Exploring applications of electrically evoked compound action potentials using the vestibulo-cochlear implant.

OBJECTIVES: Vestibulo-cochlear implants are a potential treatment approach for patients with loss of vestibular function. A critical aspect is the development of reliable and objective outcome measures, to assess their efficacy and optimize their performance. As in cochlear implant care and other neural prostheses, electrically evoked compound action potentials (eCAP) can be used to measure the neural response to implant stimulation. eCAPs might provide information about the presence and misalignment of the electrically evoked vestibulo-ocular reflex, and potential vestibulo-cochlear interacti

Implant-friendly MRI of deep brain stimulation patients at 3T.

INTRODUCTION: MRI of patients with deep brain stimulation (DBS) implants presents a significant safety risk due to RF heating. A current measurement that utilizes an inherent imaging artifact was previously used to mitigate induced current along a single electrode using a two transmit-channel system. This study extends it to bilateral DBS configurations and demonstrates a streamlined implant-friendly (IF) optimization in both unilateral and bilateral patients. METHODS: The bilateral IF mode was optimized to reduce RF-induced currents on both electrodes while maximizing the overall B1 and was

Hypersensitivity to material and environmental burden as a possible cause of late complications of cardiac implantable electronic devices.

AIMS: To evaluate whether patients with late complications of pacemakers or implantable cardioverter-defibrillators have hypersensitivity reactions to some of the materials used in generators or in electrodes, or to environmental metal burden. METHODS AND RESULTS: The cohort consisted of 20 men and 4 women (mean age: 62.3 ± 17.2 years) who had a history of late complications of implanted devices. The control group involved 25 men and 8 women (mean age: 64.6 ± 14.0 years) who had comparable devices, but no history of late complications. Lymphocyte transformation test was used to evaluate hyper

Thalamic stimulation for tremor: can target determination be improved?

High frequency stimulation of the ventral intermedius nucleus (Vim) of the thalamus is successfully used for the treatment of postural tremor. Target coordinates are most commonly calculated using a statistical method. Here, we compare a statistical and an individual targeting method, using an histology-based three-dimensional deformable brain atlas which allows localization of the Vim on individual patient's MR images by adaptation of the atlas onto the patient's brain. Twenty-nine consecutive patients had electrodes implanted in the Vim uni-or bilaterally for severe essential tremor. Thirty-

[Successful removal of an infected pacemaker electrode, drifted into the right atrium, by using a lead removal kit].

A 65-year-old male was suffered complications from an infected pacemaker in the left subclavian region. The generator was removed and its electrode was cut at the vascular entry. A new system was implanted on the other side. Two months later, he suffered from persistent fever which required administration of antibiotics for 2 months. 7 months later, the cut end of the infected electrode was found to have drifted into the right atrium. Its removal was performed to prevent a potential exacerbation of the systemic infection. We were able to pick up an end of the electrode using a basket catheter

Retrospective Analysis of Hearing Outcomes of Cochlear Implantation in Patients with Deafness Due to Congenital CMV Infection.

Cytomegalovirus (CMV) infection in pregnant women is one of the most common causes of congenital infection in children. It is often asymptomatic but can lead to serious complications, including progressive sensorineural hearing loss. Profound hearing loss is an indication for cochlear implantation (CI). Electrode impedance and neural response telemetry (NRT) thresholds can be measured to confirm correct electrode placement and speech processor programming. Background/Objectives: The aim of the study is to evaluate the hearing outcome of children with profound sensorineural hearing loss or deaf

Robot-assisted insular stereoelectroencephalography in pediatric drug-resistant epilepsy: accuracy and diagnostic value.

BACKGROUND: Insular epilepsy is a well-known cause of drug-resistant epilepsy (DRE) in the pediatric population. It can be a source of surgical epilepsy treatment failures when not ruled out pre-operatively. Non-invasive methods often provide limited information about its existence, being the invasive methods necessary to diagnose it in the vast majority of cases. The most used is stereoelectroencephalography (SEEG). We report a series of DRE pediatric patients in which insular SEEG was performed to rule out insular epilepsy. METHOD: We performed a retrospective review of pediatric DRE patien

Electrode Spacing and Current Distribution in Electrical Stimulation of Peripheral Nerve: A Computational Modeling Study using Realistic Nerve Models.

Electrical stimulation of peripheral nerves has long been used and proven effective in restoring function caused by disease or injury. Accurate placement of electrodes is often critical to properly excite the nerve and yield the desired outcome. Computational modeling is becoming an important tool that can guide the rapid development and optimization of such implantable neural stimulation devices. Here, we developed a heterogeneous very high-resolution computational model of a realistic peripheral nerve stimulated by a current source through cuff electrodes. We then calculated the current dist

Mapping the current flow in sacral nerve stimulation using computational modelling.

Sacral nerve stimulation (SNS) is an established treatment for faecal incontinence involving the implantation of a quadripolar electrode into a sacral foramen, through which an electrical stimulus is applied. Little is known about the induced spread of electric current around the SNS electrode and its effect on adjacent tissues, which limits optimisation of this treatment. The authors constructed a 3-dimensional imaging based finite element model in order to calculate and visualise the stimulation induced current and coupled this to biophysical models of nerve fibres. They investigated the imp

Early malfunction of transvenous pacemaker electrodes. A three-center study.

