Core stabilisation reduces compensatory movement patterns in patients with injury to the arm: a randomized controlled trial.

OBJECTIVE: To investigate the effects of adding core stabilisation exercises to traditional rehabilitation in patients with arm injuries. DESIGN: Randomized controlled trial. SETTING: Outpatient clinic. SUBJECTS: Twenty-seven patients with elbow and wrist injuries were randomized to a stabilisation or control group. INTERVENTIONS: The stabilisation group received core stabilisation training and traditional arm rehabilitation and the control group received traditional arm rehabilitation alone for three days/week for six weeks. MAIN MEASURES: Pre- and post-treatment assessments comprising a

[Medial epicondylitis. Etiology, diagnosis, therapeutic modalities].

Medial epicondylitis is rather uncommon, less frequent than external epicondylitis. For this reason, the diagnosis is thought of rather late. While taking the history, one should try to find out the possible causative effects. Symptoms of irritation of the cubital nerve, which are present in one out of five cases should be looked for. Several sports such as baseball, javelin or weight throwing, volleyball, climbing, tennis, golf, which need a strong flexion of the hand and fingers can induce this condition. However, in more than half of our patients, sports or professional activities were not

Mechanical failure of the Coonrad-Morrey linked total elbow arthroplasty: A case report.

Semiconstrained (linked design) total elbow arthroplasty is indicated in a wide variety of cases. Long-term survival is better than with non-linked prostheses. However, mechanical failure of the hinge mechanism is a complication that may occur during follow-up. We report a case of failure of the axle assembly of a Coonrad-Morrey elbow prosthesis 8 years after implantation for nonunion of a supracondylar distal humerus fracture. Initial revision surgery included changing the axle and the polyethylene bushings. Revision surgery was necessary 1 year later when the axle failed again. A custom-desi

Orthopedic pitfalls in emergency medicine.

Acute orthopedic problems make up a large part of everyday emergency department practice. Misdiagnosis of these injuries often results from failure to consider certain clinical entities as a cause of the patient's complaints, and may result in unnecessary complications for the patient. Indeed, missed orthopedic injuries are the leading cause of malpractice claims in emergency medicine. The orthopedic injuries that most notoriously escape detection by the primary care physician are closed tendon injuries of the hand, carpal bone injuries, occult fractures about the elbow, femoral neck fractures

Terrible Triad Elbow Fracture Dislocation with Ipsilateral Triceps Avulsion and Radial Shaft Fracture: A Case Report.

CASE: We report an unusual combination of injuries, terrible triad elbow fracture dislocation with ipsilateral avulsion of the triceps, and radial shaft fracture in a 23-year-old man. He was managed surgically and had good postoperative outcomes. CONCLUSION: Evaluation of the triceps tendon avulsion injury is difficult in this context, such as problems evaluating the rotator cuff injuries after a shoulder fracture dislocation. Because of the diagnostic difficulty, a high index of suspicion is necessary.

[Inveterated simultaneous dislocation of the proximal and distal radioulnar joints without an associated fracture].

The authors present an unusual case of the inveterated simultaneous dislocation of the proximal and distal radioulnar joints without an associated fracture in a 55-years-old man. Both dislocations persisted 4 months after injury and primary unsuccessful open reduction of the radial head. The left forearm was fixed in rigid full supination of 90 degrees and no rotational movements of the forearm were possible. Darrach's procedure and radial head resection resulted in 135 degrees of the forearm rotation (from 0 degrees to 90 degrees of supination, from 0 degrees to 45 degrees of pronation) witho

Bilateral elbow dislocations with intra-articular displacement of the medial epicondyles.

Described is a case of bilateral epicondylar entrapment following fracture dislocations of elbows in a 13-year-old girl who fell while performing on parallel bars. Close manipulative reduction was unsuccessful. Open reduction and internal fixation yielded excellent results. Ulnar nerves were not transposed anteriorly as has been recommended by some authors. Long-term follow-up in five previous cases has shown no evidence of tardy ulnar nerve palsy. The need for transposing the ulnar nerve is questioned.

Effect of Obesity on Complication Rate After Elbow Arthroscopy in a Medicare Population.

PURPOSE: To use a national insurance database to explore the association of obesity with the incidence of complications after elbow arthroscopy in a Medicare population. METHODS: Using Current Procedural Terminology (CPT) and International Classification of Diseases, 9th Revision (ICD-9) procedure codes, we queried the PearlDiver database for patients undergoing elbow arthroscopy. Patients were divided into obese (body mass index [BMI] >30) and nonobese (BMI <30) cohorts using ICD-9 codes for BMI and obesity. Nonobese patients were matched to obese patients based on age, sex, tobacco use, dia

Association of Coronoid Height Loss with Functional Outcomes in Terrible Triad Injuries Treated Using a Selective Fixation Algorithm.

Background: The purpose of this study was to assess whether coronoid fragment size, expressed as coronoid height loss, is associated with functional outcomes in terrible triad injuries treated using a selective coronoid fixation algorithm based on intraoperative elbow stability. Methods: This retrospective cohort study included adult patients with terrible triad injuries treated between 2016 and 2022. All patients underwent preoperative computed tomography evaluation with standardized measurement of coronoid height loss and fracture classification using Mason and Regan-Morrey/O'Driscoll system

Radiographic loss of contact between radial head fracture fragments is moderately reliable.

BACKGROUND: Loss of contact between radial head fracture fragments is strongly associated with other elbow or forearm injuries. If this finding has adequate interobserver reliability, it could help examiners identify and treat associated ligament injuries and fractures (eg, forearm interosseous ligament injury or elbow dislocation). QUESTIONS/PURPOSES: (1) What is the interobserver agreement on radiographic loss of contact between radial head fracture fragments? (2) Are there factors associated with the observer such as location of practice or subspecialization that increase interobserver rel

Outcomes and challenges in the surgical treatment of trans-olecranon fracture-dislocations: A case series study.

