[Ultrasonography of the borderline between normal and pathological state of the hip in newborn infants (borderline hip)].

Infant hips are classified, according to Graf, in 4 US types on the basis of the morphologic changes in both the cartilaginous and the bony roofs (type I, II, III, IV). Out of 6,000 examined hips, 170 (2.8%) were considered, which could be classified neither as type I (mature) nor as type II (delayed/immature ossification). These hips were called borderline hips. They exhibited some characteristic US features: good bone modeling, rounded cotyle, and alpha angle 60 degrees +/- 2. They were always observed during the first month of the patients' life. Anamnestic data were not specific (27.5% bre…

[Real-time sonography of the infant hip joint in the early diagnosis of congenital hip dysplasia].

The ultrasonic examination of infant hip joint means a great advantage in early diagnosis of congenital hip dysplasia. The sonographic type classification by Graf enables the experienced examiner to make up a differentiated diagnostic-therapeutic concept as early as possible. Therapeutic omissions just as well as exaggerated therapeutic measures can be avoided. Consequently the prognosis of hip joint dysplasia is considerably improved by ultrasonic examination of new-born hip joints.

[Development of the S-form hip brace of Wakayama Medical College type for osteoarthritis of the hip].

In order to improve the therapeutic results of conservative treatment for coxarthrosis and to start functional training soon after surgery, the S-form hip brace of Wakayama Medical College type has been developed with due regard to the instability of the hip joint which is related to the origin of coxarthrosis and to biomechanics of the hip joint. Functional investigation was made by electromyography, etc. Even healthy people tend to develop gradual lack of stability at the hip joint with aging and the tendency is more pronounced in secondary coxarthrosis. This brace has been designed to reinf…

Morphometric study of the fetal development of the human hip joint: significance for congenital hip disease.

Hip joints (280) from 140 human fetuses, obtained from abortions and deaths in the perinatal period, were studied. The fetuses ranged from 8.7 to 40 cm in crown-rump length and are believed to be between 12 and 42 weeks in age. The joints were dissected, morphology inspected, and measurements taken of the depth and diameter of the acetabulum, the diameter of the femoral head, length and width of the ligament of the head, the neck-shaft, and torsion angles of the proximal femur. Regression models were fitted to determine which would best predict the growth pattern. Multivariate analysis of vari…

Accessory Portals in Hip Arthroscopy.

Over 18 portals have been described in the literature for arthroscopic access to the central and peripheral compartments of the hip. While basic hip arthroscopy procedures may only warrant 2 or 3 portals, accessory portals may facilitate improved access, facile instrumentation, safe anchor placement, and suture management for a variety of procedures ranging from basic cam resection and labral repair to more advanced complex procedures such as arthroscopic labral and capsular reconstruction. This article defines common accessory portals and deconvolutes historical nomenclature and further descr…

"Squeaky Hip": Are Noises Important?

We report on an 82-year-old female who developed painless "squeaking" and a grinding sensation 23 years after a ceramic-on-metal total hip arthroplasty. Radiographs revealed linear wear through the polyethylene insert. During revision, we replaced the acetabular construct with a dual mobility system. Intraoperatively, the acetabular bone damage was significant but contained, and the ceramic head had atypical wear after the polyethylene liner broke and was articulating directly with the titanium shell. The metallosis in the surrounding tissue was surprisingly minimal despite direct ceramic-on-m…

[Hip arthroscopy for painful endoprosthesis].

OBJECTIVE: Hip arthroscopy for a painful endoprosthesis is performed to confirm and evaluate or detect pathologies (sampling for microbiology/histology, function), which are treated during the procedure or can indicate treatment options for the further course of the procedure. INDICATIONS: Hip arthroscopy is indicated for the diagnostics of unclear persistent pain after implantation of a hip endoprosthesis (low-grade infection, metal reaction/metallosis, loosening/misplacement of prosthesis components) and for treatment (iliopsoas impingement, removal of biomechanically disruptive osteophytes…

[Management of pediatric hip fractures].

Pediatric hip fractures are rare and pose significant clinical challenges in their treatment. These fractures typically result from high-energy trauma, while low-energy mechanisms should raise suspicion for pathological fractures or non-accidental injury. Pediatric hip fractures are associated with high complication rates, including avascular necrosis, coxa vara, non-union, and leg-length discrepancies. These complications arise from the unique anatomy of the proximal femur in children, particularly the presence of the growth plate and the vulnerable arterial blood supply. Early surgical inter…

Regional Anesthesia for Hip Arthroscopy.

Pain after hip arthroscopy can be severe, yet we lack a consensus method for non-narcotic analgesia. Here we describe anatomic elements of hip arthroscopy and our current understanding of the relevant sensory innervation as a prelude to the evaluation of locoregional analgesic techniques. Many regional nerve blocks and local anesthetic infiltration techniques are reviewed, including 2 newer ultrasound fascial plane blocks. Further study of targeted, motor-sparing approaches, either ultrasound-guided or under direct surgical visualization is needed.
