Deciphering copy number variations and gene implications in an Egyptian cohort with autism spectrum disorders.

BACKGROUND: Genetic factors are major contributors to autism spectrum disorders (ASD), with copy number variations (CNVs) playing a significant role. Our objective was to identify and assess CNVs and their associated gene-level impacts in Egyptian ASD patients. METHODS: Our cohort comprised 40 non-syndromic ASD children, ranging in age from one year and nine months to 12 years. We conducted karyotyping, multiple ligation-dependent probe amplification (MLPA), Methylation-Specific MLPA (MS-MLPA), and chromosomal microarray (CMA) analyses for all patients. Additionally, quantitative real-time PC

French protocol for the diagnosis and management of juvenile idiopathic arthritis including pediatric-onset Still's disease.

Juvenile idiopathic arthritis (JIA) is characterised by arthritis onset before the age of 16, persisting for at least 6weeks without a known cause. Symptoms include joint swelling, inflammatory pain (worse at night and in the morning), or also back, heel, or buttock pain. Timely diagnosis and referral to a paediatric rheumatologist are crucial to reduce errors, invasive procedures, and long-term complications. Around 5000 children under 16 are affected by JIA in France. The current international classification recognises 7 subgroups: the systemic form, oligoarthritis, polyarthritis without rhe

Cost-effectiveness of outpatient COVID-19 antiviral treatment with nirmatrelvir/ritonavir versus usual care in Swedish patients with various risk factors.

AIMS: Nirmatrelvir/ritonavir (NMV/r) is an orally administered antiviral indicated for the outpatient treatment of adult patients with mild-to-moderate COVID-19 at high risk for disease progression to severe illness. We estimated the cost-effectiveness of NMV/r versus best supportive care for 54 patient cohorts, specified according to age, vaccination status and comorbidity burden. MATERIALS AND METHODS: A previously published and validated cost-effectiveness model was utilized and adapted to the Swedish setting. The model used a short-term decision-tree (1 year) followed by a lifetime 2-stat

Adenosine 2A receptor contributes to the facilitation of post-infectious irritable bowel syndrome by γδ T cells via the PKA/CREB/NF-κB signaling pathway.

BACKGROUND: Immunological dysfunction-induced low-grade inflammation is regarded as one of the predominant pathogenetic mechanisms in post-infectious irritable bowel syndrome (PI-IBS). γδ T cells play a crucial role in innate and adaptive immunity. Adenosine receptors expressed on the surface of γδ T cells participate in intestinal inflammation and immunity regulation. AIM: To investigate the role of γδ T cell regulated by adenosine 2A receptor (A2AR) in PI-IBS. METHODS: The PI-IBS mouse model has been established with Trichinella spiralis (T. spiralis) infection. The intestinal A2AR and A2A

Exploring mechanisms of blood pressure regulation in response to device-guided and non-device-guided slow breathing: A mini review.

BACKGROUND: Hypertension is a widespread disease that, if persistent, increases the risks of coronary heart disease mortality and morbidity. Slow breathing is a recommended blood pressure-lowering strategy though the mechanisms mediating its effects are unknown. OBJECTIVE: This review aims to evaluate autonomic and vascular function as potential mediators driving BP adaptive responses with slow breathing. METHODS: We searched EBSCO host, Web of Science, Cochrane Central Register of Controlled Trials, and PubMed using key words for optimized search results. RESULTS: Nineteen studies were inc

Coding and noncoding variants in EBF3 are involved in HADDS and simplex autism.

BACKGROUND: Previous research in autism and other neurodevelopmental disorders (NDDs) has indicated an important contribution of protein-coding (coding) de novo variants (DNVs) within specific genes. The role of de novo noncoding variation has been observable as a general increase in genetic burden but has yet to be resolved to individual functional elements. In this study, we assessed whole-genome sequencing data in 2671 families with autism (discovery cohort of 516 families, replication cohort of 2155 families). We focused on DNVs in enhancers with characterized in vivo activity in the brain

When silence goes viral, Africa sneezes! A perspective on Africa's subdued research response to COVID-19 and a call for local scientific evidence.

