Obstetric emergencies as antecedents to neonatal hypoxic ischemic encephalopathy, does parity matter?

INTRODUCTION: Our aim was to investigate the risk of moderate to severe hypoxic ischemic encephalopathy (HIE) by obstetric emergencies, with focus on the distribution of obstetric emergencies by parity, taking the history of a previous cesarean into account. MATERIAL AND METHODS: Population-based cohort study of 692 428 live births at ≥ 36 weeks of gestation in Sweden, 2009-2015. Data were retrieved by linking the Swedish Medical Birth Register with the Swedish Neonatal Quality Register. Therapeutic hypothermia served as surrogate for moderate to severe HIE. Logistic regression analysis was u…

Validation of a new ENT emergencies course for first-on-call doctors.

BACKGROUND: First-on-call ENT cover is often provided by junior doctors with limited ENT experience; yet, they may have to manage life-threatening emergencies. An intensive 1-day simulation course was developed to teach required skills to junior doctors. METHODS: A prospective, single-blinded design was used. Thirty-seven participants rated their confidence before the course, immediately following the course and after a two-month interval. Blinded assessors scored participant performance in two video-recorded simulated scenarios before and after the course. RESULTS: Participant self-rated co…

Vaccine-preventable diseases in humanitarian emergencies among refugee and internally-displaced populations.

Humanitarian emergencies may result in breakdown of regular health services including routine vaccination programs. Displaced populations including refugees and internally displaced persons are particularly susceptible to outbreaks of communicable diseases such as vaccine-preventable diseases (VPDs). Common VPDs encountered in humanitarian emergencies include measles, polio, and depending on geographical location, meningococcal meningitis, yellow fever, hepatitis A, and cholera. We conducted a review of 50 published articles from 2000 to 2015 concerning VPDs in humanitarian emergencies. This a…

EMTALA and patients with psychiatric emergencies: a review of relevant case law.

STUDY OBJECTIVE: Emergency department (ED) care for patients with psychiatric complaints has become increasingly challenging given recent nationwide declines in available inpatient psychiatric beds. This creates pressure to manage psychiatric patients in the ED or as outpatients and may place providers and institutions at risk for liability under the Emergency Medical Treatment and Labor Act (EMTALA). We describe the patient characteristics, disposition, and legal outcomes of EMTALA cases involving patients with psychiatric complaints. METHODS: Jury verdicts, settlements, and other litigation

Neonatal survival in complex humanitarian emergencies: setting an evidence-based research agenda.

BACKGROUND: Over 40% of all deaths among children under 5 are neonatal deaths (0-28 days), and this proportion is increasing. In 2012, 2.9 million newborns died, with 99% occurring in low- and middle-income countries. Many of the countries with the highest neonatal mortality rates globally are currently or have recently been affected by complex humanitarian emergencies. Despite the global burden of neonatal morbidity and mortality and risks inherent in complex emergency situations, research investments are not commensurate to burden and little is known about the epidemiology or best practices …

Anastomotic dehiscence after resection and primary anastomosis in left-sided colonic emergencies.

PURPOSE: There is no consensus about the risk factors for anastomotic failure after elective or emergency colorectal surgery. The purpose of this study was to analyze the factors that may contribute in anastomotic dehiscence. METHODS: A total of 208 patients who underwent left colonic resection and primary anastomosis for distal colonic emergencies were studied. Preoperative and operative variables analyzed for each patient were gender, age, American Society of Anesthesiologists score, comorbidities, indication for surgery, etiology of the disease, presence and grade of peritonitis, preoperat…

Emergencies in the retroperitoneum: assessment of spread of disease by helical CT.

Acute pancreatitis, leaking abdominal aortic aneurysm, and renal trauma frequently occur in the setting of patients with abdominal nontraumatic and traumatic injury; it represents the most urgent conditions that may determine the presence of fluid collections or haematoma in the retroperitoneum. Single spiral CT and multidetector-row CT (MDCT) play an important role in diagnosis of retroperitoneal emergencies, providing useful informations on the type, site, extent and management of the fluid collections. An accurate CT assessment requires the awareness of the existence of dissectable retroper…

Structural and process quality in the management of diabetic emergencies in Germany.

OBJECTIVE: Although acute complications of diabetes account for approximately 3% of all emergency calls, clinically relevant indicators of structural and process quality in the management of diabetic emergencies have not yet been studied. The purpose of this investigation was, therefore, to collect representative data on these indicators for the whole of Germany. METHODS: Standardized questionnaires comprising 20 items were sent to all 312 emergency medical services in Germany. Apart from demographic data, information was obtained about the diagnostic materials and drugs carried by the ambula…

Non-traumatic abdominal emergencies: imaging and intervention in gastrointestinal hemorrhage and ischemia.

Radiologist and radiological techniques play a crucial role in imaging and interventional procedures in gastrointestinal hemorrhage and ischemia. They are involved in emergency situations that may lead to the death of the patient. Therefore correct diagnosis and treatment are absolutely decisive. We have a great potential in tools of diagnosis, with angiography serving not only as the gold standard for diagnosis even offering a potentially effective treatment for various pathologies. Nevertheless, other techniques have developed greatly during the past decade, including in standard protocols f

Acute nontraumatic spinal epidural hematomas. An important differential diagnosis in spinal emergencies.

STUDY DESIGN: The clinical data of five patients with spontaneous spinal epidural hematoma (SSEH) were reviewed. OBJECTIVES: To assess the clinical outcome of patients with SSEH after surgical decompression. SUMMARY OF BACKGROUND DATA: The outcome in SSEH is essentially determined by the timing of the operation. Therefore, early and precise diagnosis is necessary. METHODS: A retrospective analysis of five patients with SSEH was performed. The clinical data were stratified according to the Frankel Score. Special interest was given to the relevance of rapid and exact diagnosis and immediate t

[A contribution of brain imaging techniques to the diagnosis of psychiatric emergencies].

This study was performed in the psychiatric emergency unit of René-Dubos General Hospital (Pontoise, France). Two cases of female patients with motor deficiencies suggesting a diagnosis of conversion disorder because of preceding psychological stressors and lability of symptoms, are presented. In both cases, the diagnosis was reconsidered after the use of RMI for multiple sclerosis in one case, HIV related encephalopathy in the other. Recent studies have shown that organic diseases with a psychiatric presentation represent 1,2 to 4% of patients examined in emergency setting. Criteria for the d

Urologic emergencies. Trauma injuries and conditions affecting the penis, scrotum, and testicles.

Urologic emergencies involving the penis, scrotum, and testicles include priapism, paraphimosis, testicular torsion, and Fournier's gangrene. Familiarity with the signs and symptoms of these conditions can lead to prompt diagnosis. Special care is needed in differentiating testicular torsion from the more common condition of epididymitis. Evaluation of trauma injuries of the genito-urinary tract includes classification (blunt or penetrating), assessment of the extent of injury, and urologic consultation if appropriate.

Hematologic emergencies. Management of transfusion reactions and crises in sickle cell disease.

Two hematologic emergencies are reviewed in this article: transfusion reactions and crises in patients who have sickle cell disease. Transfusion reactions may be due to incompatibility, IgA deficiency, allergy or, rarely, bacterial contamination of the blood product. A major hemolytic reaction due to incompatibility may progress to hypotension and shock. To prevent this type of reaction, blood products should be given only when necessary and attention should be given to eliminating clerical errors, which are responsible for many hemolytic reactions. In patients with sickle cell disease, a pain…
