BACKGROUND: Despite an improved understanding of coronoid anteromedial facet (AMF) fractures, the optimal treatment protocol and technique have not yet been established. The goals of the study were to describe the characteristics of AMF fractures, to suggest a surgical strategy, and to report the outcomes after treatment according to this protocol.
METHODS: This was a retrospective study of 19 patients with AMF fractures between 2010 and 2012. Eight patients were excluded because of secondary olecranon fracture, radial head fracture, and elbow dislocation, leaving 11 patients with isolated AM

BACKGROUND: Radial head fractures are the most common type of elbow fracture and can severely affect the function of the elbow. There is uncertainty and controversy about when surgery is indicated as well as what type of surgical intervention is best.
OBJECTIVES: To assess the effects of surgical interventions for treating radial head fractures in adults. We aimed to compare surgical versus non-surgical treatment, and different methods of surgical intervention.
SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (May 2012), the Cochrane Central R

BACKGROUND: Open reduction and rigid internal fixation has become the generally accepted method of treatment for displaced fractures of the olecranon in order to allow early mobilisation and to prevent contracture of the elbow. Comminuted fractures of olecranon are unstable, therefore, bone graft and tension band wiring are supposed to give good stability. Here we give an account of our experience with this procedure.
METHODS: We treated ten patients with comminuted fractures of the olecranon by multiple tension-band wiring and a graft from the iliac crest between 1999 and 2002 at Ayub Teachi

The authors propose a novel surgical technique for complex fractures of the elbow necessitating a radial head arthroplasty (RHA) that spares additional soft tissue morbidity and allows for accurate sizing of the radial head. A 56-year-old woman presented with a transolecranon fracture-dislocation of the left elbow. Through a posterior transolecranon approach, there was excellent access to the intramedullary canal for insertion of a RHA to the proximal radius by reflecting the proximal ulna fragment cephalad and performing a shotgun maneuver that hyperflexes the elbow. Also, accurate sizing of

PURPOSE: To investigate the sensation in the hand after forearm cast removal in children.
METHODS: The study group included 33 consecutive children who were treated nonoperatively for a forearm fracture at our center over a 1-year period. The children were asked to report any sensation in the ipsilateral hand after cast removal, and the findings were analyzed against background and fracture-related data.
RESULTS: The patients ranged in age from 6 to 14 years (median 10.00 years). Seventeen had been immobilized in an above-elbow cast and the remainder in a below-elbow cast. All children treat

BACKGROUND: Varus posteromedial rotatory instability is a relatively rare elbow injury, that has been infrequently reported in published literature. We intended to evaluate the outcomes of surgical management of this rare injury with anteromedial coronoid fixation, and, in selected patients, lateral ulnar collateral ligament (LUCL) repair.
METHODS: Between 2017 and 2020, we identified 12 patients with anteromedial coronoid fractures, and a varus posteromedial rotatory instability, who underwent surgery for fixation of the coronoid fracture, with or without LCL repair. All the included patient

Varus posteromedial rotatory instability refers to one of the complex elbow fracture-dislocation caused by anteromedial coronoid fracture with disruption of lateral collateral ligament (LCL). Recent clinical and biomechanical studies have demonstrated that this unstable complex injury resulted in incongruence of joint, which could lead to early posttraumatic arthritis. With reports of poor result after conservative treatment, surgical treatment including anteromedial fixation and LCL repair has been strongly recommended to achieve stable joint. This case series describes three patients with an

Supracondylar fracture of the humerus is the most frequently encountered fracture around the elbow in children. Avascular necrosis is an uncommon complication after supracondylar fracture, but fishtail deformity after avascular necrosis of the trochlea is well known. However, transient osteochondrosis of the capitellum after a supracondylar fracture has not been previously reported in the English literature except 1 case abstract. The authors report their experience of transient osteochondrosis of the capitellum in a 3-year-old boy after the closed reduction and percutaneous pinning for a disp

