Heart failure documentation in outpatients with diabetes and volume overload: an observational cohort study from the Diabetes Collaborative Registry.

BACKGROUND: Heart failure is a common and devastating complication of type 2 diabetes (T2D). Prompt recognition of heart failure may avert hospitalization, facilitate use of guideline-directed therapies, and impact choice of T2D medications. We sought to determine the rate and factors associated with heart failure documentation in T2D patients with evidence of volume overload requiring loop diuretics. METHODS: DCR is an on-going, prospective US registry of outpatient T2D patients from > 5000 cardiology, endocrinology, and primary care clinicians (current analysis used data from 2013-2019). Am

Calibration of thresholds for functional engagement of vagal A, B and C fiber groups in vivo.

UNLABELLED: Vagal nerve stimulation is widely used therapeutically but the fiber groups activated are often unknown. AIM: To establish a simple protocol to define stimulus thresholds for vagal A, B and C fibers. METHODS: The intact left or right cervical vagus was stimulated with 0.1 ms pulses in spontaneously breathing anesthetized rats. Heart and respiratory rate responses to vagal stimulation were recorded. The vagus was subsequently cut distally, and mass action potentials to the same stimuli were recorded. RESULTS: Stimulating at either 50 Hz for 2 s or 2 Hz for 10 s at experimentally

[Structural and functional state of the left and right ventricle and the factors that affect the parameters of intracardiac hemodynamics in patients with hypertension and chronic alcohol intoxication].

The parameters of endocardiac hemodynamics, heart rate variability, endothelial function and function of the external breathing (FEB) were studied. 67 patients with arterial hypertension (AH) at the stage II and with chronic alcohol intoxication (CAI) were observed. Method of correlation analysis allowed to determine the heart rate variability effect on the hemodynamics, endothelial function and respiratory systems in these patients. Patients with AH and CAI in comparison with those without the syndrome of CAI had dilatation of right ventricle (RV) and left auricle and aorta on a background of

Vagus-induced changes in ventricular electrophysiology of the dog heart with and without beta-blockade.

We tested the electrophysiologic effects of vagus stimulation in six dogs in the presence or absence of beta-adrenoreceptor blockade. Monophasic action potentials were obtained by the suction electrode technique and programmed electrical stimulation was used to determine heart rate-independent changes in 50 and 90% repolarization times and in the effective (V-ERP) and functional (V-FRP) refractory periods of the right ventricle. Vagus stimulation decreased heart rate both in the presence and in the absence of beta-adrenoceptor blockade. It also increased action potential duration and refractor

Personalized ECG-based artificial intelligence models for early seizure prediction.

Seizure prediction is critically important, as it can help prevent serious injuries, improve quality of life, and potentially reduce the risk of SUDEP. Autonomic fluctuations are known to occur during the preictal and early ictal phases; however, the pattern and magnitude of these changes vary considerably across patients and seizure types. This highlights the need for developing patient-specific ECG-based seizure prediction models. In this study, ECG data from 30 patients who had at least six recorded seizures during presurgical VEEG evaluation were used to develop both machine learning and d

Omega-3 fatty acids for the prevention of recurrent symptomatic atrial fibrillation: results of the FORWARD (Randomized Trial to Assess Efficacy of PUFA for the Maintenance of Sinus Rhythm in Persistent Atrial Fibrillation) trial.

OBJECTIVES: The aim of this study was to evaluate the efficacy of polyunsaturated fatty acids (n-3 PUFA) for the prevention of recurrent atrial fibrillation (AF) in patients with normal sinus rhythm. BACKGROUND: Current pharmacological treatments to limit recurrent AF in patients with previous AF have limited efficacy and high rates of adverse events. Results of trials that tested the efficacy of n-3 PUFA provided heterogeneous results. METHODS: This was a prospective, randomized, double-blind, placebo-controlled, multicenter trial involving 586 outpatient participants with confirmed symptom

Baroreflex sensitivity and its response to deep breathing predict increase in blood pressure in type 1 diabetes in a 5-year follow-up.

OBJECTIVE: We have recently demonstrated that early autonomic dysfunction, defined as low baroreflex sensitivity (BRS), could be functional and reversible. However, potential temporal changes in BRS have not yet been addressed by longitudinal studies in type 1 diabetes. Moreover, it is not known whether low BRS predisposes to hypertension or other nonfatal diabetes complications. RESEARCH DESIGN AND METHODS: We conducted a 5-year prospective study including 80 patients with type 1 diabetes. We measured ambulatory blood pressure and autonomic function tests. BRS was assessed by six different m

Normovolaemia defined by central blood volume and venous oxygen saturation.

