The effect of medullary fill on varus collapse in AO 31A3 intertrochanteric (reverse obliquity) fracture treated with cephalomedullary nails.

OBJECTIVE: Cephalomedullary nails are tools commonly used for surgical fixation in proximal femoral fractures. The most common complications in their use are varus collapse and screw cutout of the femoral head. The objective of this study is to examine the effects of intramedullary nail movement on varus collapse and screw cutout. MATERIALS AND METHODS: The study was conducted on 70 composite femur models treated with PFNA. We divided the femurs into 4 groups based on the differences in nail diameter, fracture type and filling of the distal intramedullary area. All femurs were exposed to axia

Femoral head necrosis: A finite element analysis of common and novel surgical techniques.

BACKGROUND: Femoral head necrosis is a common cause of secondary osteoarthritis. At the early stages, treatment strategies are normally based on core decompression techniques, where the number, location and diameter of the drilling holes varies depending on the selected approach. The purpose of this study was to investigate the risk of femoral head, neck and subtrochanteric fracture following six different core decompression techniques. MATERIALS: Five common and a newly proposed techniques were analyzed in respect to their biomechanical consequences using finite element analysis. The geometr

Effects of raloxifene on development of the methotrexate-induced changes in bone mechanical properties of male rats.

Methotrexate, a cytostatic and immunosuppressive drug, has been reported to deteriorate the osseous system. Raloxifene, a selective estrogen receptor modulator, is used in the prevention and treatment of postmenopausal osteoporosis. There is a lack of data on possible ways of preventing the unwanted skeletal effects of prolonged immunosuppressive therapy. The aim of the present study was to investigate the effects of raloxifene on mechanical properties of the femur in male rats administered methotrexate. The experiments were carried out on mature male Wistar rats, which were divided into 6 gro

Finite element analysis of femoral neck strains during stair ascent and descent.

For older population, a better understanding of the hip joint loading environment is needed for the prevention of hip pain, and the reduction of the stress fractures and fall risks. Using the motion analysis and inverse dynamics methods, combined with musculoskeletal modelling, static optimization, and finite element (FE) femur model, the difference of femoral neck strains between stair ascent vs. descent, young vs. older populations was compared. A two-way repeated-measures MANOVA was applied to test the effect of age and stair direction on the femoral neck strains. The strains at the femoral

Premature femoral neck physeal closure in Perthes' disease.

One hundred premature femoral neck physeal closures in 430 hips with Perthes' disease have occurred in two patterns central and lateral. Abnormal physeal growth can be demonstrated early by a narrowed physeal plate with overlying avascular epiphysis and marked metaphyseal reaction below. Subsequently, a bony bridge forms between the metaphysis and epiphysis. If the physeal closure is central, the mature hip will have a short femoral neck, a relatively round femoral head, a trochanter that has overgrown the femoral head, a short leg, and a mildly deformed acetabulum. If the physeal closure is l

Bilateral simultaneous oblique MR imaging of the proximal femur.

Oblique axial and oblique coronal MR images parallel to the femoral neck provide more valuable information concerning the proximal femur than orthogonal images. The main disadvantage of the unilateral oblique imaging is that the imaging time is significantly increased when both hips need to be imaged. If the bilateral oblique imaging is performed simultaneously, this increase in imaging time is eliminated because the scanning time for the bilateral oblique imaging and the orthogonal imaging is the same.

The treatment of progressive coxa vara in children with bone softening disorders.

Fractures of the proximal femur frequently occur in children with osteogenesis imperfecta or fibrous dyplasia and may lead to progressive coxa vara and a "shepherds crook" deformity. We have found existing methods of fixation of the femoral neck in small children to be unsatisfactory. We have devised a method of fixation using a small signal arm nail. With a mean follow-up of 9.3 years there was a successful outcome in all 11 hips treated.

Vertical shear fractures of the femoral neck. A biomechanical study.

A biomechanical cadaver study was performed to compare the strength and stability of three cannulated cancellous lag screws with a sliding hip screw for fixation of a vertically oriented fracture of the femoral neck (Pauwels Type III). Using eight matched pairs of human cadaveric femurs, vertically oriented femoral neck osteotomies were created, reduced, and randomized to one of the two fixation methods. The constructs were tested with incremental axial loading from 100 N to 1200 N and cyclical loading at 1000 N for 10,000 cycles; fracture displacements and ultimate load to failure were determ

Osteonecrosis of the femoral head caused by bone marrow blockage: a case report.

