. The effectiveness of application of different treatment methods in each stage of the disease is linguistically evaluated by a dentist. The stages of the disease are also linguistically described by a dentist. Dentist's linguistic evaluations are represented by fuzzy sets. The total effectiveness of the each considered treatment method is

Orthodontics and its interactions with other dental disciplines.

Preservation or alteration of the natural dentition to an optimal state of health, function, comfort, and esthetics remains the primary goal of dentistry. However, referral of patients by general dentists to orthodontic specialists may be complex as it involves the interaction of the two dental professionals and the patient. Because there is a marked variation in referral rates between the dentists and many patients may be referred inappropriately, a clear understanding of what is known about current orthodontic treatment possibilities, limitations and inherent risks is necessary. The purpose …

The Mental Capacity Act 2005 and its impact on dental practice.

In 1995, the Law Commission was given the task of investigating 'the adequacy of legal and other procedures for decision-making on behalf of mentally incapacitated adults'. It concluded that the law was fragmented and confusing and called for a single statute to govern decision-making on behalf of mentally incapable adults regarding welfare, healthcare and financial matters. There followed a 15 year period of consultation, resulting in the new Mental Capacity Act 2005 which came into full force in October 2007. Dentists who administer treatment to patients suffering from mental incapacity due …

The accuracy of tooth loss data collected by nurses.

This paper reports on the accuracy of tooth counts conducted in 22 subjects by 10 trained nurses as part of a large longitudinal study of a pharmacological agent. These nurses participated in a training course consisting of seminars, discussion, demonstrations, and practice examinations. Each of the nurses then counted the teeth of 22 subjects and recorded their findings independently. The counts of the nurses were compared with those of the dentists to assess the accuracy of the nurses' counts. We found that nurses and dentists were in perfect agreement for 86% of the patient counts conducted…

Relationship of osteoporosis to excessive residual ridge resorption.

There are many systemic factors which contribute to alveolar bone loss and decreased ability to tolerate dental prostheses. Osteoporosis should be considered as a possibility. Observation of the high numbers of osteoporotic fractures in the geriatric population serves to illustrate the scope of this problem. The condition of osteoporosis results in bone loss in the maxillae and mandible as well as in other bones of the body. It is highly feasible that the disease makes a strong contribution to the deterioration of the dental health in the geriatric patient. The American diet is too low in calc

A Case Study of Community Dental Health Coordinator: Evaluating a Medical-Dental Collaboration to Address Barriers to Preventive Dental Care.

BACKGROUND: Tooth decay is common, but low-income families have low rates of preventive oral health care. The American Dental Association community dental health coordinator (CDHC) model has successfully increased referrals to co-located dentists. OBJECTIVE: To implement a CDHC to link families between the medical home and community dentists. METHODS: The CDHC universally screened patients between 2-17 years of age during medical check-ups at two sites. Iterative plan-do-study-act (PDSA) cycles aim to increase screening, increase participants, and increase phone follow-up with families. When…

The importance of clinical features and computed tomographic findings in numb chin syndrome: a report of two cases.

BACKGROUND: Dentists need to be aware of the relationship between malignancies and paresthesia or complete loss of sensation in a jaw segment. In particular, dentists should be aware of numb chin syndrome (NCS) and its clinical manifestations, as well as the limitations of using panoramic radiographs to detect the causative malignancy. CASE DESCRIPTION: The authors report two cases of paresthesia in the mental region. No lesions were readily apparent on the patients' panoramic radiographs. To exclude the presence of disease in the mandible that could have been responsible for the NCS, the aut…

Detection of hepatitis C virus in saliva before and after scaling of dental calculus.

Dentists and dental health care workers are at risk of contracting hepatitis C virus (HCV) through dental treatment, since HCV RNA was reported to be easily detectable in the saliva of patients with chronic HCV liver disease. We tested for the presence of HCV RNA in saliva before and after removal of dental calculus, and in splashes on the chin-length face shields of dentists following treatment of six patients with HCV chronic liver diseases. We used a sensitive reverse transcription polymerase chain reaction (RT-PCR) method, to estimate exposure to HCV. All patients were anti-HCV and HCV RNA…