1. The intravenous administration of fluid and blood has to balance the danger of unexpected death in response to a reduction of central blood volume (CBV) against that of developing pulmonary and/or peripheral oedema. 2. The initial cardiovascular response to haemorrhage is similar to that developed in response to standing. In the upright position, adults are subjected to a reduction of CBV of approximately 0.5 L and can therefore tolerate a blood loss of approximately 1 L when supine. 3. However, volume administration directed by cardiovascular variables is seldom precise, even with integrat

A model-based analysis of autonomic nervous function in response to the Valsalva maneuver.

The Valsalva maneuver (VM) is a diagnostic protocol examining sympathetic and parasympathetic activity in patients with autonomic dysfunction (AD) impacting cardiovascular control. Because direct measurement of these signals is costly and invasive, AD is typically assessed indirectly by analyzing heart rate and blood pressure response patterns. This study introduces a mathematical model that can predict sympathetic and parasympathetic dynamics. Our model-based analysis includes two control mechanisms: respiratory sinus arrhythmia (RSA) and the baroreceptor reflex (baroreflex). The RSA submodel

α7 nicotinic receptors play a role in regulation of cardiac hemodynamics.

The α7 nicotinic receptors (NR) have been confirmed in the heart but their role in cardiac functions has been contradictory. To address these contradictory findings, we analyzed cardiac functions in α7 NR knockout mice (α7-/-) in vivo and ex vivo in isolated hearts. A standard limb leads electrocardiogram was used, and the pressure curves were recorded in vivo, in Arteria carotis and in the left ventricle, or ex vivo, in the left ventricle of the spontaneously beating isolated hearts perfused following Langedorff's method. Experiments were performed under basic conditions, hypercholinergic con

In vivo electrophysiological characterization of TASK-1 deficient mice.

BACKGROUND/AIMS: TASK-1 is a potassium channel predominantly expressed in heart and brain. We have previously shown that anesthetized TASK-1(-/-)mice have prolonged QT intervals in surface electrocardiograms (ECGs). In addition, heart rate variability quantified by time and frequency domain parameters was significantly altered in TASK-1(-/-) mice with a sympathetic preponderance. Aims of the present study were the analysis of QT intervals by telemetric ECGs, to determine potential influences of anesthesia and β-adrenergic stimulation on repolarization in surface ECGs, to investigate in vivo el

Let's Get Physical: Myocardial Stress Tests-A Student's Perspective.

Myocardial perfusion imaging is a routine study that helps determine whether blood flow to the heart muscle is normal or abnormal. There are 3 parts to myocardial imaging: a resting scan with a radioactive tracer, stressing of the myocardium, and a stress scan with a radioactive tracer. For the resting scan, a radioactive tracer is injected into the patient to obtain a set of images of the myocardium at rest. After the resting scan comes stressing of the myocardium. There are 2 different ways to stress the myocardium. The first is to have the patient exercise on a treadmill following a specifi

Emerging role of ivabradine for rate control in atrial fibrillation.

Control of ventricular rate is recommended for patients with paroxysmal, persistent, or permanent atrial fibrillation (AF). Existing rate-control options, including beta-blockers, nondihydropyridine calcium channel blockers, and digoxin, are limited by adverse hemodynamic effects and their ability to attain target heart rate (HR). Ivabradine, a novel HR-controlling agent, decreases HR through deceleration of conduction through If ('funny') channels, and is approved for HR reduction in heart failure patients with ejection fraction less than 35% and elevated HR, despite optimal pharmacological t

Faster Heart Rate Recovery With Increased RPE: Paradoxical Responses After an 87-km Ultramarathon.

The aim of this study was to determine the relationship between heart rate recovery (HRR) and an acute training "overload" by comparing HRR responses before and after an ultramarathon road race. Ten runners completed a standardized laboratory protocol ∼7 days before and between 2 and 4 days after participating in the 87-km Comrades Marathon. The protocol included muscle pain ratings, a 5-bound test, and 20 minutes of treadmill exercise at 70% of maximal oxygen uptake followed by 15 minutes of recovery. Respiratory gases and heart rate measurements were used to calculate steady-state exercise r

Predicting first-trimester spontaneous abortion. Ratio of mean sac diameter to crown-rump length compared to embryonic heart rate.

First-trimester spontaneous abortions have been predicted from alterations in the gestational sac size and embryonic heart rate. To evaluate which parameter or combination of parameters has the greatest predictive value, a prospective study was carried out during the first trimester of pregnancy. Pregnancy sac size, crown-rump length and heart rate were measured in 603 embryos, of which 580 continued beyond 13 weeks' gestation and 23 ended in first-trimester spontaneous abortions. Based on the continuing pregnancies, we constructed nomograms relating the ratio of mean sac diameter to crown-rum
