Investigations of Potential Phenotypes of Foot Osteoarthritis: Cross-Sectional Analysis From the Clinical Assessment Study of the Foot.

OBJECTIVE: To investigate the existence of distinct foot osteoarthritis (OA) phenotypes based on pattern of joint involvement and comparative symptom and risk profiles. METHODS: Participants ages ≥50 years reporting foot pain in the previous year were drawn from a population-based cohort. Radiographs were scored for OA in the first metatarsophalangeal (MTP) joint, first and second cuneometatarsal, navicular first cuneiform, and talonavicular joints according to a published atlas. Chi-square tests established clustering, and odds ratios (ORs) examined symmetry and pairwise associations of radi…

Quantitative assessment of early biomechanical modifications in diabetic foot patients: the role of foot kinematics and step width.

BACKGROUND: Forefoot ulcers (FU) are one of the most disabling and relevant chronic complications of diabetes mellitus (DM). In recent years there is emerging awareness that a better understanding of the biomechanical factors underlying the diabetic ulcer could lead to improve the management of the disease, with significant socio-economic impacts. Our purpose was to try to detect early biomechanical factors associated with disease progression. METHODS: Thirty subjects (M/F: 22/8; mean age ± SD: 61,84 ± 10 years) with diagnosis of type II DM were included. The participants were divided into 3 …

The influence of foot orthoses on foot mobility magnitude and arch height index in adults with flexible flat feet.

BACKGROUND: Flexible flat foot is described as a reduction in the height of the medial longitudinal arch and may occur from abnormal foot pronation. A foot orthosis is thought to modify and control excessive pronation and improve arch height. OBJECTIVE: To compare the immediate effect of three types of orthoses on foot mobility and the arch height index in subjects with flexible flat feet. STUDY DESIGN: A quasi-experimental study. METHOD: The dorsal arch height, midfoot width, foot mobility and arch height index were assessed in 20 participants with flexible flat feet (mean age = 23.2 ± 3 y…

Modelling of human walking to optimise the function of ankle-foot orthosis in Guillan-Barré patients with drop foot.

INTRODUCTION: This paper deals with the dynamic modelling of human walking. The main focus of this research was to optimise the function of … ring-damper between the shank and ankle that could compensate for their weakness by designing ankle-foot orthoses based on the kinematics data obtained from the patients. The torque generated in the ankle was varied by changing the spring constant. Therefore, it was possible to decrease the torque generated in muscles which could lead to the design of …

Development of the Oxford ankle foot questionnaire: finding out how children are affected by foot and ankle problems.

BACKGROUND: A large number of children are affected by foot and ankle problems owing to congenital deformities, clinical syndromes, neuromuscular conditions or trauma. This study aimed to identify how children's lives are affected by foot and ankle problems from the child's perspective as the first stage in developing a family-assessed instrument. METHODS: This was a qualitative study using focus groups involving children with a variety of foot and ankle problems aged 5-7, 8-11 and 12-15 years, and separate concurrent groups for their parents. The focus groups were child-centred and involved …

The role of intrinsic muscle flaps of the foot for bone coverage in foot and ankle defects in diabetic and nondiabetic patients.

Local muscle flaps, pioneered by Ger in the late 1960s, were extensively used for foot and ankle reconstruction until the late 1970s when, with the evolution of microsurgery, microsurgical free flaps became the reconstructive method of choice. To assess whether the current underuse of local muscle flaps in foot and ankle surgery is justified, the authors identified from the Georgetown Limb Salvage Registry all patients who underwent foot and ankle reconstruction with local muscle flaps and microsurgical free flaps from 1990 through 1998. By protocol, flap coverage was the reconstructive choice…

Miniature load cells for the measurement of foot-ground reaction forces and centre of foot pressure during gait.

A knowledge of the function of the foot is essential for an understanding of the mechanics of gait. In particular it is necessary to measure the forces, and their directions, between the foot and the ground. Miniature shoe borne load cells were used to measure the centre of foot pressure of a normal subject, a below knee amputee and a polio patient. An analysis of the resulting force patterns has been found useful as a measure of a subject's functional mobility. This paper describes a system which has been developed to measure and analyse the distribution of the forces acting on the foot of a …

Association of Incident Diabetic Foot Ulcer With Lower-Extremity Amputation Risk: The Prospective Seattle Diabetic Foot Study.

AIMS: Although etiologic factors for diabetes-related lower-extremity amputation (LEA) have been extensively examined, the role of incident diabetic foot ulcer (DFU) has not been prospectively evaluated. We investigated the independent effect of incident DFU, among other limb- and person-level factors, on LEA risk in a cohort of veterans with diabetes at one Department of Veterans Affairs general internal medicine clinic. METHODS: We prospectively followed 1458 male U.S. veterans with 2893 lower limbs without DFU between 1990 and 2002 (mean follow-up = 4.9 years). Potential risk factors were …

Effect of foot-shaped bionic shoes on ground reaction forces and foot stress at various running speeds.

