A buried suture used in the repair of the central slip of a digital extensor resulted in an erosive lesion of the proximal interphalangeal joint of the long finger. In a review of the literature I can find no other reports describing this unusual complication.

BACKGROUND: The authors present their clinical experience and surgical methods of soft-tissue coverage for the finger using a posterior interosseous free flap.
METHODS: Twelve posterior interosseous free flaps, including two dual-paddle flaps, were performed in 12 patients. Indications included: 1) patients with soft-tissue coverage of the dorsum of the finger (n=4), pulp (n=1), fingertip and pulp (n=5), 2) patients subsequent to release of finger and palm contracture (n=1) and 3) patients subsequent to finger separation (n=1). The posterior interosseous vessels were ligated below the level a

We describe our experience of managing extensor hood injuries in boxers (57 fingers). The diagnosis was mostly clinical, with imaging only if the diagnosis was equivocal. The middle (61%) and index (26%) digits were most frequently injured. On exploration, 26% had no hood tear, however all required tenolysis from the adherent capsule. Of 42 hood tears, 15 were central splits between adjacent extensor tendons in the index or little fingers,15 tears were on the ulna side of the extensor tendon and 12 tears were on the radial side. A pseudobursa was encountered in 35%, capsular tears in 28% and c

STUDY DESIGN: Basic research (cross-sectional).
INTRODUCTION: Dupuytren disease can cause disabling contractures of the finger joints. After partial fasciectomy, postoperative hand splinting helps to maintain extension range of motion.
PURPOSE OF THE STUDY: To measure how the contraction forces of the finger on the splint change over time.
METHODS: Subjects who were treated for Dupuytren contracture with partial fasciectomy were invited to participate in this study. Force sensors were placed in their dorsal extension splint, and the applied force was measured continually for several weeks.

A 27-year-old man sustained right ring and little finger proximal interphalangeal joint fracture dislocations. He subsequently underwent bilateral hemi-hamate autograft to treat these intra-articular injuries. The patient was followed periodically and demonstrated return of normal grip strength and radiographic union of the hemi-hamate reconstruction arthroplasties. This is the first reported case of a patient who underwent bilateral hamates as donor grafts for two proximal interphalangeal joint fracture dislocations. When patients present with multiple unrepairable proximal interphalangeal jo

OBJECTIVES: This study aims to present long-term follow-up results of the reverse dorsolateral proximal phalangeal island flap designed for reconstruction of large fingertip and pulp defects.
METHODS: We examined 18 patients who underwent reverse dorsolateral proximal phalangeal island flap surgery to address ≥2.5 cm fingertip and pulp defects. Mean follow-up was 84.4 months. Sensitivity assessments were conducted using the Semmes-Weinstein monofilament and 2-point discrimination tests. Additionally, we evaluated finger joint active range of motion, complications and cold intolerance.

The analysis of knuckle creases is part of the multifactorial assessment of digital images of the hand used to assist in the identification of perpetrators captured in images depicting child sexual abuse and other offending behaviours. To quantify the impact of finger flexion on the appearance of the dorsal knuckle creases associated with the proximal interphalangeal joint (PIP) joint in digital images, the collection of knuckle crease images, at different points of flexion, was facilitated through an app-based Citizen Science project, Knuckle Down ID. A method of knuckle crease classification

BACKGROUND: Historically, the degloved finger with the total loss of nails and skin has been resurfaced in two stages. Furthermore, proximal finger amputation requires an additional bone-tendon graft and an expanded great toe wraparound flap transfer for better outcomes. This article recommends a novel strategy to address these problems in a single stage using a dorsal nail-skin flap and medial plantar artery perforator flap.
METHODS: From March of 2015 to June of 2018, nine procedures were performed to resurface with skin loss to the metacarpophalangeal joint level, and three amputated finge

BACKGROUND: A rare but typical perioperative complication in the surgical treatment of pediatric forearm fractures is injury to the extensor pollicis longus (EPL) tendon. This article presents the long-term results after reconstruction of the EPL tendon with transposition of the extensor indicis (EI) tendon in children and adolescents after surgically treated forearm fracture.
PATIENTS AND METHODS: Over a period of 15 years, 22 children up to the age of 16 were treated with EI transposition analogous to adult care. In a follow-up examination, the range of motion of the finger and thumb joints

INTRODUCTION: In recent years, the enzyme collagenase has been developed for the percutaneous treatment of Dupuytren's contracture, allowing management of the condition as an outpatient. We present early results on the treatment of Dupuytren's contracture using injectable collagenase.
METHODS: Patients awaiting Fasciectomy for Dupuytren's Contracture were selected for enrolment. Contracture cords were then marked and injected with collagenase in the outpatients department. Twenty-four hours later, patients returned for an extension procedure, performed under regional anaesthesia. Hand therapy

Bilateral subluxation of the trapeziometacarpal joint was related to abnormal insertion of the abductor pollicis longus (APL) tendon and an atrophic extensor pollicis brevis tendon. The APL tendon had four slips, all of which inserted into the fascia of the abductor pollicis brevis muscle distal and palmar to the trapeziometacarpal joint. Active pinch of the thumb resulted in subluxation of the trapeziometacarpal joint. To prevent reciprocal distal deformity, the proximal thumb was held in dynamic balance by attaching two slips of the APL tendon to the radiodorsal base of the first metacarpal.

We advise dynamic extensor splintage put on as early as possible within the first 6 weeks after the diagnosis of boutonniere deformity. This splintage runs from the dorsal aspect of the hand to the DIP joint which is left free to flex actively. This apparatus is left on for at least six weeks post injury. Sixteen patients have been treated in this way. Seven of them were monitored carefully. Only one had a bad result with DIP flexion still preserved. The others averaged a mean extension loss of 23 degrees, and no loss of extension in DIP joints. Active flexion both in PIP and DIP was perfectly

BACKGROUND: Mallet finger deformity is a prevalent disability that causes discomfort and inconvenience to the patients. Despite the existence of various surgical approaches, surgical management remains a controversial subject.
METHODS: We retrospectively analyzed the clinical data of 26 patients with isolated tendinous mallet fingers who were admitted between January 2021 and June 2022. Among them, there were 18 men and eight women, aged between 20 and 56 years, with an average age of 38 years. The causes of injury were cutting injuries (15 cases), sports impact injuries (nine cases), and spr

BACKGROUND: Multimodal analgesia is crucial for effective postoperative pain management in minor hand surgeries, enhancing patient satisfaction. The use of local wound infiltration with Ketorolac as an adjuvant pain management strategy is proposed for open trigger finger release surgery. This study aims to compare pain scores and functional outcomes between local wound infiltration with Ketorolac and oral non-steroidal anti-inflammatory drugs.
METHODS: This study is a double-blind, parallel design, randomized controlled trials. Sixty-nine patients underwent trigger finger surgery between Dece

BACKGROUND: When interventions to the hand are aimed at improving function of specific fingers or the thumb, the RIHM (Rotterdam Intrinsic Hand Myometer) is a validated tool and offers more detailed information to assess strength of the involved joints besides grip and pinch measurements.
METHODS: In this study, strength was measured in 65 thumbs in 40 patients diagnosed with thumb hypoplasia. These 65 thumbs were classified according to Blauth. Longitudinal radial deficiencies were also classified. The strength measurements comprised of grip, tip, tripod and key pinch. Furthermore palmar abd
