Predictors of renal function in primary hyperparathyroidism.

CONTEXT: Current guidelines for parathyroidectomy in primary hyperparathyroidism (PHPT) include an estimated glomerular filtration rate (eGFR) less than 60 mL/min per 1.73 m(2). Although the biochemical abnormalities associated with PHPT could impair renal function, there are currently no data examining whether more severe hypercalcemia, hypercalciuria, or nephrolithiasis are associated with chronic kidney disease (CKD) in mild PHPT. OBJECTIVE: This cross-sectional study evaluated predictors of renal function in PHPT. DESIGN: This is a case series of PHPT patients with (eGFR < 60 mL/min per …

[Diagnosis and treatment strategies on primary hyperparathyroidism].

OBJECTIVE: Through analyzing the 13 cases with primary hyperparathyroidism (PHPT) retrospectively, We detected the reasons of the misdiagnosis and the strategy of diagnosis and treatment for the disease. METHOD: Thirteen cases of clinical materials on PHPT were collected from 2002 to 2012. Initial symptom, laboratory examination and imaging findings were analyzed. Thirteen cases of patients with PHPT all were performed surgery. The period of postoperative follow up was from 3 to 24 months, averaged for 12.2 months. RESULT: Surgeries on 13 patients were successful, including 12 cases of parat…

Ultrasound in clinical setting of secondary hyperparathyroidism.

Secondary hyperparathyroidism (sHPT) is one of the most common and serious complications of chronic kidney disease (CKD) and maintenance hemodialysis (MHD). In sHPT, the biology of parathyroid cells changes significantly toward diffuse and nodular hyperplasia. Diagnosis and treatment of sHPT are based on intact parathyroid hormone (i-PTH) serum levels and on the parameters of mineral metabolism. The morphological diagnosis of sHPT relies on 2 complementary imaging techniques: high-resolution ultrasonography with color Doppler imaging (US/CD) and 99mTc-methoxyisobutylisonitrile (MIBI) scintigra…

Diagnosis and management of primary hyperparathyroidism in Europe.

BACKGROUND: There is continued debate as to the optimal strategy for diagnosis and management of primary hyperparathyroidism (PHPT). AIM: To compare the strategies used for the diagnosis and management of PHPT by physicians in five European countries. DESIGN: Questionnaire-based survey. METHODS: Physicians in France, Germany, the UK, Italy and Spain were invited to participate in the survey which was conducted using a web-based interface and were included in the evaluation if they had treated a minimum of four patients suffering from PHPT in the past year. RESULTS: A total of 421 physician…

Cost-effectiveness of parathyroidectomy for primary hyperparathyroidism.

OBJECTIVE: To review cost-effectiveness analyses of treatments for primary hyperparathyroidism (PHPT). METHODS: The PubMed database was searched with the keywords "cost effectiveness" and "primary hyperparathyroidism." The results of all studies identified were reviewed to ensure that the cost-effectiveness data were reported using incremental cost-effectiveness ratios, which are critical metrics for formal cost-effectiveness analysis. RESULTS: Six studies were identified that measured the cost-effectiveness of competing treatment strategies for PHPT with incremental cost-effectiveness ratio…

Nephrolithiasis and renal calcifications in primary hyperparathyroidism.

CONTEXT: Renal complications in terms of hypercalciuria, nephrolithiasis, and nephrocalcinosis are well-known risks in primary hyperparathyroidism (PHPT) and may lead to impaired renal function. EVIDENCE ACQUISITION: We reviewed published evidence on the occurrence, pathophysiology, and consequences of renal complications in PHPT and highlighted areas of uncertainty that should be investigated further. EVIDENCE SYNTHESIS: In asymptomatic PHPT, renal stones are present in approximately 7% of the patients, which is a significantly higher prevalence than among patients without PHPT (1.6%). Also…

Preoperative preparation of patients with hyperparathyroidism as comorbidity.

