Epidemiology of non-ischaemic dilated cardiomyopathy.

Dilated cardiomyopathy, defined by left ventricular dilatation and systolic dysfunction, is a major cause of heart failure, heart transplantation and sudden cardiac death, especially in young and middle-aged adults. Dilated cardiomyopathy of non-ischaemic aetiology is more common than once thought, with current prevalence estimated at around 1 in 220 based on cardiac magnetic resonance imaging studies, and the prevalence is twice as high in men than in women. However, the true prevalence could be even higher when early or subclinical forms of the disease are considered. Advances in genetic tec…

Can't Stomach It: A Dilated Tale of Achalasia.

Achalasia is a common esophageal motility disorder defined by impaired relaxation of the gastroesophageal junction (GEJ). Complications can occur secondary to achalasia, such as dysphagia, weight loss, chest pain, aspiration, and esophageal rupture. This report discusses a case of massive esophageal dilatation, also known as megaesophagus, in the setting of achalasia. This case highlights the complications secondary to achalasia, such as aspiration, pneumatic cysts, weight loss, dysphagia, and esophageal dilatation, and describes the multidisciplinary approach needed to manage patients with se…

Genetic counselling implementation in dilated cardiomyopathy.

Genetic testing has become an integral part of the diagnostic workup of patients with dilated cardiomyopathy (DCM). While the initial goal of genetic testing was to identify family members at risk, recent advances have now extended their relevance to clinical decision-making. Our knowledge of the genetic architecture of DCM has expanded significantly, promoting a shift from the monogenic dogma towards a broader polygenic spectrum. However, current genetic testing strategies still primarily rely on the model of monogenic inheritance with an incomplete penetrance. Large studies have shown a yiel

Genotype-Driven Prognostication in Dilated Cardiomyopathy.

PURPOSE: This study evaluated the regenerative potential of a novel polyethylene glycol (PEG)-chitosan hydrogel as an alternative to a conventional collagen membrane for guided tissue regeneration (GTR) of class II furcation defects in a canine model. METHODS: Standardized class II furcation defects were created in the maxillary third and fourth premolars of 6 beagle dogs. Sites were randomly allocated to 4 groups: 1) sham control, 2) particulate biphasic calcium phosphate (BCP) with a cross-linked collagen membrane (O3CM), 3) collagenated BCP with a collagen membrane (O3ColCM), and 4) partic

Infantile Dilated Cardiomyopathy in Alström Syndrome.

We report two cases of end-stage dilated cardiomyopathy as the initial manifestation of Alström syndrome (ALMS), in infants aged two and five months. This rare monogenic, autosomal, and recessive genetic condition is a multisystem disorder characterized by visual and hearing impairment, cardiomyopathy childhood obesity, and other anomalies. These cases highlight the importance of genetic testing targeting the ALMS1 gene in the assessment of apparently isolated dilated cardiomyopathy. We review the cardiac features of ALMS. To our knowledge, we also describe the first case of successful surgica

Dilated gut conditions: diagnosis and management.

Dilatation of the gut occurs in response to either mechanical obstruction or aperistalsis. The hallmark features are symptoms of bowel obstruction with vomiting, constipation, abdominal pain and distension. This review will primarily deal with the non-mechanical causes of gut dilatation, both intestinal and colonic, and differentiate between acute and chronic presentations.

Transcriptome studies of inherited dilated cardiomyopathies.

Dilated cardiomyopathy (DCM) is a group of heart muscle diseases that often lead to heart failure, with more than 50 causative genes have being linked to DCM. The heterogenous nature of the inherited DCMs suggest the need of precision medicine. Consistent with this emerging concept, transcriptome studies in human patients with DCM indicated distinct molecular signature for DCMs of different genetic etiology. To facilitate this line of research, we reviewed the status of transcriptome studies of inherited DCMs by focusing on three predominant DCM causative genes, TTN, LMNA, and BAG3. Besides st

OPA1, a molecular regulator of dilated cardiomyopathy.

Dilated cardiomyopathy (DCM) is a disease with no specific treatment, poor prognosis and high mortality. During DCM development, there is apoptosis, mitochondrial dynamics imbalance and changes in cristae structure. Optic atrophy 1 (OPA1) appears at high frequency in these three aspects. DCM LMNA (LaminA/C) gene mutation can activate TP53, and the study of P53 shows that P53 affects OPA1 through Bak/Bax and OMA1 (a metalloprotease). OPA1 can be considered the missing link between DCMp53 and DCM apoptosis, mitochondrial dynamics imbalance and changes in cristae structure. OPA1 regulates apoptos

The electrocardiogram in non-ischaemic-dilated cardiomyopathy.

This article summarizes the main electrocardiogram (ECG) findings in dilated cardiomyopathy (DCM) patients. Recent reports are described in the great 'pot' of DCM peculiar ECG patterns that are typical of specific forms of DCM. Patients with late gadolinium enhancement on CMR, who are at greatest arrhythmic risk, have also distinctive ECG features. Future studies in large DCM populations should evaluate the diagnostic and prognostic value of the ECG.

Metabolic and Metabolomics Insights into Dilated Cardiomyopathy.

BACKGROUND: Dilated cardiomyopathy (DCM) is the most common form of heart muscle disease characterized by progressive dilatation and ventricular dysfunction. Metabolomics is an emerging and powerful discipline that provides a global information on the phenotype of mammalian systems via the study of endogenous and exogenous metabolites in cells, tissues, and biofluids. These studies aid in the identification of biomarkers to prevent diseases in later life or help to early detect onset of diseases as well as aiding in the elucidation of disease mechanisms. SUMMARY: Metabolomics provides a uniqu…

Molecular genetic mechanisms of dilated cardiomyopathy.

Heart failure (HF) is a rapidly growing cardiovascular condition with a prevalence of ~40 million individuals worldwide [1]. While HF can be caused by acquired conditions such as myocardial infarctions and viruses [2], the genetic basis for HF is rapidly emerging particularly for dilated cardiomyopathy (DCM) that is the most prevalent HF type. In this review, insights from the rapid expansion in next-generation sequencing technologies applied in the HF clinic are merged with recent functional genomics studies to provide a contemporary view of DCM molecular genetics.

Dilated cardiomyopathy in a cat with congenital hyposomatotropism.

CASE SUMMARY: A 7-month-old domestic shorthair cat was presented for evaluation of stunted growth, recurrent hypoglycaemia during the first months of its life and altered mentation. Complete blood count and biochemistry were unremarkable, except for mildly elevated serum creatinine concentration (despite low muscle mass) and concurrent isosthenuria. Hyposomatotropism was diagnosed based on persistent low circulating insulin-like growth factor 1 concentrations and a lack of response of circulating growth hormone (GH) concentration after the administration of GH-releasing hormone. Other endocrin
