A double-blind study of hyposensitization with an alginate conjugated extract of D. pteronyssinus (Conjuvac) in patients with perennial rhinitis. 1. Clinical aspects.

The efficacy of hyposensitization with a standardised extract of Dermatophagoides pteronyssinus (D. pteronyssinus) conjugated to alginate and containing known amounts of antigen P1 (Conjuvac) was tested in a double blind, placebo controlled, multi-centre study in 66 adult patients with perennial rhinitis. Patients received 11 weekly injections of increasing concentrations of Conjuvac containing from 56 x 10(1) to 448 10(3) IU D. pteronyssinus or placebo injections of the alginate diluent to some of which 5 micrograms of histamine has been randomly added. This was followed by 15 monthly injecti

IL-37a facilitates allergen immunotherapy in a house dust mite-induced mouse model of allergic asthma.

In recent years, the incidence of allergic asthma has increased dramatically. Allergen immunotherapy (AIT) is an effective approach to achieve long-term remission of allergic asthma and has gained widespread attention. However, current AIT suffers from low efficacy and long treatment duration. In this study, we found that IL-37a transgenic mice with allergic asthma exhibited reduced pulmonary inflammation, decreased Th2 cytokines, and lower allergen-specific IgE following AIT. Mechanistic studies revealed increased IL-37+ Bregs, suggesting that IL-37a may promote AIT through Bregs. Our study d

The latest investigational drugs for patients with allergic rhinitis.

INTRODUCTION: Allergic rhinitis (AR) is a prevalent IgE-mediated inflammatory disease with significant global health and economic burdens. Common treatments include pharmacotherapy and allergen-specific immunotherapy (AIT), but challenges remain in managing some of the moderate-to-severe patients, driving development on targeted biologics. AREAS COVERED: This review covers recent advances in AR management, including optimized pharmacotherapy, biologics targeting type 2 inflammation and innovations in AIT. EXPERT OPINION: The management toward AR should fully consider the phenotypic and endot

ABO-incompatible kidney transplantation with high isoagglutinin titres: practice pattern and problem solving: a German multi-centre survey.

INTRODUCTION: ABO-incompatible kidney transplantation (ABOi-KT) expands the donor pool, yet baseline anti-ABO isoagglutinin titres ≥ 1:512 remain challenging and practice is highly variable. We surveyed the German transplant centres to capture current management of very high-titre ABOi candidates and centre responses to insufficient titre decline. METHODS: Thirty-six centres were invited to complete an anonymised 19-item web-based questionnaire (January-March 2025) on (i) centre activity, (ii) isoagglutinin testing and isoagglutinin titre limits (iii) desensitisation protocol, (iv) ABO abort

Poppy seed anaphylaxis: clinical and allergic features.

Poppy seeds are increasingly used for flavoring, for example, in cakes, bread, and fruit salads, and it is gaining popularity globally. Most case reports of poppy seed allergy are in children. Ingestion of cake, in which ground poppy seed is used, is mostly associated with anaphylactic reactions. Reactions may occur even with minimal amounts or traces. In sera of patients allergic to poppy seeds, four IgE-reactive poppy seed proteins were identified. Cross allergy with sesame and buckwheat are reported. Cross-reaction is described with nuts (cashew and walnut), wheat, rye flour, buckwheat, ric

Peanut Oral Immunotherapy in Children: An Italian Single-Center Retrospective Cohort Study.

Introduction: Peanut oral immunotherapy (P-OIT) is an emerging treatment strategy for peanut allergy (PA). Although a standardized pharmaceutical product, Peanut (Arachis hypogaea) Allergen Powder-dnfp, has been approved in several countries, it is not universally available. In such contexts, real-world protocols using readily utilizable peanut products may represent an alternative approach. This study aimed to describe the feasibility, safety, and clinical outcomes of P-OIT using toasted peanuts in a real-world effort in a pediatric population. Methods: This single-center retrospective cohort

A biscotti-based milk and egg ladder using stepwise oral food challenges: A retrospective study in high-risk children.

