Distal tibial and fibular epiphyseal fractures in children: prognostic criteria and long-term results in 158 patients.

From 1974 to 1993, 158 injuries of the ankle joint were treated at the Department of Orthopedics and Traumatology of Santobono Children's Hospital. One hundred thirty-two patients were treated conservatively; 26 patients underwent surgical treatment. Fibular fractures of the malleolus without epiphyseal separation or dislocation (68 patients) were excluded from this review. Of the 158 patients, 113 (70%) were available for follow-up. The mean age of patients was 11 years. The mean follow-up was 6 years (range 3-15 years). In this review, the authors compare, based on the degree of epiphyseal s

Neonatal osteofibrous dysplasia: report of two cases.

Skeletal neoplasms of the newborn and neonatal period are infrequently encountered and often misdiagnosed. Osteofibrous dysplasia is a tumor of childhood that may be seen in the newborn. Because of its unique natural history, with and without surgical treatment, and the similarity in radiographic appearance of this tumor to other lesions, a high index of suspicion must be maintained when a tibial lesion is encountered in the newborn. We present two additional cases of neonatal osteofibrous dysplasia and discuss the differential diagnostic considerations.

The free vascularised fibular transfer as a definitive treatment in femoral septic non-unions.

Free vascularised bone transfer (fibula, iliac crest, or rib) is an accepted method of bone grafting in malignant and non-union bone surgery. The vascular microanastomoses have transformed the bone healing by creeping substitution seen after non-vascularised grafting (a long and often insufficient process) into normal healing of the fracture site. The presence of its own vascular support allows bone healing in such compromised circumstances as sclerosis and infection. We present the clinical history of five patients with septic femoral non-unions, in which only the final vascular fibular graft

Double vascularized fibulas for reconstruction of large tibial defects.

Vascularized fibular grafts have proven to have many advantages over nonvascularized transplants for treatment of large segmental bone defects in the extremities. Fibulas are typically impacted into the medullary canal and fixed with wires or screws. Consolidation has often been delayed and full weightbearing was only possible after graft hypertrophy, usually 12 to 18 months after reconstruction. In order to shorten the time of consolidation and to achieve early full weightbearing, the authors propose a sound biomechanical reconstructive concept: a) stable but not devascularizing osteosynthesi

Fractures of the proximal tibial metaphysis in children: review of a series of 25 cases.

Twenty-five cases of fractures of the proximal tibial metaphysis in children are reported. They are divided into three groups: torus fractures, greenstick fractures, and complete fractures. Genu valgum appeared in 12 cases of complete or greenstick fractures. Eleven deformities deteriorated progressively in the year following trauma. Three of these were spontaneously compensated for by growth at the inferior epiphyseal plate. Four corrective osteotomies were performed in the series, but this procedure is not recommended because of the significant risk of recurrence and the possibility of compa

Biomechanical Evaluation of a Novel Stepped Bone Reconstruction Structure for the Mandible Under Traumatic Impact Loading.

OBJECTIVES: In daily life, following mandibular reconstruction surgery, accidental events such as falls and impacts may lead to fracture, displacement, or failure of the reconstructed site. This study investigates the biomechanical behavior of a Stepped Bone Reconstruction Structure (SBRS) when subjected to transient traumatic loads postoperatively. METHODS: On the basis of CT scans, an SBRS model was established. Two magnitudes of impact loads were simulated at 5 different locations: frontal symphyseal, left parasymphyseal, right parasymphyseal, left mandibular angle, and right mandibular an

Is It Possible to Overcompress the Syndesmosis?

The case we present suggests that it might be possible to overcompress the syndesmosis, causing subluxation of the talus within the ankle mortise. A 26-year-old female patient had had a Weber Type C ankle fracture internally fixed with a lateral plate and syndesmosis screws. Despite the fibula appearing well reduced and computed tomography imaging showing a well-aligned fibula within the fibular notch, anteromedial subluxation of the talus was present in the ankle mortise. Examination with the patient under anesthesia revealed a stable syndesmosis fixation; however, talar malpositioning was no

Osteofibrous dysplasia of long bones a new clinical entity.

A new clinico-pathologic entity is described. It is defined as osteofibrous dysplasia of long bones, and is based on twenty two personal observations to which are added seventeen cases from the literature. This dysplasic congenital lesion is clearly differentiated from fibrous dysplasia by clinical, radiographic and histological characteristics and by its clinical course. These features may be summarised as follows: 1) Slight predominance of the male sex. 2) Very early age of onset either at birth or in the first years of life. 3) Site almost exclusively tibial, sometimes also in the fibula. Fibular nailing is associated with fewer complications in high-risk ankle fractures: A propensity score-matched case-control study.