A 3-year study by three medical centers has revealed a 1-year electrode malfunction rate of 7.4%; most malfunctions occurred within the first 30 days. The incidence of unavoidable early malfunction (3.2%) fell within the 5% standards suggested by the committee report of the Inter-Society Committee on Heart Diseases. Incidences of obscure cause (3.2%) may be difficult to identify prospectively and may be, to a certain extent, unavoidable. The majority of the malfunctions (4.2%) showed specific clues that indicated that they were preventable. Successful repositioning was achieved on the first at

Some technical issues of vagal nerve stimulation. An approach using a hippocampal formation theta rhythm.

Previously, we have demonstrated that hippocampal (HPC) theta rhythm can be produced, depending on current intensity, directly during vagal nerve stimulation (VNS) or with a time delay following stimulation. This suggests that theta EEG pattern can also be used as a bio-indicator of the efficiency of VNS. In the present study, we focused on three specific, technical issues related to the stimulation procedure of the vagal nerve: i/does the type of the electrode used for VNS and the technique of its implantation affect the parameters of the HPC theta rhythm? ii/does the type of electrode used d

A new anchor electrode design for continuous neuromonitoring of the recurrent laryngeal nerve by vagal nerve stimulations.

PURPOSE: Intraoperative neuromonitoring has the limitation that the recurrent laryngeal nerve (RLN) is still at risk for damage between two stimulations with a handheld bipolar stimulation electrode. The purpose of this study was to establish the vagal anchor electrode for real-time monitoring of the RLN in surgical routine and to be alerted to imminent nerve failure by electromyography (EMG) signal analysis whereby the nerve damage becomes reversible. PATIENTS AND METHODS: This fully implantable electrode has been used in addition to a conventional handheld bipolar stimulation electrode duri

Usefulness of Leksell GammaPlan for preoperative planning of brain tumor resection: delineation of the cranial nerves and fusion of the neuroimaging data, including diffusion tensor imaging.

Leksell GammaPlan (LGP) software was initially designed for Gamma Knife radiosurgery, but it can be successfully applied to planning of the open neurosurgical procedures as well. We present our initial experience of delineating the cranial nerves in the vicinity of skull base tumors, combined visualization of the implanted subdural electrodes and cortical anatomy to facilitate brain mapping, and fusion of structural magnetic resonance imaging and diffusion tensor imaging performed with the use of LGP before removal of intracranial neoplasms. Such preoperative information facilitated choosing t

The future of bariatrics: endoscopy, endoluminal surgery, and natural orifice transluminal endoscopic surgery.

This review considers the evolving role of endoscopic techniques in the treatment of obesity, and developments in minimally invasive procedures in endoscopy, endoluminal surgery and natural orifice transluminal endoscopic surgery (NOTES). Endoscopic approaches that are similar to surgery but less invasive, relying on gastric restriction and/or malabsorption, show some promise. Intragastric injection of botulinum toxin is also considered, as is the role of minimally invasive bariatric endoscopic interventions. Gastrointestinal stimulation with implanted electrodes may have potential in the trea

Effect of CCK-8 on myoelectrical activity of the gastrointestinal tract in the conscious miniature pig.

In conscious miniature pigs, with implanted electrodes in the wall of the antrum pylori, duodenum, jejunum, and ileum, the influence of IV infusions of CCK-8 (17.5 and 175 pM/kg/min) on gastrointestinal myoelectrical activity was measured. Although both doses under study induced a decrease in antral spike activity. only the higher dose resulted in an overall decrease in integrated myoelectrical activity. In the ileum both doses augmented spiking activity during the infusion, but inhibited electrical activity after the end of the infusion. No response was observed in the duodenum and jejunum. T

[Cochlear implantation].

Since the Israeli cochlear implant program was started in June 1989, 74 candidates have been evaluated for implantation. The main causes for hearing loss were meningitis (16 patients), aminoglycoside toxicity (9), congenital (6), viral (e.g. mumps) (6), and middle ear infection. Candidates were rejected mainly due to psychosocial factors (40%) and audiological parameters (30%). 16 postlingual deaf were implanted. In 7 of them (43.7%) the results were excellent, in 4 (25.0%) good, in 3 (18.7%) fair; 1 case is currently in a training program and 1 is a failure. There were 2 major complications:

Magnesium sulfate suppresses electroencephalographic manifestations of CNS oxygen toxicity.

We studied the effects of parenteral magnesium sulfate (MgSO4) administration on electroencephalographic seizures induced by hyperbaric oxygen (HBO) in awake rats. Sixteen rats chronically implanted with electrocorticographic electrodes were preinjected i.p. with either vehicle or 3 mmol/kg MgSO4 (the latter resulted in serum levels of 3.5-5.5 mmol/liter) and then exposed to 6 ATA O2 in a pressure chamber. The time to develop an electric ichtal seizure was measured and compared to that in the same animal receiving the alternate treatment 3 days later. Mean and median times after the magnesium

Phrenic nerve pacing for the treatment of central hypoventilation syndrome - state of the art and case report.

The application of phrenic nerve pacing is an effective method of respiratory support in adults with certain forms of acquired failure of automatic ventilation. The experience with one infant suffering from congenital central hypoventilation (Ondine's syndrome) is summarized in this report. Stimulation electrodes were implanted on the thoracic portion of each phrenic nerve. After a cumbersome period of intensive medical care and conditioning of the phrenic nerve-diaphragm-system, the patient could be weaned off the respirator. Severe upper airway obstruction, however, required tracheostomy. Bi
