Since 1980, various treatments have been proposed for patients suffering from distal tubal infertility. Difficult choices between surgical (microsurgery/laparoscopy) treatments and in-vitro fertilization (IVF) still confront many workers. In this study, we evaluated the cumulative results of both therapeutic methods for this group of patients. Bioactive FSH and immunoreactive FSH were determined in 193 infertile men and in 23 men with proven fertility using the Sertoli cell aromatase bioassay for bioactive FSH measurement and a two-site fluoroimmunoassay for immunoreactive FSH measurement. Chromosomal abnormalities are found in a considerably high percentage of cases of male infertility, in particular azoospermia. We report a case of the XYY syndrome and review the literature. A 36-year-old man, a factory hand, presented with infertility. He was safely delivered at term as a fourth child when his father was 41 years old and his mother 38. We studied the relation between colonization with Mycoplasma hominis and Ureaplasma urealyticum, and the results of infertility studies in 205 women with involuntary infertility of at least one year's duration. During a three years period, evaluation of etiologic factors in 385 consecutive infertile men revealed chronic prostato-vesiculitis as the commonest cause (25.7%) followed by varicocele (21.8%). In 77% of the patients with chronic prostatitis the authors found a decreased spermatozoa motility rate. AN obstruction of the sperm duct was found in 7,8%, autoagglutination in 5.7%. Idiopathic infertility was diagnosed in 10.9% of the patients. The significance of these findings and the contributions of other causes in this series are discussed.

A simplified scheme for the management of anovulatory and of ovulatory (usually called unexplained) infertility was evaluated in 244 women. Eighteen patients were excluded because of primary ovarian failure, 164 were treated for ovulatory failure, and 62 with ovulatory infertility remained untreated. In a retrospective study of 163 women with endometriosis, 76% had a history of menorrhagia, a larger percentage than that which might be normally expected. Only 10% of the women complained of infertility, while 13% had used oral contraceptives regularly, a much smaller proportion than expected. These findings are discussed in terms of the pathogenesis of endometriosis and the possible prophylactic role of oral contraceptives. The authors suggest the possibility that increased sensitivity to estrogens may lead to the development of endometriosis in some women.

We studied 77 women with hyperprolactinemic infertility and possible ovulatory disturbances. Galactorrhea was present in 27. Ovulation was normal in 15, 21 were anovulatory and 41 had luteal phase deficiency. All patients received bromocriptine for three months, resulting in normal serum prolactin levels. After that time, if no pregnancy occurred, clomiphene (with or without human chorionic gonadotropin) or human menopausal gonadotropin and human chorionic gonadotropin were added to the treatment. Case records of 206 pregnant patients who had not had a viable pregnancy for 10 years or more, are presented. In the antenatal period, 59.7% of patients went through their pregnancy with no complications. The commonest complications were pre-eclampsia and non-proteinuric hypertension. Labour was essentially normal in the majority of cases, and of the 183 patients who were allowed to labour, 171 (93.4%) were delivered vaginally.
