Equity and unmet need of non-communicable diseases services in Saudi Arabia using a National Household Survey (2019).

BACKGROUND: Saudi Arabia is implementing a comprehensive health system transformation in health services provision, governance, and financing. Given the high burden of non-communicable diseases (NCD), a key objective of the transformation is to integrate NCD prevention and treatment into primary care. The study objectives were to assess primary care service use for treatment of NCDs, to quantify existing inequities in preventive services utilization, and to identify regional and sociodemographic factors associated with these inequities. METHODS: Using the 2019 Kingdom of Saudi Arabia World He

Somatosensory cortectomy induces motor cortical hyperexcitability and scoliosis: an experimental study in developing rats.

BACKGROUND CONTEXT: Dysfunctions in sensorimotor integration, reminiscent to those described in idiopathic dystonia, have been found in idiopathic scoliosis (IS) and might be involved in its pathogenesis. Studying the effects of experimental disruption of sensory cortex may shed further insight into the etiopathology of IS. PURPOSE: To evaluate whether disruption of central sensorimotor integration through partial ablation of the somatosensory cortex leads to scoliosis in developing rats and to describe the effects of such an intervention on motor cortico-cortical inhibition and facilitation.

Social and cultural features of cholera and shigellosis in peri-urban and rural communities of Zanzibar.

BACKGROUND: Responding to the high burden of cholera in developing countries, the WHO now considers vaccination as a supplement to the provision of safe drinking water and improved sanitation in the strategy for cholera control in endemic settings. Cultural concepts of illness affect many aspects of public health. In the first step of a two-step strategy to examine determinants of cholera vaccine acceptance, this study identified social and cultural features of diarrhoeal illness for cholera control in endemic communities. METHODS: A cultural epidemiological study with locally adapted vignett

Transferable Design Principles for Scalable Mental Health Systems: A Conceptual Framework Informed by Innovations Across African Contexts.

IMPORTANCE: Mental disorders are a leading cause of disability worldwide, and their burden continues to increase across Africa. Despite growing need, access to care remains limited, with treatment gaps reaching up to 90% in some settings. These gaps reflect longstanding constraints in workforce, financing, infrastructure, and sociocultural factors, alongside uneven policy implementation. At the same time, countries and communities across the continent are developing new approaches to mental health care that can complement conventional, specialist-centered models and offer transferable lessons

Global feasibility of cervical cancer elimination: a multidimensional scenario-based modelling analysis across 175 countries.

BACKGROUND: Cervical cancer remains a major cause of preventable cancer mortality worldwide. Progress towards elimination remains uneven, with persistent disparities in health system capacity, governance, and education. Existing models often rely on income classifications, overlooking structural determinants critical for success. We aimed to evaluate the global feasibility of cervical cancer elimination under structurally differentiated policy scenarios. METHODS: We developed a multidimensional scenario-based model integrating 270 structural indicators obtained from WHO, IARC, UN, World Bank,

Ebola Virus Disease in the Era of One Health and Global Preparedness: Evolving Epidemiology, Genomic Surveillance, and Future Challenges.

Ebola virus disease (EVD) remains one of the most severe viral hemorrhagic fevers, with recurrent outbreaks challenging global healthcare systems. This narrative review traces the evolving epidemiology of EVD from the first recognized outbreaks in 1976 to the ongoing 2026 Bundibugyo virus public health emergency in the Democratic Republic of the Congo (DRC) and Uganda. Over nearly five decades, the recognized range of Ebola outbreak contexts and amplification mechanisms has broadened considerably: rural zoonotic spillovers remain the predominant mode of emergence, but the scale and reach of su

Legal status, sex work engagement and inequities in the HIV pre-exposure prophylaxis cascade among migrant women in 11 European countries.

BACKGROUND: Migrant women account for a substantial proportion of new HIV diagnoses in Europe, yet remain underrepresented in HIV prevention research. Little is known about how legal status and national policy context shape engagement with PrEP among migrant women, particularly those without secure documentation. We assessed the PrEP cascade and HIV prevention preferences among migrant women, examining how legal status, sex work engagement and national reimbursement policies drive inequities in access. METHODS: We conducted a community-led, cross-sectional survey (June-July 2025) of 521 HIV-n

Evaluation of the implementation of the Montreal At Home/Chez Soi project.

BACKGROUND: Homelessness and mental disorders constitute a major problem in Canada. The purpose of the At Home/Chez Soi pilot project was to house and provide supports to marginalised groups. Policymakers are in a better position to nurture new, complex interventions if they know which key factors hinder or enable their implementation. This paper evaluates the implementation process for the Montreal site of this project. METHODS: We collected data from 62 individuals, through individual interviews, focus groups, questionnaires, observations and documentation. The implementation process was an

Diversity and critical forces in dental education.

Critical issues that affect dental education and the connection between science and practice include the community, managed patient care, public skepticism and fiscal support, and shifting demography in patterns of oral disease as well as the background of students entering the dental profession. During the past several decades, there have been major shifts in the patterns of oral disease, most recently described in results of the NHANES III dental survey. The diversity and background of students entering the dental profession have changed dramatically as well, and present estimates of change

HIV service delivery after the PEPFAR and USAID HIV funding withdrawal in Uganda and Zimbabwe: an interrupted time-series study.

BACKGROUND: In January 2025, the US Government froze foreign aid and halted the President's Emergency Plan for AIDS Relief (PEPFAR), the largest external funder of HIV services globally. Early modelling studies and rapid assessments warned of major harm to millions, but robust quantitative evidence from those who relied most on the lost funding has been absent. We aimed to estimate the effect of the US funding withdrawal on HIV prevention, pre-exposure prophylaxis (PrEP), testing, and treatment services provided by non-governmental organisations (NGOs) serving primarily key populations in Ugan

Primary health care-oriented reforms: the political economy dynamics underlying health-system shifts across nine countries.

Using evidence from nine countries (Democratic Republic of Congo, Dominica, Egypt, Kazakhstan, Kenya, New Zealand, Thailand, Tunisia, and Uruguay), we analyze the political economy dynamics that emerged during implementation of primary health care (PHC)-oriented reforms. Across these cases, we identify 10 recurring health-system "shifts" toward stronger PHC orientation, which serve as a descriptive framework for examining the political economy challenges reformers faced and the strategies they used to navigate them. Primary data were collected from 356 participants through key informant interv
