How social class shapes breast cancer risk perspectives and prevention practices of Australian midlife women: a qualitative study using the concept of 'breast cancer candidacy'.

BACKGROUND: The increasing incidence of breast cancer and disease burden is a significant public health concern. While 30% of breast cancers could be prevented through addressing modifiable risk factors, misconceptions among women about breast cancer risks hamper primary prevention. In the absence of primary prevention, secondary prevention such as mammography increases the early detection of breast cancer and improves health outcomes. However, current population-level screening rates indicate secondary prevention is suboptimal. More effective public health efforts to improve breast cancer pre

Bibliometric analysis of dietary patterns in Parkinson's disease: from Mediterranean diet to emerging trends.

BACKGROUND: Parkinson's disease (PD) is one of the fastest-growing neurological disorders. Treatments remain symptomatic, with no disease-modifying therapies available. Recent preclinical and epidemiological studies suggest that dietary patterns may influence PD pathology through the microbiota-gut-brain axis by modulating gut microbiota. However, evidence remains conflicting: some studies report protective effects of specific diets, while large-scale prospective studies find no significant associations. These discrepancies reflect differences in study design and dietary assessment and a lack

Breast cancer and hormonal contraceptives: further results. Collaborative Group on Hormonal Factors in Breast Cancer.

The Collaborative Group on Hormonal Factors in Breast Cancer has brought together and reanalysed the worldwide epidemiological evidence on breast cancer risk and use of hormonal contraceptives. Original data from 54 studies, representing about 90% of the information available on the topic, were collected, checked and analysed centrally. The 54 studies were performed in 26 countries and include a total of 53,297 women with breast cancer and 100,239 women without breast cancer. The studies were varied in their design, setting and timing. Most information came from case-control studies with contr

A protocol for investigation of the effects of outdoor air pollution on stroke incidence, phenotypes and survival using the South London Stroke Register.

Stroke is a major cause of death and disability. About 5.3 million people die every year from stroke worldwide with over 9 million people surviving at any one time after suffering a stroke. About 1 in 4 men and 1 in 5 women aged 45 years will suffer a stroke if they live to their 85th year. It is estimated that by 2023 there will be an absolute increase in the number of people experiencing a first ever stroke of about 30% compared with 1983. In the UK, stroke is the third commonest cause of death and the most common cause of adult physical disability and consumes 5% of the health and social se

Stochastic modeling of pneumonia transmission dynamics and implications for public health control.

OBJECTIVE: This study investigates the transmission dynamics of pneumonia using deterministic and stochastic SCIR compartmental models. The main objective is to examine how environmental randomness, contact-rate fluctuations, and individual-level interactions influence pneumonia persistence, extinction, and public health control outcomes. METHODS: The population was divided into four epidemiological classes: susceptible, carrier, infected, and recovered individuals. A deterministic model based on ordinary differential equations was first formulated, and stochastic perturbations were then intr

A critical review of the epidemiological evidence of effects of air pollution on dementia, cognitive function and cognitive decline in adult population.

Dementia is arguably the most pressing public health challenge of our age. Since dementia does not have a cure, identifying risk factors that can be controlled has become paramount to reduce the personal, societal and economic burden of dementia. The relationship between exposure to air pollution and effects on cognitive function, cognitive decline and dementia has stimulated increasing scientific interest in the past few years. This review of the literature critically examines the available epidemiological evidence of associations between exposure to ambient air pollutants, cognitive performa

Metabolic syndrome and incident thyroid cancer risk in adults: A systematic review and meta-analysis.

The global incidence of thyroid cancer (TC) has increased, with metabolic syndrome (MetS) identified as a potential modifiable risk factor. However, epidemiological evidence regarding this association remains inconsistent. We conducted a meta-analysis to quantify the relationship between MetS and the risk of incident TC in adult populations. We searched PubMed, Embase, and Web of Science for longitudinal observational studies assessing MetS in relation to incident TC. Risk ratios (RRs) with 95% confidence intervals (CIs) were pooled using random-effects models to account for heterogeneity. Our

Joint effects of depression, lipid metabolic dysfunction, and their trajectories on incident cardiometabolic multimorbidity: a prospective multi-cohort analysis from two national cohorts.

