Size and location of the fetal diaphragm during the fetal period in human fetuses.

AIM: The aim of this study was to determine the development and the size and localization of the diaphragm during the fetal period in human fetuses. MATERIALS AND METHODS: One hundred and eighty-four diaphragm obtained from human fetuses aged between 9 and 40 weeks of gestation was used in this study. The localization of the diaphragm with respect to abdominal cavity and the level of costodiaphragmatic recess with respect to the ribs were determined. The dimensions (height, width, and depth) of the diaphragm were measured. The diameter and localization of apertures on the diaphragm were deter…

Distribution of costameric proteins in the diaphragm of patients with chronic obstructive pulmonary disease.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is associated with an increased load on the diaphragm. Increased (eccentric) loading has been shown to result in disturbances in the cytoskeleton. OBJECTIVES: We hypothesized that due to a continuous overload of the diaphragm in COPD patients, distinct alterations in the membrane-associated cytoskeleton occur, especially in the costameres. METHODS: Diaphragm biopsies from 7 COPD patients (forced expiratory volume in 1 s 62 +/- 3% predicted) and 5 non-COPD patients (forced expiratory volume in 1 s 105 +/- 6% predicted) were obtained. Cr…

Activation of the ubiquitin-proteasome pathway in the diaphragm in chronic obstructive pulmonary disease.

RATIONALE: Studies show that the myosin content of the diaphragm in patients with mild to moderate chronic obstructive pulmonary disease (COPD) is reduced, compromising diaphragm contractile performance. The mechanisms for reduced contractile protein content are unknown. In the present study we hypothesized that the loss of contractile protein content is associated with activation of the ubiquitin-proteasome pathway in the diaphragm of patients with mild to moderate COPD. METHODS: Proteolytic activity of isolated 20S proteasomes was determined in diaphragm biopsies from patients with and with…

Phrenic nerve distribution in the rabbit diaphragm and morphometric analysis of nerve branches.

The best method to evaluate the pathogenesis of diaphragmatic disorders is to demonstrate the distribution pattern of the phrenic nerve in the diaphragm. For this purpose the branching pattern and the microanatomic features of the phrenic nerve were observed in six rabbits. All diaphragms were stained by using Sihler's stain method. The phrenic nerve divided into three to four branches when entering the diaphragm. These branches were classified as sternal, anterolateral, posterolateral and crural. The crural branches were the thickest whereas the anterolateral branches were the thinnest. Knowl…

Introducing diaphragms into the mix: what happens to male condom use patterns?

The objective of this analysis was to assess the effect of introducing the diaphragm on condom use patterns. Participants included one hundred eighty-nine women attending family planning clinics in Harare, Zimbabwe who reported less than 100% condom use. The proportion of acts where at least one method was used significantly increased over using follow-up; male condom use remained stable. A diaphragm was used with 50% to 54% of acts; male condoms were also used about 50% of the time. The proportion of acts where a female condom was used decreased. Women who used both male and female condoms we…

Effects of chronic intermittent asphyxia on rat diaphragm and limb muscle contractility.

OBJECTIVE: In obstructive sleep apnea (OSA), there is intermittent upper airway (UA) collapse due to an imbalance between the collapsing force generated by the diaphragm and the stabilizing force of the UA muscles. This results in chronic intermittent asphyxia (CIA). We have previously shown that CIA affects UA muscle fatigue, but little is known about the effects of chronic hypoxia on diaphragm or on limb muscle contractile properties and structure. DESIGN: Rats were exposed to asphyxia and normoxia twice per minute for 8 h/d for 5 weeks to simulate the intermittent asphyxia of OSA in humans…

Transcriptional induction of slit diaphragm genes by Lmx1b is required in podocyte differentiation.

LMX1B encodes a LIM-homeodomain transcription factor. Mutations in LMX1B cause nail-patella syndrome (NPS), an autosomal dominant disease with skeletal abnormalities, nail hypoplasia, and nephropathy. Expression of glomerular basement membrane (GBM) collagens is reduced in Lmx1b(-/-) mice, suggesting one basis for NPS nephropathy. Here, we show that Lmx1b(-/-) podocytes have reduced numbers of foot processes, are dysplastic, and lack typical slit diaphragms, indicating an arrest in development. Using antibodies to podocyte proteins important for podocyte function, we found that Lmx1b(-/-) podo…

The number of podocyte slit diaphragms is decreased in minimal change nephrotic syndrome.

The pathophysiology of proteinuria in acquired kidney diseases is mostly unknown. Recent findings in genetic renal diseases suggest that glomerular epithelial cells (podocytes) and the slit diaphragm connecting the podocyte foot processes play an important role in the development of proteinuria. In this work we systematically evaluated the podocyte slit pores by transmission electron microscopy in two important nephrotic diseases, minimal change nephrotic syndrome (MCNS) and membranous nephropathy (MN). As controls, we used kidneys with tubulointerstitial nephritis (TIN). Effacement of podocyt…

Dual influence of disease and increased load on diaphragm muscle in heart failure.

