Attachment and post-traumatic stress disorder in french military personnel: characterization and exploration of biopsychosocial factors - the at-home study protocol.

INTRODUCTION: In military service members, post-traumatic stress disorder (PTSD) often becomes chronic, with symptoms that significantly interfere with daily life and social functioning. These difficulties pose a major challenge for the rehabilitation and professional reintegration of injured service members. Our protocol describes a study that investigates emotional contagion processes in military personnel with PTSD, in relation to attachment style, along with selected biophysiological and psychosocial factors. METHOD: The method compares two groups of French military personnel: one group w

Renal Adverse Events Associated With Ibuprofen Treatment After Hip and Knee Arthroplasties-A Protocol for the Preplanned Substudy of the PERISAFE Trial.

INTRODUCTION: Non-steroidal anti-inflammatory drugs (NSAIDs) are commonly used for postoperative pain management after hip and knee arthroplasties but have been associated with renal adverse effects. The risk may increase when NSAIDs are combined with diuretics and angiotensin converting enzyme (ACE) inhibitors or Angiotensin-II-receptor antagonists. The aim of this substudy is to assess potential renal adverse effects of an eight-day postoperative treatment with ibuprofen in patients undergoing hip or knee arthroplasty during 90-days follow-up. METHODS AND ANALYSIS: This substudy is part of

Investigating haemodynamic changes during vaginal delivery in low-risk pregnant women and the association between haemodynamic and adverse pregnancy outcomes: an ambispective cohort study protocol.

INTRODUCTION: The progressive adaptation of the maternal cardiovascular system throughout pregnancy is essential to maintaining adequate uteroplacental circulation and meeting maternal physiological demands. In recent years, maternal haemodynamic parameters have gained attention as useful indicators for screening and managing pregnancy, particularly for identifying women at high risk for complications and predicting adverse pregnancy outcomes. The aim of this study is to assess the trajectory of haemodynamic parameters during labour, establish reference ranges for different stages of labour an

From morphology to molecular validation: ethical re-evaluation of 3PN-derived embryos.

In assisted reproduction, the presence of three pronuclei (3PN) in zygotes has long been considered a clear indicator of abnormal fertilization. Current international guidelines recommend the systematic discarding of 3PN-derived embryos due to presumed triploidy and the associated risk of molar pregnancy. However, some studies have demonstrated that a proportion of 3PN-derived embryos exhibit a diploid and euploid chromosomal constitution, with reports of healthy live births following the transfer of genetically confirmed diploid 3PN-derived embryos, and no documented cases of molar pregnancy.

Technology-Enabled Upper Limb Rehabilitation for Neurological Impairment: A Feasibility Randomized-Controlled Trial Protocol.

Background. Technology-enabled upper limb (UL) rehabilitation improves therapy intensity and impairment outcomes, however clinical usage remains low and evidence for functional outcomes is limited. While benefits of single-mode technologies have been demonstrated, a combination of technology modes or "hybrid" approach is an emerging option with shoulder to fingertip capability. Investigation of this approach within a hospital setting is warranted to inform occupational therapy practice with neurological patients. Purpose. This study examines feasibility of hybrid technology-enabled UL rehabili

Human Albumin infusion in liver Cirrhosis and overt Hepatic Encephalopathy (HACHE): protocol of an investigator-initiated, open-label, multicentre, randomised controlled trial.

INTRODUCTION: Hepatic encephalopathy (HE), a severe complication of decompensated cirrhosis, is characterised as neurocognitive dysfunction. Emerging evidence suggests the potential role of human albumin infusion for the treatment of HE, but its optimal dosage remains undefined. Therefore, we planned a randomised controlled trial (RCT) to compare the efficacy of human albumin infusion at different dosages in patients with liver cirrhosis and overt HE. METHODS AND ANALYSIS: This RCT will be conducted in the Departments of Gastroenterology or Hepatology of 16 tertiary hospitals in China. Overal

The Hypotension Prediction Index in Free Flap Transplant in Head and Neck Surgery: Protocol of a Prospective Randomized Controlled Trial.

INTRODUCTION: Microvascular free flap surgery is a treatment method for patients with head and neck cancer requiring reconstruction surgery. Patients undergoing this complex, long-lasting surgery are prone to prolonged episodes of intraoperative hypotension, which is associated with increased incidence of postoperative mortality, morbidity, and free flap failure. A new technology recently approved, named the Hypotension Prediction Index (HPI), allows precise hemodynamic monitoring of patients under general anesthesia, with a significant reduction of intraoperative hypotension events. This stud

Rumination, dyadic coping and parenting sense of competence in couples transitioning to parenthood during puerperium: a longitudinal study protocol.

BACKGROUND: The transition to parenthood presents significant psychological and emotional challenges for mothers and their partners. Stress during the puerperium can undermine parenting confidence. Understanding how cognitive processes and couple-level coping mechanisms influence parenting competence is essential for designing effective nursing interventions. METHODS AND ANALYSIS: This longitudinal study will recruit couples from maternity wards during the early postpartum period. Assessments will be conducted at 3 days, 2 weeks and 6 weeks postpartum using validated instruments: the Chinese

Protocol for the efficacy and safety of fecal microbiota transplantation in children with autism spectrum disorder: a prospective single-center, single-arm interventional study.

