Rehabilitation of shoulder and elbow injuries in tennis players.

Optimization of treatment of the tennis player with an upper extremity overuse injury requires a thorough understanding and analysis of the biomechanical and physiologic stresses inherent in the game of tennis. Anatomic adaptations found in the upper extremity of elite tennis players provide the framework for evaluation and treatment of overuse injury through both an anatomically based total-arm strength program and efforts to normalize joint arthrokinematics. Integration of these key factors allows the clinician to rehabilitate the patient and design preventive conditioning programs in a scie

Arthroscopic surgery of the elbow. Therapeutic benefits and hazards.

In this study, 22 patients were reviewed who underwent arthroscopic surgery of the elbow mainly for loose bodies or osteochondrotic lesions. Of these, 12 patients rated the result as good or excellent, 4 had slight improvement, and 6 were not satisfied. The average score (Hospital of Special Surgery scoring system; maximum 100 points) was raised from 70 points to 87 points after surgery. Patients with loose bodies in the elbow and no severe damage of the articular cartilage benefitted most. The results in patients with marked degenerative changes differed considerably.

MR assessment of posttraumatic flexion contracture of the elbow.

Preoperative assessment of posttraumatic flexion contracture of the elbow includes plain radiographs and tomograms, which are difficult to obtain in the coronal plane due to the contracture. We conducted this study to determine the usefulness of MR imaging in the work-up of these patients. Twelve patients with flexion contracture of the elbow were studied. In addition to standard spin-echo sequences, a sagittally acquired spoiled gradient-recalled echo 3D data set of the flexed elbow was obtained and reformatted coronally using a curved plane of reconstruction. The MR findings were compared to…

The restoration of elbow flexion with intercostal nerve transfers.

Seventeen patients with absent elbow flexion secondary to brachial plexus avulsion injury underwent intercostal neurotization of the biceps muscle. Followup was performed at an average of 5 years. The average age in this series was 21.8 years; the mean time interval from injury to the surgical procedure was 6 months. Eight of the 17 patients (47%) obtained good or excellent results as defined by Nagano et al. Five patients had muscle function ratings of M2 but were unable to power the elbow against gravity. The overall success rate theoretically may be increased by (1) decreasing the time inte…

[Pathogenesis of insertion tendinitis of the elbow in insurance medicine].

The pathogenesis of insertion tendinosis of the elbow,--lateral as well as medial epicondylitis--is difficult to define precisely. Different hypotheses have been postulated with a correspondingly large number of alternative therapeutic proposals. At first view, the prime factor may be a repetitive stress of the groups of muscles involved, respectively of the extensors of the hand and the wrist. In fact, the etiology is multifactorial and includes a cluster of causes. A series of 661 cases of insertion tendinosis of the elbow--lateral and medial epicondylitis--seen over 16 years in a private ou…

Archery-related injuries of the hand, forearm, and elbow.

The five patients reported herein had various archery-related injuries of the upper extremities. Acute injuries included arrow laceration of a digital nerve and artery, contusion of forearm skin and subcutaneous tissue, and compression neuropathy of digital nerves from the bowstring. Chronic injuries included bilateral medial epicondylitis and median nerve compression at the wrist, de Quervain's tenosynovitis, and median nerve compression at the elbow. Essential measures for archery safety include use of archery protective gear, use of a light-weight bow, conditioning of the forearm flexor mus…

Chronic stress injuries of the elbow in young gymnasts.

The radiological changes of chronic stress injuries of the elbow in 19 adolescent elite gymnasts are reported. The principal abnormalities were avascular necrosis of the capitellar epiphysis (Panner's disease) (one patient), post-traumatic subarticular necrosis (osteochondritis dissecans) of the capitellum (six patients) and the medial articular eminence of the distal humerus (one patient). Flattening and anterior depression of the radial head epiphysis with an underlying metaphyseal notch associated with an epiphyseal cleft (three patients), which, in one patient who continued training, devel…

Total replacement for post-traumatic arthritis of the elbow.

Fifty-three of 55 consecutive elbow replacements for post-traumatic arthritis were followed for a minimum of two years (mean 6.3, range 2 to 14.4). The patients presented difficult management problems, having undergone an average of two previous operations per joint; 22 joints had suffered prior complications; 18 had less than 50 degrees of flexion and six were flail. One of three versions of the Coonrad prosthesis was employed in all. During the follow-up period, 10 patients underwent 14 revision procedures for aseptic loosening; 38 elbows are currently without progressive radiolucent lines. …

The use and effectiveness of elbow splints in the Rett syndrome.

Incessant hand to mouth movements are often noted as part of the movement disorder of the hands in the Rett syndrome (RS). Elbow splints were designed to inhibit this behavior and prevent further injury to the hands. A study of eight RS patients was conducted to determine the effectiveness of splinting. Results were very positive and indicated increased socialization, increased interaction with the environment, decreased hand to mouth movements, and decreased hand wringing behavior.

[Hemophilic osteoarthropathy with special reference to the elbow joint].

Hemophilia is a rare disorder, whereby recurrent bleedings into the joint can result in osteoarthropathy. Radiological changes consist of osteoporosis, enlargement of the epiphyses, irregularity of the subchondral bone surface, narrowing of the joint space, cysts, erosions, joint incongruence and joint deformity. The earlier and the more frequent bleedings have occurred that have not been treated adequately, the more of the changes mentioned above are present. In children, osteoarthropathy of the elbow is present in only about 50% of cases, and in the remaining cases the degree is mostly minim…

[Aseptic necroses and osteochondrosis dissecans of the elbow joint].

In our opinion, complete (Panner's disease) and incomplete (osteochondrosis dissecans; OD) necrosis of the elbow epiphysis form a nosologic unit. A review of the localization, pathogenetic hypotheses, symptoms, diagnosis and treatment of aseptic necrosis and OD of the elbow is presented with reference to the older and, especially, the current literature. In contrast to the procedures reported in the literature, we performed subchondral focus drilling in 10 patients over 10 years of age suffering from Panner's disease, with good functional results. Operative therapy was performed on 50 elbow af…

Metastatic skeletal disease distal to the elbow and knee.

Metastatic disease of bone is the most common example of "bone cancer." The incidence of skeletal metastases varies from 6% to 85% depending on the primary tumor and the method of evaluation. Axial skeletal involvement is more common than appendicular involvement, and metastatic lesions below the elbow and knee are relatively uncommon. Fifty-seven autopsy subjects had had clinical evidence of metastatic involvement below the knee and elbow with an incidence of 7% (57/827). The lung was the most common primary site, and the tibia was the most common bone affected. Pain, local soft-tissue extens…

Pseudarthrosis of the ulna and osteoarthritis of the elbow. A case report.

A 10-year-old boy who sustained a fracture through a cyst in the ulna later developed a pseudarthrosis. During 13 years' follow-up the characteristic short bowed forearm developed but, instead of the more usual spontaneous dislocation of the head of the radius, the patient also developed the previously unreported complication of osteoarthritis of the elbow.

Arthroscopy of the elbow. Diagnosis and extraction of loose bodies.

Arthroscopy of the elbow was performed in 35 cases of suspected loose bodies. Loose bodies were found in 13 cases. Preoperative radiographs were falsely negative in two cases and falsely positive in 12. In ten cases arthroscopic removal was attempted; it succeeded in six cases. When no loose bodies were found, the most common diagnosis was degenerative joint disease. Arthroscopy is recommended for diagnosis and treatment of loose bodies in the elbow joint.