By mid-September 2020, over 1.33 million confirmed COVID-19 cases and 32 thousand deaths had been reported in Africa. Global research on COVID-19 went 'viral' with a record 3487 research contributions comprising of 2062 journal papers and 1425 preprints published within the first three months following the outbreak of COVID-19. Surprisingly, the silence of the African research community has been unprecedented - contributing a paltry 0.6% (22 contributions), a figure nearly matched by Colombia (18 publications). Until now, a comprehensive perspective on the reasons for this subdued research res

Psychopharmacotherapy of Obsessive-Compulsive Symptoms within the Framework of Tourette Syndrome.

While Behavioral Therapy (BT) should be recommended as the first step in the treatment of OCD as well as TS, medication can be added for augmentation and in certain situations (e.g. family preference, BT not available or feasible) the priority may even reverse. This narrative review is given on the complexity of drug treatment in patients comorbid with obsessive-compulsive disorder (OCD) and Tourette syndrome (TS) and other tic problems. OCD with TS is a co-occurring combination of the two generally delimitable, but in detail, also overlapping disorders which wax and wane with time but have di

[Novel factors playing a role in the pathomechanism of diffuse liver diseases: apoptosis and hepatic stem cells].

Liver diseases can basically be categorised into the forms diffuse and focal. In case of diffuse liver diseases the damages in general affect the whole of the liver, but the alterations are usually present in varying degrees and severity. Relatively stereotypical reactions develop in the liver tissues on the effect of various damaging factors. Some of these reactions and their pathogenic backgrounds are well known, while others have only recently come into focus, like apoptosis and the regeneration-proliferation related hepatic stem cells. The number of damaging effects are naturally much high

On the functions of monoamine oxidase, the emotions, and adaptation to stress.

Monoamine oxidase is an iron containing enzyme that exists as 2 isozymes, A and B, that have different affinities for various amines as substrates. The activity of monoamine oxidase helps to maintain neuron firing rates throughout the body within homeostatic limits. It does this by metabolizing in the liver bioactive amines absorbed into the bloodstream from food, by metabolizing in the endothelial cells of cerebral vascular microvessels, as part of the blood brain barrier, bioactive amines in the bloodstream, and by metabolizing in the cytoplasm of neurons, molecules of biogenic amine neurotr

Immunoglobulin G complexes from post-infectious ME/CFS, including post-COVID ME/CFS disrupt cellular energetics and alter inflammatory marker secretion.

BACKGROUND: Autoimmunity is a key clinical feature in both post-infectious Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) and Post-Acute Sequelae of COVID (PASC). Passive transfer of immunoglobulins from patients' sera into mice induces some clinical features of PASC. However, the physiological effects of immunoglobulins on cellular alterations remain elusive. In this study, we tested the potential effects of immunoglobulins from ME/CFS patients on endothelial cell dysfunction. METHODS: We have isolated immunoglobulins from 106 individuals, including ME/CFS (n = 39), PCS-CFS (n =

Chitosan nanoparticles as next-generation carriers for veterinary DNA vaccines: Mechanisms, immune responses, and translational prospects.

Chitosan-based DNA nanoparticles have emerged as a promising next-generation platform for veterinary vaccines, addressing several limitations of conventional attenuated, inactivated, and recombinant formulations. Chitosan is a biodegradable, biocompatible, and low toxicity polymer with mucoadhesive properties that enhance cellular uptake and protect nucleic acids from enzymatic degradation. These characteristics make it an attractive candidate for delivering plasmid DNA encoding viral antigens across diverse animal species. Recent advances demonstrate that chitosan-DNA nanoparticles can induce

Let's Code! How Can Programming Logic Make a Vascular Neurologist Even Better?

BACKGROUND: As digital health and artificial intelligence (AI) become integral to medicine, there is a growing need for physicians to develop computational thinking skills. In vascular neurology, a specialty reliant on algorithmic decision-making and complex data interpretation, programming logic (PL) offers a powerful cognitive framework. This review argues that PL can enhance diagnostic precision, clinical efficiency, and data-driven reasoning. By aligning core programming structures - such as conditional statements, loops, and data abstraction - with clinical workflows, neurologists can imp

Uncomplicated monochorionic twins: neurodevelopmental insights and implications.

