Surgical Treatment of Adams Type IV Anterolateral Fracture of the Ulna Coronoid Process.

OBJECTIVE: Anterolateral coronal fractures are so rare that the mechanism of injury, the type of combined fracture and ligament injury, and the optimal treatment are unknown. To study the outcome of surgical treatments for anterolateral (AL) fracture of the ulna coronoid process (Adams Type IV) and summarize the characteristics of this type of fracture and to guide clinical applications. METHODS: From February 2015 to April 2021, 32 patients were included in the study. All patients had standard radiography with anteroposterior and lateral views, computed tomography, and intraoperative fluoros

Predictive Algorithm for Surgery Recommendation in Thoracolumbar Burst Fractures Without Neurological Deficits.

STUDY DESIGN: Predictive algorithm via decision tree. OBJECTIVES: Artificial intelligence (AI) remain an emerging field and have not previously been used to guide therapeutic decision making in thoracolumbar burst fractures. Building such models may reduce the variability in treatment recommendations. The goal of this study was to build a mathematical prediction rule based upon radiographic variables to guide treatment decisions. METHODS: Twenty-two surgeons from the AO Knowledge Forum Trauma reviewed 183 cases from the Spine TL A3/A4 prospective study (classification, degree of certainty of

Surgical treatment of coronal shear fractures: short- to mid-term results and risk factor analysis.

BACKGROUND: Coronal shear fractures of the distal humerus are rare but severe injuries. Reconstruction is often challenging, especially in comminuted cases, which is why many surgeons opt for an elbow arthroplasty in those cases. However, total elbow arthroplasty is associated with a variety of potential problems itself. Therefore, the aim of this study was to present the functional and clinical outcome of coronal shear fractures treated by osteosynthetic reconstruction in a short- to mid-term follow-up, and to identify possible risk factors for an inferior outcome. METHODS: We performed a re

Open reduction and internal fixation of Mason type III radial head fractures with and without an associated elbow dislocation.

PURPOSE: The purpose of this study was to evaluate the treatment of patients with Mason type III radial head fracture with and without elbow dislocation by open reduction with internal fixation, collateral ligament repair, and early mobilization. An additional purpose was to investigate whether there is any effect of elbow dislocation on the severity and functional outcome. METHODS: Twenty-five patients were treated with open reduction with internal fixation using low-profile miniplate and screws for comminuted fracture of the radial head. Eighteen fractures were Mason type III, and 7 fractur

[Treatment results of fractures of the patella using pre-patellar tension wiring. Analysis of a series of 203 cases].

Two hundred three fractures of the patella in 200 patients were treated by tension band wire fixation using one or several wire loops and 2 longitudinally directed Kirschner wires. They were reviewed with a mean follow-up of six years (range 1 to 10 years). There were 143 men and 57 women. Age at operation averaged 36 years (range 18 to 83 years). The most frequent etiology was a road traffic accident. The fractures were simple in 35.5%, slightly comminuted in 37%, and very comminuted in 27.5% of cases. Thirty-four fractures were open, and there were other associated fractures in 35 cases (17%

Management of post-traumatic elbow instability after failed radial head excision: A case report.

Radial head excision has always been a safe commonly used surgical procedure with a satisfactory clinical outcome for isolated comminuted radial head fractures. However, diagnosis of elbow instability is still very challenging and often underestimated in routine orthopaedic evaluation. We present the case of a 21-years old female treated with excision after radial head fracture, resulting in elbow instability. The patient underwent revision surgery after four weeks. We believe that ligament reconstruction without radial head substitution is a safe alternative choice for Mason III radial head f

[Le Fort type I segmentary and nasoorbitalethmoidal fractures. A case report].

The association of Le Fort fractures and those of the nasoorbito-etmoidal complex represent severe injuries usually associated to traffic accidents or physical violence. The occurrence of such combination of fractures represents a challenge for the maxillofacial surgeon due to the great index of comminuted and displacement fragments. The initial evaluation represents the opportunity to identify and treat possible life-threatening complications such as brain damage or airway compromise; this first evaluation will also allow the establishment of diagnosis and a suitable surgical plan in order to

Distal radius fractures: treatment using the Epibloc system.

The Epibloc system of percutaneous endomedullary internal fixation is best indicated for AO type A2-3 extra-articular fractures of the distal radius and AO type C1 slightly comminuted articular fractures. This system includes pins that can be inserted into the medullary canal and advanced without breaking through the second cortex. This fixation is stabilised by an external plate and rendered dynamic by the pins' elasticity which compresses the fractured surfaces. Moreover, this system results in transversal ligamentotaxis which helps prevent secondary reduction losses. The result of a prelimi

Acute volar compartment syndrome of the forearm secondary to fractures of the distal radius.