We have recently shown that mitochondrial function and energy metabolism are altered in the myocardium as well as in slow and fast locomotor muscles of rats subjected to prolonged congestive heart failure (CHF) suggesting a generalized metabolic myopathy in heart failure. Here, we investigate whether the diaphragm of CHF animals, which experiences both increased work and the general systemic influence of heart failure, will also be susceptible to altered energy metabolism. Biopsies were obtained from the costal diaphragm of failing rats 8 months after aortic banding. A marked increase in type …

Injury of the human diaphragm associated with exertion and chronic obstructive pulmonary disease.

Injury of the diaphragm may have clinical relevance having been reported in cases of sudden infant death syndrome or fatal asthma. However, examination of diaphragm injury after acute inspiratory loading has not been reported. The purpose of this study was to determine whether an acute inspiratory overload induces injury of the human diaphragm and to determine if diaphragm from chronic obstructive pulmonary disease (COPD) is more susceptible to injury. Eighteen patients with COPD and 11 control patients with normal pulmonary function (62 +/- 10 yr) undergoing thoracotomy or laparotomy were stu…

[Effect of dietary protein on the function and morphology of diaphragm in rats with experimental emphysema].

The effects of dietary protein on the function and morphology of diaphragm of rats with emphysema were observed in 50 SD rats, 10 rats in each group. The emphysema in groups E(emphysema control), HP(emphysema with high protein diet) and LP(emphysema with low protein diet) was induced by intra-tracheal instillation of elastase (750 U/kg BW). The control rat (C1, C2) were instilled with normal saline. After 6 weeks of experiment, the total lung volume and the average area of alveolus was significantly increased in the group E compared with group C1 (P < 0.05) and the number of alveolus per unit …

Postural activity of the diaphragm is reduced in humans when respiratory demand increases.

1. Respiratory activity of the diaphragm and other respiratory muscles is normally co-ordinated with their other functions, such as for postural control of the trunk when the limbs move. The integration may occur by summation of two inputs at the respiratory motoneurons. The present study investigated whether postural activity of the diaphragm changed when respiratory drive increased with hypercapnoea. 2. Electromyographic (EMG) recordings of the diaphragm and other trunk muscles were made with intramuscular electrodes in 13 healthy volunteers. Under control conditions and while breathing thro…

Elicited pontogeniculooccipital waves and phasic suppression of diaphragm activity in sleep and wakefulness.

Fractionations are 20- to 100-ms pauses in diaphragm activity that occur spontaneously during rapid-eye-movement (REM) sleep, sometimes in association with pontogeniculooccipital (PGO) waves. Auditory stimuli can elicit fractionations or PGO waves during REM sleep, non-REM (NREM) sleep, and waking; however, their interrelationship has not been investigated. To determine whether the two phenomena are produced by a common phasic-event generator in REM sleep, we examined PGO waves and fractionations that were elicited by auditory stimuli (tones) presented to freely behaving cats across states. To…

Effects of cooling the ventrolateral medulla on diaphragm activity during NREM sleep.

Dysfunction through cooling of neurons near the ventrolateral medullary (VLM) surface results in apnea in the anesthetized state, whereas similar neuronal dysfunction in the awake state only modestly decreases breathing. The purpose of this study was to investigate effects on breathing, as measured by diaphragm electromyogram (EMGdi), of VLM neuronal dysfunction during NREM sleep, a naturally occurring change in state. In six goats, thermodes for cooling were chronically implanted between the first hypoglossal rootlet and the pontomedullary junction (area M and area S). During wakefulness and …

The effects of oesophageal distension on diaphragm and laryngeal muscle activity in the anaesthetized cat.

The electromyographic (EMG) activities of diaphragm and laryngeal muscles were recorded during oesophageal distension in anesthetized cats. The responses to distension of the thoracic oesophagus differed from those evoked by distension of the cervical oesophagus. The crural component of the diaphragm (CD) was inhibited by distension of the thoracic oesophagus; distension of the cervical oesophagus did not affect CD EMG. Thyroarytenoid (TA) muscle EMG increased markedly and consistently in response to distension of the cervical oesophagus. Distension of the thoracic oesophagus only produced sta…

Pathogenesis and management of the lens-iris diaphragm retropulsion syndrome during phacoemulsification.

After the ultrasonic tip is introduced into the highly myopic eye and inflow is activated, the anterior chamber deepens, the lens-iris diaphragm is displaced posteriorly, the iris becomes concave, and the pupil dilates from the weight of the water column above the eye. We define a known but undescribed syndrome--the lens-iris diaphragm retropulsion syndrome (LIDRS)--which is thought to be caused by abnormally loose zonules. The surgeon's reflexive action, lowering the infusion bottle height, reduces the weight of the water column but also reduces the infusion limit. We managed this situation w…

Improvement by diazepam of neuromuscular transmission blocked by anticholinesterase agents in mouse diaphragms.

Effects of diazepam on the neuromuscular transmission blocked by neostigmine were studied in isolated mouse phrenic nerve-diaphragm preparations. Diazepam, in the absence of neostigmine, had no significant effect on the neuromuscular transmission at concentrations lower than 100 microM, except to enhance muscle contractility. Single and train pulses-evoked endplate potentials (e.p.p.s) and miniature e.p.p.s (m.e.p.p.s) were also unaffected. At concentrations of 175 microM or higher, diazepam caused an axonal conduction block. However, neostigmine-induced twitch potentiation, spontaneous fascic…
