A dynamic framework for the study of optimal birth intervals reveals the importance of sibling competition and mortality risks.

Human reproductive patterns have been well studied, but the mechanisms by which physiology, ecology and existing kin interact to affect the … ality in different populations decreases interbirth intervals. We show that sibling competition and infant mortality interact to lengthen interbirth intervals. In lower-mortality populations, intense sibling competition pushes births further apart. Varying the adult risk of mortality alone has no effect on birth intervals between populations; competition between o …

Impact evaluation of a maternal and neonatal health training intervention in private Ugandan facilities.

Global and country-specific targets for reductions in maternal and neonatal mortality in low-resource settings will not be achieved without improvements in the quality of care for optimal facility-based obstetric and newborn care. This global call includes the private sector, which is increasingly serving low-resource pregnant women. The primary aim of this study was to estimate the impact of a clinical and management-training programme delivered by a non-governmental organization [LifeNet International] that partners with clinics on adherence to global standards of clinical quality during lab

Effect of healthcare spending on the relationship between the Human Development Index and maternal and neonatal mortality.

BACKGROUND: Several factors affect morbidity and mortality the world over. Previous research shows mortality rates are higher among individuals of lower socio-economic status. We investigated the trajectory of neonatal (NM) and maternal (MM) mortality between 2010 and 2014 and the effect of healthcare spending on the relationship between the Human Development Index (HDI) and NM and MM. METHODS: Data were obtained from the United Nations Development Program and World Bank. Latent growth curve models (LGCMs) were estimated to determine the trajectory of NM and MM across the study period and the

Retarded fetal growth patterns and early neonatal mortality in a Mexico City population.

The study reported here classified 9,660 newborn infants delivered at a maternal and child health center in Mexico City by length of gestation, presence or absence of growth retardation, and (in the case of growth-retarded infants) proportionate or disproportionate growth retardation in terms of the infants' weight and length. It was found that preterm infants (delivered before 38 weeks of gestation) had nine times the early neonatal mortality of term infants, irrespective of growth retardation patterns. Also, the type of fetal growth retardation involved (proportionate or disproportionate) in

Effect of Fetal Growth on 1-Year Mortality in Neonates With Critical Congenital Heart Disease.

Background Infants with critical congenital heart disease ( CCHD ) are more likely to be small for gestational age (GA). It is unclear how this affects mortality. The authors investigated the effect of birth weight Z score on 1-year mortality separately in preterm (GA <37 weeks), early-term (GA 37-38 weeks), and full-term (GA 39-42 weeks) infants with CCHD . Methods and Results Live-born infants with CCHD and GA 22 to 42 weeks born in California 2007-2012 were included in the analysis. The primary predictor was Z score for birth weight and the primary outcome was 1-year mortality. Multivariabl

Deaths: final data for 1998.

OBJECTIVES: This report presents final 1998 data on U.S. deaths and death rates according to demographic and medical characteristics such as … for all other age groups, although the decline for children aged 1-4 years was not significant. The infant mortality rate was unchanged from 1997 at 7.2 infant deaths per 1,000 live births. CONCLUSIONS: The overall improvements in general mortality and life expectancy in 1998 continue the long-term downward trend in U.S. mortality. Although unchanged from 1997, t …

Precision-weighted estimates of neonatal, post-neonatal and child mortality for 640 districts in India, National Family Health Survey 2016.

BACKGROUND: The conventional indicators of infant and under-five mortality are aggregate deaths occurring in the first year and the first five years, respectively. Monitoring deaths by <1 month (neonatal), 1-11 months (post-neonatal), and 12-59 months (child) can be more informative given various etiological causes that may require different interventions across these three mutually exclusive periods. For optimal resource allocation, it is also necessary to track progress in robust estimates of child survival at a smaller geographic and administrative level. METHODS: Data on 259 627 children

Causes of deaths in children younger than 5 years in China in 2008.

BACKGROUND: Previous estimates of the global burden of disease for children have not included much information from China, leading to a large gap in data. We identified the main causes of deaths in neonates (<1 month), postneonatal infants (1-11 months), and children (<5 years) in China using information that was available to the public. METHODS: The Child Health Epidemiology Reference Group in collaboration with colleagues from Peking University systematically searched Chinese databases that were available to the public. Information was obtained from the Chinese Ministry of Health and Bureau

Increasing Prenatal Care Compliance in At-Risk Black Women: Findings from a RCT of Patient Navigation and Behavioral Incentives.

