The Israeli eye care system through a public and global health lens.

Global eye health is increasingly recognized as integral to achieving universal health coverage (UHC), with equity, access, and service integration serving as key indicators of system performance. Israel provides a distinctive case study, having established UHC through the 1994 National Health Insurance Law, which guarantees essential services to all residents. This review assesses Israel’s eye care system through a public and global health lens, applying the WHO health system building blocks to examine its organization, strengths, and persistent challenges. Israel benefits from strong integra

Screening patients requiring secondary lumbar surgery for degenerative lumbar spine diseases: a nationwide sample cohort study.

This study aims to identify healthcare costs indicators predicting secondary surgery for degenerative lumbar spine disease (DLSD), which significantly impacts healthcare budgets. Analyzing data from the National Health Insurance Service-National Sample Cohort (NHIS-NSC) database of Republic of Korea (ROK), the study included 3881 patients who had surgery for lumbar disc herniation (LDH), lumbar spinal stenosis without spondylolisthesis (LSS without SPL), lumbar spinal stenosis with spondylolisthesis (LSS with SPL), and spondylolysis (SP) from 2006 to 2008. Patients were categorized into two gr

Hyperthyroidism Due to Graves Disease After Radiofrequency Ablation.

Management options for benign, autonomously functioning, and malignant thyroid nodules were limited to surgery or targeting by radioactive iodine before the availability of radiofrequency ablation (RFA). Despite being a relatively new technique, RFA may be favored for patients of high surgical risk, and for those who wish to avoid hypothyroidism. Although insurance coverage for the procedure can be a significant barrier, several groups of investigators have shown improved quality of life for RFA compared to surgery, due to the less invasive nature and favorable risk profile. Hyperthyroidism du

Incidence and Risk of Carpal Tunnel Syndrome in End-Stage Renal Disease Patients on Dialysis: A Nationwide Population-Based Matched Cohort Study.

Carpal tunnel syndrome (CTS) is a common cause of compression neuropathy of the upper extremities. This retrospective nationwide matched cohort study was conducted using the data from the Taiwan National Health Insurance Research Database between January 2003 and December 2012. The incidence rate of CTS was 975.84 and 544.12/100,000 person-years, respectively, yielding an incidence rate ratio of 1.79 (95% confidence interval, 1.68-1.91, P < 0.0001). CTS patients with End-stage renal disease (ESRD) in the dialysis group were more likely to receive surgical intervention than those the control gr

[Sentinel lymph node biopsy and low axillary node sampling for breast cancer].

Since sentinel lymph node(SLN) biopsy has a higher negative predictive value than that of four-node sampling, SLN biopsy might become the new acknowledged standard of clinical care for patients with early breast cancer. SLN biopsy is widely used in Western countries despite the lack of data from randomized trials. Clinical practice guidelines document that SLN biopsy should be performed with prudent informed consent and thorough surgical technique. Before surgeons replace axillary dissection with SLN biopsy as the staging procedure at their institution, they should perform backup axillary diss

Social determinants of health influence clinical outcomes of patients undergoing rotator cuff repair: a systematic review.

BACKGROUND: Social determinants of health (SDOH) are the collection of environmental, institutional, and intrinsic conditions that may bias access to, and utilization of, health care across an individual's lifetime. The effects of SDOH are associated with disparities in patient-reported outcomes after hip and knee arthroplasty, but its impact on rotator cuff repair (RCR) is poorly understood. This study aimed to investigate the influences that SDOH have on accessing appropriate orthopedic treatment, as well as its effects on patient-reported outcomes following RCR. METHODS: This systematic re

Perioperative Antibiotics and Other Factors Associated with Postcesarean Infections: A Case-Control Study.

OBJECTIVE: Our objective was to determine modifiable risk factors associated with surgical site infection (SSI) and postpartum endometritis. We hypothesized that inappropriate surgical antibiotic prophylaxis would be a risk factor for both types of infections. STUDY DESIGN: This was a single-center case-control study of SSI and endometritis after cesarean delivery over a 2-year period from 2016 to 2017. Cases were identified by International Classification of Diseases, 10th Revision diagnosis codes, infection control surveillance, and electronic medical records search and were subsequently co

Oncoplastic breast reduction surgery decreases rates of reoperation with no increased medical risk.

The purpose of this study is to compare the oncologic, medical, and surgical outcomes of lumpectomy versus oncoplastic breast reduction surgery (OBRS) on a national scale. A national insurance-based database was queried for patients who had a lumpectomy with or without a same-day breast reduction by Current Procedural Terminology (CPT) codes. Patients were then matched by obesity, body mass index range, age, region, neoadjuvant chemotherapy, and outcomes were compared. There were 421,455 patients in the lumpectomy group and 15,909 patients in the OBRS group. After matching, 15,134 patients wer

Factors Influencing Robotic Surgery Adoption in the United States.

INTRODUCTION: Robotic surgery has expanded rapidly in the United States despite high financial commitments of a robotic surgery program. Factors that drive uneven robotic surgery adoption has not been adequately explored at the populational level. METHODS: A retrospective analysis was performed using the National Cancer Database to evaluate facility-level adoption of robotic colorectal surgery. Facilities were stratified by the timing of the first robotic colorectal case into adoption quartiles (1st: earliest to 4th: latest) and nonadopters. A multivariable ordinal logistic regression model w

Impact of Limited Access to Surgical Care in Medically Underserved Communities.

Summary/BackgroundMedically underserved communities and ethnic minorities constitute a significant portion of the vulnerable population within the United States. Recent changes in the health care structure, rising inflation with a decline in median household income, and the SARS-CoV-2 pandemic have disproportionately impacted communities of low socioeconomic status. Healthcare providers and federal organizations must be aware of how these factors influence access to surgical care to tailor treatment, interventions, and policies better to meet the needs of these populations.MethodWe systematica