OBJECTIVE: Emerging evidence indicates that uncomplicated monochorionic diamniotic (MCDA) twins deviate physiologically from singletons, probably owing to adaptations to their unique intrauterine environment. However, their long-term neurodevelopmental outcomes remain poorly understood. Early identification of neurodevelopmental risks is critical to optimize neuroplasticity during key developmental periods. The aim of this study was to assess neurodevelopmental outcomes in uncomplicated MCDA twins up to 3 years of age and propose recommendations for future research and follow-up protocols. ME

Use of an adaptive sensory environment in patients with autism spectrum disorder (ASD) in the perioperative environment: a parallel, randomized controlled trial.

BACKGROUND: Patients with autism spectrum disorders (ASD) experience higher rates of perioperative anxiety and are likely to receive premedication. Little is known about nonpharmaceutical interventions which may decrease anxiety. This study aims to evaluate the use of an adaptive sensory environment (ASE) to reduce ASD patient anxiety during the perioperative process. METHODS: Our feasibility study (ClinicalTrials.govNCT04994613) enrolled 60 patients in two parallel groups randomized to a control (no ASE) or intervention group (ASE). We included all surgical patients aged three to twelve year

Longitudinal neurobehavioral profiles in children and young adults with PTEN hamartoma tumor syndrome and reliable methods for assessing neurobehavioral change.

BACKGROUND: Individuals with PTEN hamartoma tumor syndrome (PHTS) demonstrate a distinct neurobehavioral profile suggesting primary disruption of frontal lobe symptoms, with more severe cognitive deficits in those with associated autism spectrum disorder (ASD) that extend to other areas of neurobehavioral function as well (e.g., adaptive behavior, sensory deficits). The current study sought to characterize longitudinal neurobehavioral profiles in individuals with PHTS who completed serial assessments (2-3 evaluations) over a 2-year time period. METHODS: Comprehensive neurobehavioral evaluatio

Characterisation of macrophage infiltration and polarisation based on integrated transcriptomic and histological analyses in Primary Sjögren's syndrome.

BACKGROUND: Primary Sjögren's syndrome (pSS) is a progressive inflammatory autoimmune disease. Immune cell infiltration into glandular lobules and ducts and glandular destruction are the pathophysiological hallmarks of pSS. Macrophages are one of the most important cells involved in the induction and regulation of an inflammatory microenvironment. Although studies have reported that an abnormal tissue microenvironment alters the metabolic reprogramming and polarisation status of macrophages, the mechanisms driving macrophage infiltration and polarisation in pSS remain unclear. METHODS: Immune

A discrepant presentation of bacteremia in the emergency department linked to a Fusobacterium nucleatum infection: a case report.

BACKGROUND: Fusobacterium nucleatum is an anaerobic bacterium mainly responsible for acute or chronic infection of the ear, nose, and throat, potentially bacteremic with a risk of extraoral metastatic infection. Bacteremia occurs mainly in the elderly or in immunodeficient individuals, with high mortality. F. nucleatum is not the first cause of tonsillar infection in emergency departments, which are more often the consequence of a viral or streptococcal infection, but it is a risk factor for severe bacterial infection, especially in a viral pandemic context. Deep learning-based monocular placental pose estimation: towards collaborative robotics in fetoscopy.

PURPOSE: Twin-to-twin transfusion syndrome (TTTS) is a placental defect occurring in monochorionic twin pregnancies. It is associated with high risks of fetal loss and perinatal death. Fetoscopic elective laser ablation (ELA) of placental anastomoses has been established as the most effective therapy for TTTS. Current tools and techniques face limitations in case of more complex ELA cases. Visualization of the entire placental surface and vascular equator; maintaining an adequate distance and a close to perpendicular angle between laser fiber and placental surface are central for the effective
