We present a case report of a 60-year-old white woman, found to have advanced angle closure glaucoma in the right eye and appositional closure for about half of the left eye and no glaucoma. The remaining angle of the left eye was open with localized heavy pigmentation of the ciliary body band and trabecular meshwork. Ultrasound biomicroscopy showed a ciliary body tumor displacing the iris root centripetally preventing angle closure.

PURPOSE: We sought to investigate the effect of external subretinal fluid drainage (SRFD) on secondary or impending secondary glaucoma caused by bullous exudative retinal detachment for saving eyes with Coats' disease.
METHODS: By retrospective chart review, we collected the treatment results of 56 patients younger than 15 years of age with Coats' disease. External SRFD was performed when exudative retinal detachment became bullous enough to cause anterior displacement of the lens-iris diaphragm or when secondary angle-closure glaucoma occurred. Treatment results were regarded as successful w

Lens-induced glaucoma may occur as either secondary angle-closure or open-angle glaucoma. Dislocation or swelling of the lens can cause pupillary block and subsequent angle-closure glaucoma. Leakage of soluble lens proteins from a relatively intact cataractous lens can result in a severe secondary open-angle glaucoma (phacolytic glaucoma). Heavy molecular weight protein, believed to be of lens origin, has now been identified in 12 of 12 anterior chamber specimens from such patients. This liberated lens protein can directly obstruct the trabecular outflow pathways. After extracapsular cataract

BACKGROUND: Angle-closure glaucoma is a leading cause of irreversible blindness in the world. Treatment is aimed at opening the anterior chamber angle and lowering the IOP with medical and/or surgical treatment (e.g. trabeculectomy, lens extraction). Laser iridotomy works by eliminating pupillary block and widens the anterior chamber angle in the majority of patients. When laser iridotomy fails to open the anterior chamber angle, laser iridoplasty may be recommended as one of the options in current standard treatment for angle-closure. Laser peripheral iridoplasty works by shrinking and pullin

The occurrence of chronic postoperative glaucoma in 502 consecutive penetrating keratoplasty cases performed in 421 patients during 1978 through 1986 was 18% (91 cases). Immediate postoperative glaucoma occurred in 9% (44 cases) and progressive angle closure occurred in 2% (13 cases) of keratoplasties. Statistically significant risk factors for development of glaucoma were preexisting glaucoma and aphakia. The type of cataract extraction, use of an intraocular lens, or performance of vitrectomy did not pose significant risk of glaucoma. Surgical therapy of glaucoma was required in 24% of those

AIM: To describe the surgical management of glaucoma in a patient with severe scleromalacia, and secondary angle closure.
INTRODUCTION: The management of glaucoma with coexisting scleromalacia plus secondary angle closure is challenging as most commonly performed incisional glaucoma surgery as well as minimally invasive glaucoma surgery (MIGS), which targets the drainage angle are all contraindicated.
CASE REPORT: Medically refractory glaucoma in a 60-year-old male with a 30-year history of granulomatosis with polyangiitis resulting in extensive severe scleromalacia, cicatricial lower lid re

Morphological changes in the iris are one of the major causes of angle-closure glaucoma, and an anteriorly-bowed iris may be further associated with greater risk of disease progression from primary angle-closure suspect (PACS) to chronic primary angle-closure glaucoma (CPCAG). In consequence, the automated detection of abnormalities in the iris region is of great importance in the management of glaucoma. In this paper, we present a new method for the extraction of the iris region by using a local phase tensor-based curvilinear structure enhancement method, and apply it to anterior segment opti

BACKGROUND Drug-induced acute angle closure glaucoma is an uncommon ocular emergency that may follow the administration of certain topical and systemic medications. Acute angle closure can be triggered by various classes of drugs, including adrenergic agonists, anticholinergics, and serotonergic medications. Here, we report a rare case of drug-induced acute angle closure glaucoma secondary to olanzapine. CASE REPORT A 59-year-old male patient of Arabian Peninsula descent, known to have schizophrenia, presented to our Emergency Department with a 3-day history of right ocular pain and decrease i

PURPOSE OF REVIEW: Most people affected by glaucoma live in developing countries. Recent trials and reports provide sound evidence for management of glaucoma. This review extrapolates relevant articles to the developing world.
RECENT FINDINGS: The predictive value of gonioscopy for progression of primary angle closure suspects (PACS) to primary angle closure (PAC) is only 22% (95% CI: 9.80-34.2). PACS are not uncommon; laser peripheral iridotomy (LPI) is neither indicated nor feasible for all. Twenty-eight and a half percent of PAC progress to primary angle closure glaucoma; the number needed

Angle closure can recur following peripheral iridotomy in up to 58% of cases, due to plateau iris syndrome, lens disproportion or ciliary block. Ciliary block glaucoma is an important differential diagnosis of shallow anterior chamber, angle closure and high intraocular pressure, which may occur spontaneously or following laser or surgery. Some underlying mechanisms of ciliary block glaucoma remain poorly understood but lens-ciliary body apposition and anterior hyaloid changes with increased hydraulic resistance are major pathogenic factors. An understanding of the pathogenic factors facilitat

A prospective study of 161 consecutive glaucoma patients conducted from June 1983 to May 1984 showed that 81 (50.3%) had unilateral glaucoma. The causes of the unilateral disease were primary open-angle glaucoma (38.3%) and secondary glaucoma (61.7%). During this year long study primary angle-closure glaucoma was not observed. The relatively frequent occurrence of unilateral primary open-angle glaucoma poses a riddle for which I have no explanation to offer.

Angle closure glaucoma is emerging as a leading cause of blindness in the densely populated countries of Asia. It has a greater propensity to cause bilateral blindness than either primary open-angle glaucoma or secondary glaucoma. An estimated 9.4 million people in China aged 40 years and older have glaucomatous optic neuropathy. Approximately 5.2 million people (55%) would be blind in at least one eye. Around 1.7 million (18.1%) would be blind in both eyes. it is likely that PACG is responsible for the vast majority (91%) of bilateral glaucoma blindness in China. The number of Chinese people

Primary iridociliary cysts can induce plateau iris configuration and angle closure glaucoma. We report a patient with bilateral, primary, ring-shaped, solitary iridociliary cysts. The right eye displayed normal intraocular pressure, oppositional iridocorneal angle closure, and healthy optic nerve head. The left eye had advanced chronic angle closure glaucoma. The management strategy varied between eyes and is discussed. This, to the best of our knowledge, is the first report of transscleral Diode laser application in an eye with a large iridociliary cyst.

Functional neuroimaging studies have reported alterations in cortical activity indicating glaucoma as a progressive neurodegenerative disease. Hence the current study aimed to assess the cortical activity using high-density EEG in patients with mild glaucoma during resting state. Treatment-naive 37 patients with primary open angle glaucoma (POAG), 34 patients with primary angle closure glaucoma (PACG), and 32 healthy controls were included in the study. Resting state EEG i.e., eyes closed (EC) and eyes open conditions (EO) were acquired using 128-channel for 3 min. After preprocessing, the cur

Potential obstruction of the anterior chamber angle by iris root is a key factor of the pathogenesis of angle-closure glaucoma. Development and adoption of ultrasound biomicroscopy and optical coherence tomography in clinical practice have significantly expanded the capabilities of studying the structures of the anterior eye segment in terms of angle closure risk factors through improving visualization depth and quality of examination, including different variants of iris structure and potential changes of its shape in mydriasis. The article reviews various studies dedicated to the progression