BACKGROUND: Autism spectrum disorder (ASD) is a neurodevelopmental condition affecting 0.7% of children globally, with 90% experiencing comorbid gastrointestinal (GI) symptoms. Fecal microbiota transplantation (FMT) may modulate ASD symptoms via the microbiota-gut-brain axis (MGBA). METHODS: This open-label single-arm trial enrolls 30 children (2-12 years) with moderate-to-severe ASD, defined as a Childhood Autism Rating Scale (CARS) score of ≥36. Participants receive 3 nasojejunal FMTs (5 mL/kg) over 5 days. The primary outcomes are GI symptom improvement, assessed using the Gastrointestinal

Living with pain and Parkinson's developing an understanding of the impact, trajectory and pain management needs: a qualitative interview study protocol.

INTRODUCTION: Pain is reported as one of the most troubling symptoms for people with Parkinson's (PwP); however, the literature exploring their lived experience of pain and how to manage it is limited. Pain affects PwP at all stages of their condition and can fluctuate and change over time. Therefore, it is pertinent to speak to PwP to understand their experiences of pain to inform the development of tailored behavioural interventions to manage pain. How pain interacts with other Parkinson's symptoms lacks consensus. Gaining a better understanding of this from the perspective of PwP is importa

Immediate versus delayed single blastocyst transfer following the first stimulated IVF cycle in the freeze-all strategy: a study protocol for a randomised controlled trial.

INTRODUCTION: In recent years, the use of frozen embryo transfers (FET) has rapidly increased following the freeze-all strategy due to the advantages of increased maternal safety, improved pregnancy rates, lower ectopic pregnancy rates and better obstetric and neonatal outcomes. Currently, there is still no good scientific evidence to support when to perform FET following a stimulated in vitro fertilisation (IVF) cycle in the freeze-all strategy. METHODS/ANALYSIS: This will be a randomised controlled trial. A total of 828 women undergoing their first FET following their first stimulated IVF c

Individual on Death Row Receiving Electroconvulsive Therapy (ECT) for Catatonia: A Case Report.

Electroconvulsive therapy (ECT) is underused, logistically challenging for those who are justice-involved, and laced with ethical problems for those on death row. Herein we describe a case of a man without history of long-standing psychiatric illness who, after more than 15 years on death row, was hospitalized for altered mental status. After medical stabilization, the altered mental status persisted. On exam, he displayed signs of catatonic stupor with repetitive non-goal-directed motor activity, repetition of words, rigidity, and negativism, among others. The Bush-Francis Catatonia Rating Sc

Do expectations determine postoperative disability in women with endometriosis? Study protocol for a clinical mixed-methods observational cohort study.

INTRODUCTION: Overall, 20%-30% of women with endometriosis report endometriosis-related disability after successful laparoscopy. This indicates a potential impact of psychological factors, such as expectations, on treatment outcomes. It is already known that expectations determine treatment outcomes in various health conditions, such as cardiologic or gynaecology. Therefore, we investigate the impact of expectations and other psychological factors on patients' course of treatment outcomes after laparoscopy. METHODS AND ANALYSIS: A longitudinal mixed-methods study with N=300 women treated at a

Multicentre open label randomised controlled trial of immediate enhanced ambulatory ECG monitoring versus standard monitoring in acute unexplained syncope patients: the ASPIRED study.

INTRODUCTION: Diagnosing underlying arrhythmia in emergency department (ED) syncope patients is difficult. There is a evidence that diagnostic yield for detecting underlying arrhythmia is highest when cardiac monitoring devices are applied early, ideally at the index visit. This strategy has the potential to change current syncope management from low diagnostic yield Holter to higher yield ambulatory monitoring, reduce episodes of syncope, reduce risk of recurrence and its potential serious consequences, reduce hospital admissions, reduce overall health costs and increase quality of life by al

Study protocol for a single-centre randomised controlled trial to investigate the effect of lung recruitment in paediatric patients after cardiac surgery.

INTRODUCTION: A number of published studies have revealed that lung recruitment can improve oxygenation, shorten the duration of mechanical ventilation (MV) and decrease mortality in adults with acute hypoxaemic respiratory failure, especially patients with acute respiratory distress syndrome. However, few articles have assessed lung recruitment in paediatric patients, especially after cardiac surgery. This clinical trial aimed to determine whether lung recruitment can reduce the duration of MV in paediatric patients with hypoxaemic respiratory failure after cardiac surgery. METHOD AND ANALYS

Study protocol: a multicentre, open-label, parallel-group, phase 2, randomised controlled trial of autologous macrophage therapy for liver cirrhosis (MATCH).

INTRODUCTION: Liver cirrhosis is a growing global healthcare challenge. Cirrhosis is characterised by severe liver fibrosis, organ dysfunction and complications related to portal hypertension. There are no licensed antifibrotic or proregenerative medicines and liver transplantation is a scarce resource. Hepatic macrophages can promote both liver fibrogenesis and fibrosis regression. The safety and feasibility of peripheral infusion of ex vivo matured autologous monocyte-derived macrophages in patients with compensated cirrhosis has been demonstrated. METHODS AND ANALYSIS: The efficacy of auto

Timing of evidence-based non-surgical interventions as part of multimodal treatment guidelines for the management of cervical radiculopathy: a Delphi study protocol.

INTRODUCTION: Cervical radiculopathy (CR) is a clinical condition whereby motor, reflex and/or sensory changes such as radicular pain, paraesthaesia or numbness can exist. Conservative management is a preferred first treatment option as the risk-benefit ratio for surgery is less favourable. Systematic reviews and treatment guidelines gather evidence on the effectiveness of non-surgical management of patients with CR from randomised controlled trials, which do not consider the natural course of recovery to modify the management strategy accordingly. The aim of this study is to establish consens