Interruption of the blood flow may occur in intra-osseous arteries within the femoral head. We report a 72-year-old woman who developed osteonecrosis of the femoral head 11 months after surgery involving massive cementing of a segmental distal femoral rotating hinge prosthesis to treat nonunion of the distal femur. The bone cement filled the cavity up to the femoral neck and the superolateral portion of the femoral head, blocking the bone marrow.

Elevated 1-Year Mortality Rate in Males Sustaining Low-Energy Proximal Femur Fractures and Subgroup Analysis Utilizing Age-Adjusted Charlson Comorbidity Index.

INTRODUCTION: Low-energy proximal femur fractures are common in the aging population and the ability to identify patients at increased mortality risk provides surgeons information to improve informed decision-making with patients and families. We evaluated for gender differences in 1-year mortality after sustaining low-energy proximal femur fractures with subgroup analysis to identify the impact of fracture location, age, and comorbidities on mortality. MATERIALS AND METHODS: Patients ≥40 years of age sustaining a low-energy proximal femur fracture identified at our institution between Januar

Study of three-dimensional morphology of the proximal femur in developmental adult dysplasia of the hip suggests that the on-shelf modular prosthesis may not be an ideal choice for patients with Crowe type IV hips.

PURPOSE: The purpose of this study was to investigate the three-dimensional morphological features of the proximal femur of developmental dysplasia of the hip (DDH). METHODS: From January 2012 to December 2014, 38 patients (47 hips) of DDH were admitted and 30 normal hips were selected as controls. All hips from both groups were examined by CT scan. CT data were imported into Mimics 17.0. Three-dimensional models of the proximal femur were then reconstructed, and the following parameters were measured: neck-shaft angle, neck length, offset, height of the centre of femoral head, level of isthm

Definition of ideal configuration for femoral neck screw fixation in older people.

Femoral neck fractures are the most common injuries encountered by older individuals, and they are associated with high mortality and morbidity. Internal fixation to femoral neck fracture with cannulated screws placed with a configuration of an inverted triangle remain a feasible and effective treatment for femoral neck fractures. The objection of this research was to evaluate the femoral neck morphology, especially the shape of the femoral neck in Chinese people to find the optimal screw position and interval between the screws. 96 consecutive normal subjects without any previous proximal fem

Giant cell tumour of the femoral neck: Failure of curettage-cavity filling cementation with screw fixation, a case report.

BACKGROUND: Giant cell tumours are common primary long bone tumours with femoral neck locations infrequent and notorious for pathological fractures. Treatment with simple curettage often results in local recurrence. Aggressive treatment that combines tumour resection with cement filling and internal fixation aims at preserving native joint function. CASE REPORT: The authors intend to illustrate the short falls of such conservative approach through a case report of a femoral neck giant cell tumour in a 37 year old patient. Patient had undergone curettage-cavity filling with screw fixation for

Ipsilateral fracture of the femoral neck and shaft, treatment by reconstruction interlocking nail.

This study involved nine patients with ipsilateral fractures of the neck and shaft of the femur. They were all male with an average age of 28.5 years. All of the fractures resulted from high-energy trauma. The neck fracture was initially missed in one case. All fractures were fixed by a Russell-Taylor reconstruction femoral nail. Surgery was delayed for an average of 6.6 days (range 2-21 days). The patients were followed up for an average of 2.1 years. All fractures healed; the average time of union for the neck fracture was 4.2 months (range 3-6 months) and for the shaft fracture, 6.9 months

Making a case for anterior inferior iliac spine/subspine hip impingement: three representative case reports and proposed concept.

Femoroacetabular impingement is typically described as occurring due to a conflict between the femoral head-neck junction and acetabular rim. A prior case report described an open decompression of the anterior inferior iliac spine (AIIS) due to impingement against the proximal femur. AIIS impingement may be developmental or the result of a prior AIIS avulsion or pelvic osteotomy. We describe 3 representative cases with minimum 1-year follow-up treated with an arthroscopic AIIS decompression.

Three-dimensional analysis of the proximal femur in patients with slipped capital femoral epiphysis based on computed tomography.

A three-dimensional (3D) analysis based on computed tomography was performed to study the 3D geometry of the proximal femur in cases of slipped capital femoral epiphysis (SCFE). For this purpose, new interactive software was developed to analyze hip joint geometry using 3D models without pelvis tilting and projected errors. Twenty-two patients, 8 girls and 14 boys, with a total of 30 slipped capital femoral epiphyses, were reviewed. In the affected hips, we observed a reduced femoral anteversion of 7.0 degrees (vs. 12.7 degrees) and a reduced femoral shaft neck angle of 134.2 degrees (vs. 141.