INTRODUCTION: Transolecranon fracture-dislocations represent a complex subset of elbow injuries characterized by concomitant fractures of the olecranon, coronoid process, and radial head, often associated with posterior dislocation. These injuries pose significant surgical challenges due to their inherent instability and high risk of poor functional outcomes. Despite previous studies describing different fixation techniques, there is no standardized surgical protocol to optimize outcomes. This study aims to evaluate the clinical and functional results of a structured surgical approach in the m…

Terrible triad injuries are no longer terrible! Functional outcomes of terrible triad injuries: a scoping review.

BACKGROUND: The terrible triad injury (TTI) of the elbow is a combination of a posterolateral dislocation of the elbow joint combined with fractures of the radial head and coronoid process most often caused by a fall on an outstretched hand. The injury pattern was named for its poor outcomes and high complication rates following surgical repair, but increased understanding of elbow anatomy and biomechanics has led to the development of standardized surgical protocols in an attempt to improve outcomes. Most existing literature on terrible triad injuries is from small retrospective cohort studie

A Novel Reconstruction Technique for Chronic Distal Biceps Tendon Rupture: A Case Report.

CASE: A 71-year-old otherwise healthy man presented with an 8-week history of elbow pain and weakness in both elbow flexion and forearm supination. Magnetic resonance imaging revealed complete rupture of the distal biceps tendon insertion associated with 65 mm of proximal retraction. At 10 weeks after initial injury, the patient underwent a novel reconstruction technique using a periosteal flap from the iliac crest; subsequently, all symptoms resolved. CONCLUSION: Chronic distal biceps tendon injuries can be reconstructed safely and effectively using a periosteal flap from the iliac crest tha

Arthroscopic-assisted Fixation of a Nonunited Coronoid Stress Fracture in a Competitive Gymnast: A Case Report.

CASE: Gymnasts may suffer elbow injuries due to repetitive loading. However, coronoid stress fractures remain uncommon. We report a case of a high-level female competitive gymnast with a nondisplaced coronoid stress fracture that failed to heal with conservative management. Arthroscopic-assisted in situ fixation of the coronoid nonunion was performed, and the gymnast returned to competition after the fracture healed. CONCLUSION: Coronoid stress fractures should be considered in gymnasts presenting with chronic elbow pain. If nonoperative treatment is unsuccessful, arthroscopic fixation can be…

[Elbow dislocation with ipsilateral proximal radial shaft fracture and radial head dislocation].

Elbow dislocation with ipsilateral proximal radial shaft fracture and dislocated radial head is a rarely described injury. In this article we present the case of a 23-year-old man with this injury. After the initial diagnostics, the radial shaft fracture was osteosynthetically fixed, whereby the anatomical positions of all parts of the elbow joint were correctly aligned and the medial collateral ligament was reconstructed. After 4.5 months the radial shaft fracture was healed with nearly complete functional recovery of the upper extremity. Thus, a good outcome can be expected when all aspects

An elbow dislocation in a child with missed medial epicondyle fracture and late ulnar nerve palsy.

Elbow dislocations associated with a medial epicondyle fracture and ulnar nerve palsy are uncommon injuries. We present the case of an 11-year-old girl with an elbow dislocation treated by closed manual reduction. The medial epicondyle fracture was missed initially until she developed an ulnar nerve palsy 2 months later. Intraoperatively we discovered the bony epicondyle piercing the joint capsule and compressing the ulnar nerve. Removal of the bony fragment relieved her symptoms and she returned to normal activities at 1 year follow-up. We would like to highlight this rare occurrence and pres

Successful closed manipulation of a pure lateral traumatic dislocation of the elbow joint using a modified Stimson's technique: a case report.

INTRODUCTION: Pure lateral elbow dislocation is rare, and a successful closed reduction is even rarer. Reduction can be hindered by swelling, soft tissue interposition or associated fractures. CASE PRESENTATION: We present a pure lateral traumatic dislocation of the elbow joint in a 40-year-old man. This was successfully manipulated and reduced in casualty using a modification of the gravity-aided 'hanging arm' technique originally described for shoulder dislocations by Stimson. CONCLUSION: We strongly recommend the use of this simple technique in these rare yet difficult injuries, in order

[Arthroscopy of the wrist and elbow joint].

Arthroscopy of the wrist and elbow is a helpful diagnostic procedure in patients with chronic disability not demonstrated by clinical and radiological examinations. At the wrist, especially triangular fibrocartilage, chondral or ligamentous defects can often not be demonstrated by other tests. Arthroscopy of the wrist allows examination of more than a dozen separate articular surfaces and the function of eight discrete joints. The geometry of the elbow joint limits intra-articular inspection and the use of instruments. The risk of neurovascular injuries is great and depends on exact puncture s

The Usefulness of Dynamic Ultrasonography in Nursemaid's Elbow: A Prospective Case Series of 13 Patients Reconsideration of the Pathophysiology of Nursemaid's Elbow.

BACKGROUND: Nursemaid's elbow is a common musculoskeletal disorder among children under 5 years of age. However, diagnostic imaging to confirm a nursemaid's elbow diagnosis is still unavailable. Through the use of a high-frequency ultrasound probe, we determined the etiology and possible pathophysiology of nursemaid's elbow. METHODS: Thirteen consecutive patients with the clinical suspicion of nursemaid's elbows were examined. A high-frequency linear array 6 to 24 MHz hockey stick transducer was used to detect small changes (partial eclipse signs) of the radial head in the axial view before a