Background: Cow's milk and hen's egg allergy are the most common immunoglobulin E (IgE) mediated food allergies in childhood. Current oral immunotherapy protocols use the native raw allergen, with severe anaphylaxis reported. Objective: The objective was to retrospectively evaluate the safety and clinical outcomes of a biscotti (twice baked cake) based milk and egg ladder by using stepwise oral food challenges in children at high risk. Methods: In this single-center retrospective review (June 2024 to April 2025), records of 33 children with IgE-mediated cow's milk protein allergy (n = 11) or h

The Role of Tocilizumab in Kidney Transplantation: A Narrative Review on Desensitization and Antibody-Mediated Rejection Treatment.

Hyponatremia is a relatively frequent finding in multiple myeloma (MM) and may result from either pseudohyponatremia, due to marked hyperproteinemia, or true hyponatremia from genuine sodium-water imbalance. Differentiating between these two entities is essential, as they differ in pathogenesis, clinical relevance, and management. Pseudohyponatremia, observed in approximately 15-20% of MM patients, is a measurement artifact occurring with indirect ion-selective electrode techniques when plasma water fraction is reduced by high M protein levels. Serum osmolality remains normal, and no sodium co

Safety and Tolerability of Accelerated Venom Immunotherapy with Depot Extracts in Insect-Venom-Allergic Patients.

Background and Objectives: Hymenoptera venom allergy (HVA) represents one of the most frequent causes of anaphylaxis in adults. Venom immunotherapy (VIT) remains the only causal intervention that effectively reduces the risk of systemic reactions following subsequent stings. Accelerated-induction protocols enable rapid attainment of the maintenance dose; however, data on their safety with depot preparations are still limited. To evaluate the safety of accelerated-induction VIT using a depot preparation in a real-world clinical setting. Materials and Methods: A retrospective, single-center stud

Updated Treatment of Non-Steroidal Anti-Inflammatory Drug-Exacerbated Respiratory Disease: How to Decide on Aspirin Therapy After Desensitization or Biologics? When? How? An EAACI Task Force Report.

Nonsteroidal anti-inflammatory drug (NSAID)-exacerbated respiratory disease (NSAID-ERD) is a heterogeneous condition characterized by chronic eosinophilic airway inflammation in patients with chronic rhinosinusitis with nasal polyps (CRSwNP) and asthma whose symptoms are aggravated after ingestion of aspirin and other NSAIDs. As a unique treatment, aspirin treatment after desensitization (ATAD) has been in use for several years after showing the inhibitory effect on CRS symptoms and nasal polyp growth as well as severe asthma. However, in recent years, biologics have also shown to cause a decr

Microneedles Coated with Cow's Milk Proteins: Immunogenicity, Stability, and Safety Assessment.

Cow's milk allergy (CMA) is one of the most frequently occurring food allergies in children, especially in infants less than 3 years old. Mindful avoidance of CMA-triggering foods and prompt epinephrine injection to overcome anaphylaxis in the case of accidental ingestion are the only options currently available to allergic subjects. This study investigates the potential of coated microneedles for delivering CMA into the skin as a novel approach to allergen immunotherapy. Precise amounts of cow's milk proteins (CMP) were dip-coated onto stainless steel microneedle patches and reproducibly deli

Abbreviation of Desensitization Protocol for Pediatric Patients with Lysosomal Storage Diseases Receiving Enzyme Replacement Therapy.

INTRODUCTION: Lysosomal storage diseases (LSDs) constitute a group of metabolic disorders characterized by the accumulation of substrates within lysosomes. For their treatment, disease-specific enzyme replacement therapy (ERT) is employed. In cases of hypersensitivity reactions that may occur during these treatments, desensitization of enzyme therapy becomes necessary. Repeated desensitization procedures may result some degree of tolerance. This study presents cases of patients received abbreviated desensitization protocols following repeated desensitization procedures. METHOD: During the per

Venom Component Allergen IgE Measurement in the Diagnosis and Management of Insect Sting Allergy.

Accurate identification of allergy-eliciting stinging insect(s) is essential to ensuring effective management of Hymenoptera venom-allergic individuals with venom-specific immunotherapy. Diagnostic testing using whole-venom extracts with skin tests and serologic-based analyses remains the first level of discrimination for honeybee versus vespid venom sensitization in patients with a positive clinical history. As a second-level evaluation, serologic testing using molecular venom allergens can further discriminate genuine sensitization (honeybee venom: Api m 1, 3, 4, and 10 vs yellow jacket veno