This study examined ground reaction forces(GRFs) and bone stress differences between bionic running shoes (with foot-mimicking soles) and traditional shoes during running.Sixteen experienced male runners ran at 10, 12, and 14 km/h in both shoe types. Two-way ANOVA and SPM1d showed that bionic shoes had significantly lower peak propulsive but higher peak braking forces than traditional shoes.Bionic shoes also exhibited lower vertical forces in early stance and altered anterior-posterior forces patterns in late stance; finite element analysis indicated lower metatarsal stress in the bionic midso…

Impact of Foot Width on Patient-Reported Outcomes Assessed by 3-Dimensional Foot Morphometry in Hallux Valgus.

BACKGROUND: Patients with hallux valgus (HV) deformity often experience shoe-wearing discomfort, and increased foot width is considered a contributing factor. We assessed foot width using standing radiographs and 3-dimensional (3D) scans. This study aims to evaluate how these measurements are related to clinical outcomes in HV patients. METHODS: This cross-sectional study enrolled 19 consecutive adults presenting for hallux valgus evaluation. All participants underwent weight-bearing radiographs and 3D structured-light scanning, followed by completion of patient-reported outcome measures incl…

Cold Environment-Associated Extremity Lesions or Severe Hypothermia Help Differentiate Between Frostbitten and Trench Foot (Immersion Foot).

Cold associated foot injuries, such as frostbite and trench foot (immersion foot) are relatively common in cold environments. The former is classified as a freezing cold injury, and the latter as a nonfreezing cold injury. Trench foot is sometimes misdiagnosed as frostbite, and rapid rewarming exacerbates the condition. This paper aims to help differentiate between frostbitten foot and trench foot. This study included patients with frostbitten foot or trench foot treated at our hospital between December 2005 and May 2023. The differences in sex, age, month of injury, conditions at the time of …

Evaluation of Adjuvant Antibiotic Loaded Injectable Bio-Composite Material in Diabetic Foot Osteomyelitis and Charcot Foot Reconstruction.

The management of diabetic foot osteomyelitis (DFO) is extremely challenging with high amputation rates reported alongside a five-year mortality risk of more than fifty percent. We describe our experience in using adjuvant antibiotic-loaded bio-composite material (Cerament) in the surgical management of DFO and infected Charcot foot reconstruction. We undertook a retrospective evaluation of 53 consecutive patients (54 feet) who underwent Gentamicin or Vancomycin-loaded Cerament application during surgery. The feet were categorised into two groups: Group 1, with infected ulcer and DFO, managed …

Surgical management of the acute severely infected diabetic foot - The 'infected diabetic foot attack'. An instructional review.

Diabetic Foot Infection (DFI), in its severest form the acute infected 'diabetic foot attack', is a limb and life threatening condition if untreated. Acute infection may lead to tissue necrosis and rapid spread through tissue planes, in the patient with poorly controlled diabetes facilitated by the host status. A combination of soft tissue infection and osteomyelitis may co-exist, in particular if chronic osteomyelitis serves as a persistent source for recurrence of soft tissue infection. This "diabetic foot attack" is characterised by acutely spreading infection and substantial soft tissue ne…

Chronic Painless, Multiple Papulo-Nodular Skin Lesion at Foot Resembles Mycetoma Infection (Madura Foot): A Case Report.

Madura foot is a chronic, progressively destructive bacterial or fungal infection of the subcutaneous tissues, which may affect skin, muscle, and bone. It is endemic in tropical and subtropical areas known as the "Mycetoma belt" between latitude 30°N and 15°S. Mycetoma is prevalent in the poor population living in remote areas of developing countries that lack proper reporting system and management. The World Health Organization recognized mycetoma as a neglected tropical disease back in 2016. Though the diagnosis is challenging, early detection and proper treatment can reduce morbidity and pr…

Lower limb biomechanical characteristics of patients with neuropathic diabetic foot ulcers: the diabetes foot ulcer study protocol.

BACKGROUND: Foot ulceration is the main precursor to lower limb amputation in patients with type 2 diabetes worldwide. Biomechanical factors have been implicated in the development of foot ulceration; however the association of these factors to ulcer healing remains less clear. It may be hypothesised that abnormalities in temporal spatial parameters (stride to stride measurements), kinematics (joint movements), kinetics (forces on the lower limb) and plantar pressures (pressure placed on the foot during walking) contribute to foot ulcer healing. The primary aim of this study is to establish th…

Feasibility of combining a sensorimotor foot orthosis with a prefabricated carbon-fiber ankle-foot orthosis: A pilot study.

BACKGROUND: Stroke, traumatic brain injury, cerebral palsy, and other neuromuscular impairments can result in foot drop, which reduces toe clearance, increases fall risk, and limits mobility. Ankle-foot orthoses (AFOs) are frequently prescribed to address foot drop. Sensorimotor foot orthoses (SMFOs) represent an alternative approach intended to influence muscle activation through sensory stimulation; however, evidence supporting their combined use with AFOs remains limited. OBJECTIVE: To evaluate the feasibility of a subsequent Randomized Controlled Trial (RCT) comparing a prefabricated carb…
