General anaesthesia versus other types of anaesthesia in patients undergoing surgery for treatment of cutaneous melanoma: a systematic review and meta-analysis.

General and local anaesthetics alter tumour behaviour in experimental models. Objectives: To investigate the relationship between general anaesthesia and recurrence or survival in patients who received surgery for malignant melanoma. A meta-analysis was performed based on a comprehensive literature search. Controlled and observational studies of patients undergoing surgery for melanoma under general anaesthesia, compared with other types of anaesthesia, were included. The primary outcomes were overall survival and disease-free survival. The secondary outcomes included cancer-specific survival,

Outcomes After Arthroscopic Surgery for Anterior Impingement in the Ankle Joint in the General and Athletic Populations: Does Sex Play a Role?

BACKGROUND: Although anterior ankle impingement is a common pathology within the athletic population, there have been limited data evaluating outcomes of arthroscopic intervention and whether patient sex affects treatment outcomes. PURPOSE: To provide an overview of the clinical outcomes of arthroscopic procedures used as a treatment strategy for anterior ankle impingement and to determine if patient sex affects outcomes. STUDY DESIGN: Systematic review. METHODS: A systematic literature search of the MEDLINE, Embase, and Cochrane databases was performed during August 2019. The following com

Young adults undergoing ACDF surgery exhibit decreased health-related quality of life in the long term in comparison to the general population.

BACKGROUND CONTEXT: The leading surgical treatment of cervical radiculopathy is anterior cervical discectomy and fusion (ACDF). However, it has been suggested that ACDF procedures could lead to accelerated degeneration of the adjacent cervical discs (adjacent segment disease, or ASD) and the effect of ACDF surgery on neck symptoms and quality of life in the long term is not fully understood. Patients operated on at young ages generally have a long life expectancy and a long number of working years ahead of them. Thus, this patient group is of special interest when considering the accumulation

Creation and implementation of a novel clinical workflow based on the AAST uniform anatomic severity grading system for emergency general surgery conditions.

OBJECTIVE: Emergency general surgery (EGS) conditions encompass a variety of diseases treated by acute care surgeons. The heterogeneity of these diseases limits infrastructure to facilitate EGS-specific quality improvement (QI) and research. A uniform anatomic severity grading system for EGS conditions was recently developed to fill this need. We integrated this system into our clinical workflow and examined its impact on research, surgical training, communication, and patient care. METHODS: The grading system was integrated into our clinical workflow in a phased fashion through formal educat…

Abnormally low Bispectral index and severe hypoglycemia during maintenance of and recovery from general anesthesia in diabetic retinopathy surgery: two case reports.

BACKGROUND: Hypoglycemia is one of the most fatal complications during the perioperative period. General anesthesia or sedation can mask a hypoglycemia-altered mental status. Acute hypoglycemia might result in permanent brain injury. There is no way to detect hypoglycemia during general anesthesia, except for intermittent blood glucose monitoring. CASE PRESENTATION: Hypoglycemia is associated with changes in electroencephalogram readings. Here, we report two cases of patients with an abnormally low Bispectral Index (BIS) associated with diabetic retinopathy surgery, one in the recovery stage

Surgical Site Infection after Primary Closure of High-Risk Surgical Wounds in Emergency General Surgery Laparotomy and Closed Negative-Pressure Wound Therapy.

BACKGROUND: We hypothesized that the universal adoption of closed wounds with negative pressure wound therapy (NPWT) in emergency general surgery patients would result in low superficial surgical infection (SSI) rates. STUDY DESIGN: We performed a retrospective observational study using primary wound closure with external NPWT, from May 2017 to May 2018. Patients with active soft tissue infection of the abdominal wall were excluded. Data were analyzed by Fisher's exact tests and Wilcoxon-Mann-Whitney tests, with significance is set at a value of p < 0.05. RESULTS: Eighty-five patients (53% f…

Cerebral oxygenation in patients undergoing shoulder surgery in beach chair position: comparing general to regional anesthesia and the impact on neurobehavioral outcome.

BACKGROUND: Ischemic brain damage has been reported in healthy patients after beach chair position for surgery due to cerebral hypoperfusion. Near-infrared spectroscopy has been described as a non-invasive, continuous method to monitor cerebral oxygen saturation. However, its impact on neurobehavioral outcome comparing different anesthesia regimens has been poorly described. METHODS: In this prospective, assessor-blinded study, 90 patients undergoing shoulder surgery in beach chair position following general (G-group, n=45) or regional anesthesia (R-group; n=45) were enrolled to assess the pr

Combined popliteal and saphenous nerve blocks at the knee: an underused alternative to general or spinal anesthesia for foot and ankle surgery.

Peripheral nerve blocks at the ankle have long been used for foot surgery. However, when local foot and ankle blocks are inappropriate or contraindicated, general and spinal anesthesia are the common alternatives. Both have disadvantages and require added equipment and monitors. Combined popliteal and saphenous nerve blocks at the knee can offer a desirable alternative to general and spinal anesthesia for foot and ankle surgery. In addition, popliteal and saphenous nerve blocks provide anesthesia of the entire lower leg, thus permitting a greater variety of procedures to be performed. This art

Impact of opioid-free general anesthesia with locoregional blocks during breast cancer surgery on systemic inflammatory response: results from a randomized controlled trial.

BACKGROUND: For breast cancer patients with localized disease, surgery remains a cornerstone of treatment. Anesthesia during surgery is essential for pain management and influences patient recovery. Opioid-free general anesthesia with locoregional blocks (OfGA), as an alternative to traditional opioid-based general anesthesia (GA), may improve patient outcomes. METHODS: We set up a randomized trial to compare the impact of different anesthetic approaches on post surgical inflammation in breast cancer patients undergoing quadrantectomy. Patients were randomized to receive GA (n=34) or OfGA (n=

Effect of noise isolation during general anaesthesia on the incidence of moderate-to-severe pain after major abdominal surgery: multicentre randomized clinical study.

BACKGROUND: Postoperative pain remains a major challenge following major abdominal surgery. Noise in the operating room is a modifiable stressor, and the efficacy of targeted noise isolation requires prospective investigation. This study investigated the effects of intraoperative noise isolation on the incidence of moderate-to-severe postoperative pain. METHODS: This multicentre randomized clinical trial assessed patients who underwent elective major abdominal surgery under general anaesthesia in four medical centres in China between April, 2024 and May, 2025. Following anaesthesia induction,

Postoperative Recovery Time After Knee Arthroscopic Surgery Using Low Dose Mepivacaine Spinal Anesthesia Versus General Anesthesia: A Retrospective Propensity Score Matched Cohort Study.

General anesthesia (GA) is the preferred technique in ambulatory surgery because GA is associated with a predictable recovery time. However, it carries higher risk of postoperative nausea and vomiting and drowsiness when compared with spinal anesthesia (SA). This study aimed to determine whether the recovery time in mepivacaine of 30 mg SA is noninferior when compared with GA. Our single-center retrospective study used a multivariable logistic regression to model anesthetic modality as a function of age, sex, body mass index, and American Society of Anesthesiologists physical status to generat