BACKGROUND: Pediatric musculoskeletal (pMSK) radiograph interpretations are common, but the specific radiograph features at risk of incorrect diagnosis are relatively unknown.
OBJECTIVE: We determined the radiograph factors that resulted in diagnostic interpretation challenges for emergency physicians (EPs) reviewing pMSK radiographs.
METHODS: EPs interpreted 1850 pMSK radiographs via a web-based platform and we derived interpretation difficulty scores for each radiograph in 13 body regions using one-parameter item response theory. We compared the difficulty scores by presence or absence of

Introduction Supracondylar fractures are the predominant type of pediatric elbow fractures. The usual mechanism of injury is falling over the hand with the elbow extended and the hand open. The management of these fractures encompasses a range of treatment options, and their goal is to recover the anatomy and achieve a stable contact area between them. There are some controversies on the management of these injuries mainly focused on those that present some degree of displacement. A review and analysis of the current treatment at our institution and a comparison with the guidelines suggested b…

The radial head is pivotal in the functional anatomy of the elbow and forearm. It is an important stabiliser, together with the collateral ligaments and the processus coronoideus. Due to the pivotal importance, optimal therapies are essential for the function of elbow and forearm, independently of trauma severity. The present article presents current diagnostic testing, classifications and treatment options for radial head fractures.

Posttraumatic instability of the distal radioulnar joint (DRUJ), occurring in isolation or in association with fractures of the radius, is well-documented. Few reports have identified simultaneous ipsilateral DRUJ instability and elbow dislocation. The authors of the present study describe a case of persistent radiocapitellar subluxation after closed reduction of an elbow dislocation, secondary to irreducible volar subluxation of the DRUJ.

Olecranon fractures are seen substantially more often in children than in adult and are potentially occult in nature. We noticed this fracture pattern in two cases of delayed Monteggia fracture. Our study purpose was to confirm whether olecranon fracture is associated with radial head subluxation or annular ligamentous displacement in children. In addition, we have developed an effective and simple approach to avoid serious treatment-related complications as much as possible. Forty-eight pediatric patients with olecranon fracture and normal radiocapitellar alignment on radiography were enrolle

Terrible triad injuries are typically treated surgically to restore elbow stability, as the radial head acts as a secondary stabilizer to valgus stress, while the coronoid provides stability against posterior elbow dislocations. The lateral ulnar collateral ligament (LUCL) is also commonly injured in terrible triad of the elbow injuries, and if not repaired, leads to posterolateral rotatory instability. Depending on the fracture pattern and size, the radial head fracture may be treated with open reduction internal fixation (ORIF), arthroplasty, or excision, whereas the coronoid fracture is mos

Total elbow arthroplasty (TEA) with the GSB III prosthesis was performed in 32 patients (36 elbows) with rheumatoid arthritis between 2001 and 2009. At final follow-up, 31 patients (35 TEAs) were available for clinical and radiological evaluation. The mean follow-up period was 6.3 (2.0-10.3) years, with a minimum follow-up of 2 years. The mean Mayo elbow performance score was significantly improved from 48 points preoperatively to 83 points at final follow-up. The radiographic loosening rate was 14.3% for humeral components and 5.7% for ulnar components. There were 4 cases of intraoperative fr

Competitive and recreational athletes sustain a wide variety of soft tissue, bone, ligament, tendon and nerve damage to their upper extremities. Most such injuries are related to direct trauma or repetitive stress, and account for a significant amount of 'down time' for athletes participating in a wide range of sports, particularly those in which the arm is utilised for throwing, catching or swinging. Overuse injuries to the elbow include musculotendinous injuries, ulnar nerve injuries and ligamentous injuries. Osteochondrol lesions of the capitellum and posterior impingement injuries in the j

BACKGROUND: Radial head fractures are among the most common elbow injuries in adults. The choice between osteosynthesis and radial head arthroplasty (RHA) remains controversial, particularly in comminuted or unstable fracture patterns. This study aims to compare functional and radiographic outcomes, complication rates, and patient satisfaction following osteosynthesis or arthroplasty in the surgical management of radial head fractures.
METHODS: A retrospective cohort of 35 patients treated surgically for radial head fractures between 2018 and 2022 was analyzed. Fractures were classified using