PURPOSE: Ankle fractures are common, with higher complication rates in elderly, comorbid patients and those with complex injury patterns. Minimally invasive techniques such as intramedullary fibular fixation have gained interest for their potential to reduce morbidity. This study compared outcomes of fibular nailing versus plating in high-risk ankle fractures and aimed to identify the patient profile most likely to benefit from intramedullary fixation. METHODS: A retrospective, propensity score-matched case-control study was conducted at a level I Trauma Center. Patients with low-energy rotat

Limb salvage in polytrauma-pushing the boundaries.

This report presents a polytrauma patient with a pelvic ring injury, multiple lower extremity fractures and a mangled left lower leg. After initial vascular repair and a temporising external fixation, the patient's injuries were sequentially addressed. The left tibia was stabilised with an Ilizarov apparatus. Extensive bone and soft tissue damage necessitated the excision of a non-viable tibial shaft segment, enabling the creation of an artificial deformity for extensive temporary soft tissue coverage. Nursing and soft tissue management involved negative pressure wound therapy, copper-ion dres

ALT flap and reverse sural flap for simultaneous soft tissue coverage of both medial and lateral calf wounds in a diabetic patient.

Defects of the lower limb are generally difficult to correct, especially in patients with associated pathologies, and they represent a challenge for the reconstructive surgeon. The sural flap provides considerable versatility for lower limb reconstructions, successfully covering defects located between the knee and the ankle or the heel. Most frequently, this flap is used for defects located in the middle to distal third of the lower limb. The scope of reconstructing defects from the lower limb extremities must be concentrated both on covering the defect, and on functional recovery. When use o

An aggressive case of pseudomyogenic haemangioendothelioma of bone with pathological fracture and rapidly progressive pulmonary metastatic disease: case report and review of the literature.

Pseudomyogenic haemangioendothelioma (PMH) is a rare recently described vascular tumour typically presenting with soft tissue disease in distal extremities of young adults. Multi-focal and multi-layered involvement is commonly recognised. The majority of cases described so far have shown an indolent clinical course and distant metastatic spread is rare. We report a case of PMH in an 82-year-old male diagnosed following a pathological fracture of the distal tibia. Further bone lesions were identified in the fibula, patella and distal femur. The patient was found to have multiple nodules suspici

A durable solution for challenging multidrug-resistant bone infections: a retrospective study on long-term efficacy of the flow-through fibula osteocutaneous flap in limb reconstruction.

BACKGROUND: Infectious bone and soft tissue defects caused by multidrug-resistant bacteria (MDRB) represent infrequent yet challenging medical conditions. Vascularized fibular grafting constitutes a crucial treatment modality in limb reconstruction procedures, offering improved recovery by supplying nutrients and structural support, particularly in large defects and compromised vascularity. The present study assessed the long-term outcomes of flow-through fibula osteocutaneous flaps for reconstructing multidrug-resistant infected bone and soft tissue defects. METHODS: Between January 2015 and

Fracture detection using a low-dose computed tomography paediatric bone fracture phantom: A multi-reader study of radiographers.

INTRODUCTION: Low-dose computed tomography (CT) has been proposed as an alternative approach to investigate suspected physical abuse (SPA). This is an important consideration for children under 1 year of age due to the inconvenience of initial and follow-up imaging in addition to CT of the head. Results from a pilot study using a bone fracture paediatric phantom suggested that fracture detection was inconsistent using low-dose CT. The aim of this study was to evaluate whether those findings were consistent for a larger cohort of observers. METHODS: Nine CT datasets were reviewed by 18 radiogr

Posterior Inferior Tibiofibular Ligament Release to Achieve Anatomic Reduction of Posterior Malleolar Fractures.

We assessed the clinical and radiographic outcomes of posterior inferior tibiofibular ligament (PITFL) release to achieve anatomic reduction of posterior malleolar fractures (PMFs). Nineteen PMFs (>25% of tibial plafond) that could not be reduced after anatomic reduction of distal fibula fractures were managed by PITFL release. The syndesmosis was stressed intraoperatively and by 2 surgeons unaware of the postoperative measurements to increase reliability. The pre- and postoperative fracture gaps and articular step-offs were measured on lateral radiographs of all patients and computed tomograp…

Upper extremity fractures in pedestrian versus motor vehicle accidents: an underappreciated concern.

BACKGROUND: Though pedestrian versus motor vehicle (PVMV) accidents are a common cause of trauma admission and subsequent orthopaedic consult, the prevalence of upper extremity fracture (UEF) in such events and its association with lower extremity injury (LEI) is unknown. We sought to describe UEF in PVMV accident patients at the time of orthopaedic consult. METHODS: A retrospective chart review was conducted for all pedestrian hit by motor vehicle cases for which an orthopaedic consult was performed at Jackson Memorial Hospital between July 2006 and January 2008. Fractures were recorded by l