Preoperative preparation of patients with hyperarathyroidism planned to be operated and/or already operated because of some other disease have specific characteristics in function of the type of hyperparathyroidism, primary or secondary. In primary hyperparathyroidism, repercussions of pronounced hypercalcemia on organs and systems are of essential importance. The most important aspect of preoperative preparation of these patients is therefore the treatment of hypercalcemia. In patients with secondary hyperparathyroidism as comorbidity, calcium level is of lesser importance since it stays most…

Unusual presentation in a case of primary hyperparathyroidism.

This report describes a case of classic severe primary hyperparathyroidism (PH) with clinical presentation that is very infrequent nowadays, which was osteitis fibrosa cystica. As bone scintigraphy demonstrated multiple areas of increasing uptake associated with hypercalcemia, a thorough investigation was conducted to exclude the neoplasms which most frequently are responsible for bone secondarisms. A fludeoxyglucose (FDG) positron emission tomography/CT demonstrated diffuse and multiple foci of increased FDG uptake and a focal uptake at the left thyroid region. Parathyroid function was studie…

Clinical Use of Cinacalcet in MEN1 Hyperparathyroidism.

Background. Management of multiple-endocrine neoplasia type 1- (MEN1-) associated hyperparathyroidism is associated with high recurrence rates and high surgical morbidity due to multiple neck explorations. Cinacalcet, a calcimimetic agent licensed for the treatment of secondary hyperparathyroidism and parathyroid carcinoma, may provide a medical alternative for the management of these complex patients. Methods. A prospective audit was performed of eight patients; three males and five females, aged 20-38 at diagnosis. Two patients commenced cinacalcet as primary treatment and six had previous s…

Reoperation for primary hyperparathyroidism: tips and tricks.

Reoperation for primary hyperparathyroidism (PHPT) remains a major challenge for both the patient and surgeons. Before considering reoperation, the surgeon must confirm the diagnosis of PHPT and assess patient risk factors. The goal of reoperative surgery is to excise the abnormal parathyroid gland(s) and limit exploration to help minimize the potential complications. At least two positive and concordant localizing studies should be available before reoperation, but the surgeon must keep in mind that the operative and histology reports from previous operation are the first localization techniq…

Primary hyperparathyroidism as a secondary cause of depression.

A 27-year-old woman was initially diagnosed and treated for depression with suicide ideation. Thirteen months later, kidney stones, an elevated parathyroid hormone, and elevated calcium levels led to a diagnosis of primary hyperparathyroidism. The patient was treated for hyperparathyroidism by resection of the superior right parathyroid gland. When the calcium levels were regulated, the patient's moods, concentration, and memory were back to baseline.

Brown tumour of the wrist with primary hyperparathyroidism.

Employers commonly use adjustments to health insurance premiums as incentives to encourage healthy behavior, but the effectiveness of those adjustments is controversial. We gave 197 obese participants in a workplace wellness program a weight loss goal equivalent to 5 percent of their baseline weight. They were randomly assigned to a control arm, with no financial incentive for achieving the goal, or to one of three intervention arms offering an incentive valued at $550. Two intervention arms used health insurance premium adjustments, beginning the following year (delayed) or in the first pay p

Recurrent pancreatitis induced by hyperparathyroidism in pregnancy.

INTRODUCTION: As primary hyperparathyroidism affects mainly middle-aged and elderly women, it is an infrequent finding during gestation and breastfeeding. To date, less than 200 pregnant patients with primary hyperparathyroidism diagnosed during pregnancy have been described. Like in other disorders of the parathyroid gland, the recognition of primary hyperparathyroidism during pregnancy and lactation may be difficult, as clinical symptoms are not specific, while laboratory findings may be masked by some typical pregnancy-induced changes in calcium and phosphate homeostasis. If remains untreat…

Alendronate therapy in men with primary hyperparathyroidism.

OBJECTIVE: To determine the skeletal effects of alendronate therapy in men with primary hyperparathyroidism (PHPT) in comparison with those in postmenopausal women. METHODS: There essentially are no published data on the effects of bisphosphonate therapy in men with PHPT. We previously conducted a double-blind, randomized, single- crossover trial of alendronate, 10 mg daily, in PHPT and reported that alendronate significantly increases bone mineral density (BMD) at 12 months relative to baseline values. That study sample included both women (n = 28) and men (n = 9) and both premenopausal (n =…