We present five cases of acute volar compartment syndrome developing as a complication of fractures of the distal radius. The patients were all males under the age of 50 years who sustained comminuted or displaced fractures of the distal radius. We suggest that this indicates an 'at risk' group. The interval between injury and the onset of symptoms of volar compartment syndrome varied between 12 and 48 hours. The diagnosis of each case was made clinically and confirmed at operation. With early extensive decompression full neurovascular recovery can be expected.

Redisplacement of Distal Radius Fracture after Initial Closed Reduction: Analysis of Prognostic Factors.

BACKGROUND: To evaluate risk factors of redisplacement and remind surgeons of key factors regarding conservative treatment of distal radius fracture. METHODS: A total of 132 patients who received conservative treatment for distal radius fractures between March 2008 and February 2011 were included in this study. Radial inclination, radial length, volar tilting angle, ulnar variance, fragment translation, and presence of dorsal metaphyseal comminution were measured on the X-rays taken immediately after reduction, one week after injury during the first follow-up outpatient clinic visit, and afte

Humeral shaft fractures: union outcomes in a large cohort.

BACKGROUND: Humeral shaft fractures can be managed conservatively or operatively. Fracture characteristics were analyzed to identify patients who would benefit from early operative fixation. METHODS: We performed a retrospective cohort study of 126 consecutive humeral shaft fractures (2008-2015). Fractures were classified according to fracture type, location, separation, and comminution. RESULTS: Of 126 patients, 96 were managed conservatively. In 54%, union occurred before 26 weeks, and 13% had delayed union after 26 weeks, whereas 33% did not achieve union. Of 30 patients managed surgicall

Maxillofacial and mandibular fractures.

Any traumatic event that produces maxillofacial and/or mandibular fractures generally results in gross and usually severe patient disfigurement and often results in the patient's inability to eat and drink. These fractures are exceptionally rewarding cases as simple techniques may be performed resulting in a successful functional outcome (ability to eat and drink) within a very short period of time (24 hours) after fracture stabilization. A markedly improved cosmetic appearance follows shortly thereafter once inflammation and edema resolve. The primary principle of fracture treatment, ie, prov

Stab Wound in the Skull Treated with a Medial Supraorbital Craniotomy Through an Incision in the Eyebrow-a Minimally Invasive Approach.

BACKGROUND: The surgical management of osteoporotic pelviacetabular fractures poses distinct challenges due to poor screw purchase, severe comminution of fractures, and the inability to perform prolonged surgeries in patients with significant comorbidities. These fractures necessitate tailored modifications in surgical approaches, implant selection, and techniques based on the patient's overall health, fracture complexity, and bone quality. METHODS: A comprehensive literature search was conducted using PubMed and Google Scholar databases to identify relevant articles on the management of oste

Outcome after open reduction and internal fixation of capitellar and trochlear fractures.

BACKGROUND: Capitellar and trochlear fractures are uncommon fractures of the distal aspect of the humerus. There is limited information about the functional outcome of patients managed with open reduction and internal fixation. METHODS: The functional outcome of twenty-eight patients, with a mean age (and standard deviation) of 43 +/- 13 years, who were treated with open reduction and internal fixation for capitellar and trochlear fractures was evaluated at a mean duration of follow-up of 56 +/- 33 months. Patient outcomes were assessed with physical and radiographic examination, range-of-mot

Long-term physical impairment and functional outcomes after complex facial fractures.

To develop an understanding of the expected functional outcomes after facial trauma, a retrospective cohort study of patients with complex facial fractures was conducted. A cohort of adults aged 18 to 55 years who were admitted to the R. Adams Cowley Shock Trauma Center between July of 1986 and July of 1994 for treatment of a Le Fort midface fracture (resulting from blunt force) was retrospectively identified. Outcomes of interest included measures of general health status and psychosocial well being in addition to self-reported somatic symptoms. General health status was ascertained using the

Functional recovery after fractures of the distal forearm. Analysis of radiographic and other factors affecting the outcome.

Functional recovery of the wrist and hand after a fracture of the distal forearm in 207 consecutive patients was analysed. A good or excellent result was achieved in 77 percent, fair in 22.5 percent and poor in 0.5 percent of cases. The result in 14 unstable fractures treated with external fixation was as good as that of the whole series. Good functional results were associated with extra-articular fractures, good anatomical results, male sex and low age. Comminuted intra-articular fractures of the radiocarpal joint, fracture line into the distal radio-ulnar joint, fracture of the ulnar styloi

Complete Reduction for Pilon Fracture Can Make Complete Failure.

The surgical management of distal intra-articular comminuted fracture of the tibia (pilon fracture) is difficult because complications frequently develop. The minimally invasive plate osteosynthesis technique is generally accepted for this type of fracture. In this study, complications developed after open reduction and internal fixation using multiple miniplates for accurate reduction of small fracture fragments. Therefore, when we use this technique, we need to pay attention to the development of complications such as nonunion, avascular necrosis, and osteomyelitis by the disruption of both