BACKGROUND: In the USA, infant mortality remains a major public health concern, particularly for Black women and their infants who continue to experience disproportionately high mortality rates. Prenatal care is a key determinant of infant health, with inadequate prenatal care increasing risk for prematurity, stillbirth, neonatal loss, and infant death. The aim of the present study was to determine if concurrent delivery of patient navigation and behavioral incentives to at-risk Black pregnant women could improve prenatal care attendance and associated maternal and infant outcomes. METHODS: P…

Necrotizing Enterocolitis in Infants with Hypoplastic Left Heart Syndrome Following Stage 1 Palliation or Heart Transplant.

Previous studies of necrotizing enterocolitis (NEC) among infants with hypoplastic left heart syndrome (HLHS) were conducted in single centers or had small sample sizes. This study aimed to determine the mortality rate and the risk factors for NEC among infants with HLHS who were discharged over a 10-year period (2004-2013) from 41 Pediatric Health Information System affiliated children's hospitals. Either stage 1 palliation and/or heart transplant were completed prior to patient's death or hospital discharge. We compared the characteristics of infants with HLHS who did not develop NEC and tho

Outcomes associated with surfactant in more mature and larger premature infants with respiratory distress syndrome.

OBJECTIVE: Examine the effect of off-label surfactant on mortality and morbidity in more mature and larger premature infants diagnosed with respiratory distress syndrome (RDS). STUDY DESIGN: Cohort study of premature infants born at 30-36 weeks, birth weight > 2 kg, and a diagnosis of RDS. We compared the odds of mortality and morbidity between infants who were exposed vs unexposed to surfactant. We used a treatment effects model to balance covariates between groups. RESULTS: Of 54,964 included infants, 25,278 (46%) were exposed to surfactant. The frequency of mortality and morbidities were

Cost-Effectiveness of Treatment for Opioid Use Disorder in Pregnancy and Its Impact on Birth Outcomes.

IMPORTANCE: For the first time in nearly 2 decades, the US infant mortality rate has increased, coinciding with a rise in overdose-related deaths as a leading cause of pregnancy-associated mortality in some states. Prematurity and low birth weight-often linked to opioid use in pregnancy-are major contributors. OBJECTIVE: To assess the health and economic impact of perinatal opioid use disorder (OUD) treatment on maternal and postpartum health, infant health in the first year of life, and infant long-term health. DESIGN, SETTING, AND PARTICIPANTS: This was a cost-effectiveness, population-bas…

Association between risk of infant death and birth-weight z scores according to gestational age: A nationwide study using the Finnish Medical Birth Register.

OBJECTIVE: To investigate the association between infant mortality and birth weight using estimated fetal weight (EFW) versus birth-weight charts, by gestational age (GA). METHODS: This nationwide population-based study used data from the Finnish Medical Birth Register from 2006 to 2016 on non-malformed singleton live births at 24-41+6 weeks of gestation (N = 563 630). The outcome was death in the first year of life. Mortality risks by birth-weight z score, defined as a continuous variable using Maršál's EFW and Sankilampi's birth-weight charts, were assessed using generalized additive models…

Neonatal encephalopathy mortality rate in Aotearoa, New Zealand: review of national data 2010-2020.

BACKGROUND: Neonatal encephalopathy (NE) is a condition that affects newborn and can lead to long-term neurological impairments and death. Hypoxic ischaemic encephalopathy is the subset of NE with evidence of a hypoxic ischaemic insult. In Aotearoa New Zealand, national data have been collected for infants ≥37 weeks since 2010 and ≥35 weeks from 2016. Although the mortality rate associated with NE has not been a focus of annual reports, it is an important outcome for counselling whānau (families) and for international comparison. METHODS: The Perinatal and Maternal Mortality Review Committee

Lack of difference in neonatal mortality between blacks and whites served by the same medical care system.

To study the influence of health care systems on racial differences in low birthweight and neonatal mortality, an historical cohort analysis was conducted using birth and linked birth and death certificates of infants delivered in Pierce County, Washington, between 1982 and 1985. Overall, black infants had significantly higher rates of low birthweight than white infants. Black infants served by civilian medical care had approximately twice the neonatal mortality of white infants; however, black infants born in the military hospital had a neonatal mortality rate comparable to white infants. Trends in mortality caused by respiratory distress syndrome in the United States, 1969-83.

Using United States vital statistical data we examined trends in infant deaths from Respiratory Distress Syndrome/Hyaline Membrane Disease (RDS/HMD) for 1969 to 1983, by race and age at death. In order to improve comparability of diagnosis across two revisions of the International Classification of Diseases, deaths from RDS/HMD were ascertained using both underlying and associated causes of death. These data document a 2 per cent per year increase in infant mortality attributed to RDS/HMD for all races during interval I (1969-73) followed by 9 per cent per year decreases during intervals II (1…