BACKGROUND: Cardiometabolic multimorbidity (CMM) imposes a growing public health burden on middle-aged and older adults. Depression and lipid metabolic dysfunction are modifiable risk factors for CMM, yet their joint effects and dynamic co-trajectories on CMM risk across diverse ageing populations remain poorly understood. METHODS: This prospective multicohort study used individual-level data from the China Health and Retirement Longitudinal Study (CHARLS) and the English Longitudinal Study of Ageing (ELSA). Two analytical risk sets were defined to support static and dynamic analyses. Metabol

Food as Friend or Foe: A Decadal Narrative Review of Dietary Patterns as Determinants of Gastrointestinal Pathophysiology and Clinical Outcomes (2015-2025).

Diet is a major modifiable determinant of gastrointestinal (GI) health, influencing disease risk and progression through coordinated effects on the gut microbiome, immune regulation, epithelial barrier integrity, oxidative balance, and epigenetic mechanisms. This narrative review synthesizes epidemiological, mechanistic, and clinical evidence from the past decade to examine bidirectional relationships between dietary patterns and seven common GI disorders: celiac disease, irritable bowel syndrome (IBS), Crohn's disease, ulcerative colitis, Helicobacter pylori-associated gastritis, peptic ulcer

Myopia and eye disease: From correlation to causation.

Myopia is increasingly recognised as a risk factor strongly linked to visual impairment and sight-threatening ocular disease rather than a simple refractive state. The assumption of causality to these secondary effects underpins myopia control and its long-term public health value. This review considers epidemiologic, longitudinal, mechanistic, interventional, natural-experiment, and genetic evidence to evaluate causality based on Bradford Hill's nine criteria: strength, consistency, specificity, temporality, biological gradient, plausibility, coherence, experiment, and analogy. The graded dos

Urate-lowering therapy in cardiovascular pharmacotherapy: from Mendelian randomization data to cardio-renal-metabolic risk modulation.

AIMS: Serum uric acid (SUA) and hyperuricaemia have re-emerged as determinants of cardiovascular (CV) risk beyond gout. This review integrates epidemiological, Mendelian randomization (MR), and pharmacological evidence to define the role of SUA and urate-lowering therapy (ULT) in contemporary CV pharmacotherapy. METHODS AND RESULTS: We synthesized population-based studies, MR and drug-target MR analyses, and randomized trials of xanthine oxidase inhibitors (XOIs), uricosurics/URAT1 inhibitors, biologic uricases, and CV drugs with urate-modifying effects. Hyperuricaemia is prevalent and rising

Targeting progressive multiple sclerosis: Toward mechanism-informed precision medicine.

Multiple sclerosis has undergone a therapeutic revolution over the past three decades. Randomized clinical trials and real-world data demonstrate that modern disease-modifying therapies substantially reduce relapse rates and acute inflammatory activity detected by magnetic resonance imaging (MRI). However, disability accumulation increasingly occurs independent of relapse activity, highlighting progression biology as the principal unmet need. Converging epidemiological and molecular evidence supports a pivotal role for Epstein-Barr virus (EBV) infection in disease initiation, whereas later sta

Sex differences in the association between Korean Healthy Eating Index and type 2 diabetes mellitus in Korean adults: a prospective cohort study.

OBJECTIVES: Dietary quality is a modifiable determinant of type 2 diabetes mellitus (T2DM). However, evidence on the Korean Healthy Eating Index (KHEI) and sex-specific differences in its association with T2DM risk remains limited. This study is to examine the longitudinal association between KHEI and incident T2DM in Korean adults, with a focus on potential sex differences. METHODS: We analyzed 56,000 adults (37,684 women and 18,316 men) from the Health Examinee cohort of the Korean Genome and Epidemiology Study. Dietary intake was assessed using a validated semi-quantitative food frequency

Towards healthy sleep environments: Ambient, indoor, and personal exposure to PM2.5 and its implications in children's sleep health.

The growing impact of climate change and escalating wildfire seasons has led to heightened ambient air pollution, potentially affecting children's sleep health. However, current epidemiological research often relies on outdoor weather data to model the environmental impacts on sleep health, potentially mischaracterizing the actual bedroom environment. To address these challenges, we conducted experiments to investigate the relationships among ambient, indoor, and personal exposure to PM2.5 concentrations and obstructive sleep apnea (OSA) in children. We employed computational fluid dynamics (C
