Natural language processing to identify suicidal ideation and anhedonia in major depressive disorder.

BACKGROUND: Anhedonia and suicidal ideation are symptoms of major depressive disorder (MDD) that are not regularly captured in structured scales but may be captured in unstructured clinical notes. Natural language processing (NLP) techniques may be used to extract longitudinal data on suicidal behaviors and anhedonia within unstructured clinical notes. This study assessed the accuracy of using NLP techniques on electronic health records (EHRs) to identify these symptoms among patients with MDD. METHODS: EHR-derived, de-identified data were used from the NeuroBlu Database (version 23R1), a lon

COMPEERS: A Cluster Randomised Feasibility Study of Contingency Management and PEER Support to Promote Attendance, Increase Treatment Engagement and Improve Outcomes for People with Dual Diagnoses of Psychosis and Drug/Alcohol Problems. Study Protocol with Commentary.

Dual diagnoses of psychosis and drug or alcohol problems (DDp) are prevalent, high-cost presentations, reliably associated with poorer outcomes and greater difficulty engaging with mental health services compared to either condition alone or other mental health problems. Engagement difficulties restrict access to recommended care that could improve outcomes, and thus are a priority treatment target. Contingency management (financial incentives for positive health behaviours, CM), peer support, and text messaging each improve engagement for psychosis or drug/alcohol problems. Despite the high p

Factors Associated with Nutritional Status in Grassroots Recyclers in Ecuador: A Machine Learning Approach.

Grassroots recyclers play a fundamental role in solid waste management in Ecuador; however, they often work under precarious conditions that may compromise their health. This study aimed to identify factors associated with nutritional status, operationalized as the presence or absence of nutritional alterations, among grassroots recyclers through supervised machine learning approaches. Data from 303 recyclers from three Ecuadorian cities (Cuenca, Macas, and La Libertad) were analyzed, incorporating sociodemographic, occupational, and health-related variables. Nutritional alterations were defin

Effects of the 2002 sniper attacks on the homeless population in Washington, DC.

OBJECTIVE: Despite the prevalence of homelessness, this population has rarely been included in disaster and terrorism planning. To better understand the mental health needs of the homeless during a terrorist event and to highlight the need to address methodological limitations in research in this area, we examined responses to the October 2002 Washington, DC, sniper attacks. METHODS: We interviewed 151 homeless individuals 1 year after the Washington, DC, sniper attacks. RESULTS: The majority (92.7%) was aware of the sniper events; 84.1% stayed informed through the media and 72.7% had someon

Living with risk and uncertainty: a qualitative phenomenological study of women's pregnancy and postpartum experiences in Saskatchewan during COVID-19.

BACKGROUND: The COVID-19 pandemic disrupted maternal healthcare globally, yet localized experiences within structurally constrained settings remain underexplored. In Saskatchewan, Canada, pre-existing inequities relating to geography and health care access shaped how women experienced pregnancy and early motherhood during the pandemic. This study explored the lived experiences of women who were pregnant and gave birth during the COVID-19 pandemic, with particular attention to how healthcare disruptions, social restrictions, and structural conditions shaped care experiences, distress, and resil

Student Involvement In Health-Related Conferences In Nigeria: A Scoping Review.

Participation in scientific conferences supports the career trajectories of healthcare students. Although these benefits are well established, student involvement in conferences in low- and middle-income countries has not been systematically examined. This scoping review mapped the extent to which health-related conferences held in Nigeria intentionally incorporated student-focused provisions. This was conducted in accordance with the Joanna Briggs Institute methodology and reported using the PRISMA ScR framework. Fully in-person and hybrid national and international health-related conferences

Aging alone: lifestyle and unmet needs among unpartnered middle-aged adults preparing for later life in urban Taiwan.

BACKGROUND AND OBJECTIVES: Population aging, delayed marriage, one-person households, and weakening assumptions of spouse-based care make unpartnered aging an emerging concern in Taiwan. This study examined how unpartnered middle-aged adults in urban Taiwan prepare for later life and developed two grounded models of solitary aging. RESEARCH DESIGN AND METHODS: A convergent, qualitatively weighted mixed-methods design was used. 11 unpartnered adults aged 43-56 completed interviews and questionnaires assessing demographics, subjective aging, personality traits, social connectedness, isolation,

The Perspective and Needs of Older Adults With Osteoporosis With a Recent Upper Extremity Fragility Fracture Concerning Fall Risk and Fear of Falling.

BACKGROUND AND PURPOSE: Osteoporosis increases the risk of fragility fractures, contributes to significant morbidity, and is a psychological challenge-fear of falling (FoF)-which can limit activity and engagement in rehabilitation. These challenges are often overlooked after a fracture, despite their relevance to rehabilitation planning. Understanding these challenges is crucial for health care practitioners, particularly physical and occupational therapists, who are central to fall prevention and post-fracture care. This study explored the perspectives of older adults with osteoporosis who ha

Implementing a randomised controlled trial through community pharmacies to support people living with severe and persistent mental illness: Lessons learnt during the COVID-19 pandemic.

Public health orders were introduced in many countries, including Australia, during the COVID-19 pandemic to reduce the spread of the virus. However, for many people this led to an exacerbation of mental health symptoms, particularly those living with severe or persistent mental illness (SPMI). Additionally, the conduct of clinical research was severely impacted during the pandemic, with many difficulties encountered in the conduct of clinical trials. This paper describes the COVID-related impacts experienced during the implementation of a randomised controlled trial (RCT) testing the effectiv

Maternal Health in Sri Lanka: 75 years of national commitment towards excellence.

Sri Lanka has a legacy of religious and cultural practices promoting health, and its rulers have been responsive to health needs of the populace. The healthcare milieu that prevailed in the pre-colonial and colonial periods favorably influenced the evolution of maternal health in the last 75 years. Since independence, maternal health in the country improved in many dimensions and directions, in the backdrop of multiple sociodemographic changes and geopolitical fluxes, while far-reaching advances in the medico-technological and communication fields were taking place at global level. [Supporting caregivers in the age of AI: the role of an ethical digital companion].

In psychiatry, mental exhaustion among caregivers stems from exposure to suffering, suicide risk, and violence, as well as organizational constraints. Despite numerous support systems, teams describe the services available as unclear and difficult to access. After describing the gray area of exhaustion, between individual experience, institutional norms, and demands for resilience, guidelines for framing digital or artificial intelligence projects presented as support for well-being can be proposed. An action plan by the Strategic Partners Initiative for Data and Digital Health, based on opera

Estimating Active Transportation Behaviors to Support Health Impact Assessment in the United States.

Health impact assessment (HIA) has been promoted as a means to encourage transportation and city planners to incorporate health considerations into their decision-making. Ideally, HIAs would include quantitative estimates of the population health effects of alternative planning scenarios, such as scenarios with and without infrastructure to support walking and cycling. However, the lack of baseline estimates of time spent walking or biking for transportation (together known as "active transportation"), which are critically related to health, often prevents planners from developing such quantit
