Incontinence-associated dermatitis: new insights into an old problem.

Incontinence-associated dermatitis (IAD) is a common skin disorder seen in patients with incontinence. Typically IAD presents as inflammation of the skin surface characterised by redness, and in extreme cases, swelling and blister formation. If untreated this usually rapidly leads to excoriation and skin breakdown, which may subsequently become infected by the skin flora. While this is a common condition encountered in all areas of nursing practice, gaps remain in our understanding of the many contributing factors. A lack of standardised definitions of IAD, differences in terminology, and a be

[A multicenter randomized controlled clinical trial of Ligation of the Intersphincteric Fistula Tract Plus Bioprosthetic Anal Fistula Plug in the treatment of chronic anal fistula].

OBJECTIVE: To evaluate the effectiveness and safety of Ligation of the Intersphincteric Fistula Tract Plus Bioprosthetic Anal Fistula Plug (LIFT-plug) in the treatment of chronic anal fistula. METHODS: A total of 239 patients (199 males, 40 females) with chronic anal fistula were recruited from 5 hospitals between March 2011 and April 2013. These patients were randomly assigned to the experimental group (n=119) treated with LIFT-plug or the control group (n=120) treated with LIFT. The follow-up period was 180 days. The collected data included healing rate, the median healing time, the recurre

Empowering patients to self-manage in the context of incontinence.

Chronic diseases cause complex challenges and difficulties for patients, families and health professionals. With the increasing prevalence of chronic diseases, long-term treatment interventions may be required. Health professionals must empower patients to adhere to therapies and develop self-management strategies. The aetiology of bladder and bowel dysfunctions is multi-dimensional with some individuals who present with chronic disease being predisposed to urinary or faecal incontinence. This article will discuss the concepts of compliance, adherence and concordance approaches in the manageme

The LION procedure to the pelvic nerves for treatment of urinary and faecal disorders.

Bladder dysfunctions have been treated for decades through medical treatments or surgical procedures, especially in the context of prolapse conditions and stress incontinence. Over the last decade, sacral nerve modulation (SNM) has been introduced as a further option in the treatment of some urinary and fecal symptoms. Current techniques of percutaneous implantation are limited to superficial extrapelvic nerves that expose patients to lead migration and dislocation or infections, complications that cannot be ignored. Access to endopelvic nerves and plexuses will be mandatory in the future, esp

Bladder and bowel dysfunction and the resolution of urinary incontinence with successful management of bowel symptoms in children.

AIM: To investigate the effect of treating defecation problems on urinary incontinence in children suffering from combined urinary bladder and bowel dysfunction (BBD). METHODS: We established a clinical database from medical records of all children referred to the urinary incontinence and gastroenterology outpatient clinics with BBD. The following variables were extracted: symptoms of constipation, faecal incontinence, urinary incontinence, age at onset of symptoms, treatment, including duration and response. All children went through the same treatment protocol. Faecal disorders were treated

Incontinence: enhancing care in women's prisons.

BACKGROUND: There is no evidence on the prevalence of urinary and faecal incontinence symptoms in women prisoners. AIM: To explore the extent and management of bladder and bowel symptoms to inform prison health services and prison nursing practice. METHOD: An anonymous self-report questionnaire tailored to low levels of English literacy, and administered in one women's prison. RESULTS: Women prisons have a higher reported prevalence of urinary and faecal incontinence, constipation and nocturnal enuresis than community populations; this is an unrecognised health problem. CONCLUSION: Prison

Caesarean section is protective against stress urinary incontinence: an analysis of women with multiple deliveries.

All women who had three elective caesarean sections were selected from a database of 40,000 women delivering between 1977 and 1998, and age-matched with women having three vaginal births. They all completed a (validated) urinary and bowel symptom questionnaire. Women who had vaginal births had a significantly higher prevalence of stress incontinence but not other urinary or faecal symptoms compared with those delivered by caesarean section. The prevalence of faecal incontinence was lower than the prevalence of urinary incontinence. Although the prevalence of faecal incontinence was lower after

Uterine didelphys and occult spinal dysraphism: an unusual case.

This report describes a case of uterine didelphys, lipomeningocele, meatal stenosis, and inguinal hernia presenting in a single individual. This unusual group of birth defects suggests a common mechanism of malformation in tailbud maturation which involves both the müllerian duct and the distal spinal cord.

Moderation of urinary and faecal incontinence with loperamide.

The author used Imodium in a clinical trial for the treatment of urinary incontinence following radical prostatectomies, urinary and faecal incontinence due to carcinoma or cerebral accident in history. The drug significantly improved incontinence and did not cause adverse effects. On the basis of the similarity of innervation the author opines that loperamide also influences the function of detrusor and/or bladder orifice besides exerting an antiperistaltic and anal sphincter influencing action.

Primary outcomes were the treatment success rate (recurrence and healing rates) and incidence of fecal incontinence. RESULTS: A total of 184 patients were included in our study. The average age of the patients was 44 years (range, 16-75 years), and 88.0% were male. Twenty-four (13.0%), 13 (7.1%), and 68 patients (37.0%) underwent operation for recurrent fistula, …

Fever associated with machine activation after sacral neuromodulation: Case report.

INTRODUCTION AND IMPORTANCE: Sacral neuromodulation (SNM) is commonly used in the treatment of refractory overactive bladder (OAB), non-obstructive urinary retention (NOR) and fecal incontinence. Here, we report an atypical symptomatic case to enrich the limited international case series. CASE PRESENTATION: We report a case of a male patient with cauda equina nerve injury left over from a traumatic injury and dysfunction of urinary and fecal functions who, after undergoing phase I sacral nerve stimulator placement, developed fever when the machine was switched on, and the symptoms resolved wh…

Translation and cultural adaptation of the Hirschsprung's Disease/Anorectal Malformation Quality of life Questionnaire (HAQL) into Swedish.

PURPOSE: Children with anorectal malformation or Hirschsprung's Disease (HD) often have functional problems with constipation or incontinence. The Hirschsprung's Disease/Anorectal malformation Quality of life Questionnaire (HAQL) developed in the Netherlands is a disease-specific instrument measuring the quality of life (QoL) of children and adolescents with fecal incontinence. HAQL includes several domains with questions concerning diet, laxatives, constipation, diarrhea, urine and fecal incontinence, in addition to social and emotional functioning, body image, and physical symptoms. The purp…
