[Unusual presentation of metastatic osteosarcoma as a giant duodenal polyp. A case report].

INTRODUCTION: Osteosarcoma is a malignant bone neoplasm with an usually high metastatic potential. Besides the common metastatic sites such as lungs, bone, and pleura, metastases to unusual sites such as liver, brain and regional lymph nodes have also been reported with increasing frequency; among them, gastrointestinal metastases represent an extraordinarily rare event in the natural history of this neoplasia. MATERIALS AND METHODS: We describe a case of a 27 year old man, who was diagnosed with a grade IV osteoblastic osteosarcoma of the left tibia and submitted to 5 courses of pre-surgical

Metastatic osteosarcoma causing intussusception.

Osteosarcoma is the most common malignant bone neoplasm. It has a very high metastatic potential, and lungs are the most common site of metastasis followed by bones. Multiagent chemotherapy has been reported to alter the biological behavior of the disease so that the tumor metastasizes to more rare sites such as brain, bowel, and soft tissues. The authors report a case of jejuno-jejunal intussusception caused by metastases from osteosarcoma.

Cronkhite-Canada syndrome: report of two cases, biopsy findings in the associated alopecia, and a new treatment option.

Two patients with Cronkhite-Canada syndrome (CCS) are reported, both of whom had diffuse alopecia, nail and skin changes, gastrointestinal polyposis, diarrhea, and wasting. A scalp biopsy was performed in one patient, and the specimen showed a marked noninflammatory loss of follicular units, miniaturization of the hair shafts, markedly dilated follicles, and a heavy deposition of glycosaminoglycans in the reticular dermis. This patient responded to prednisone therapy. The other patient was found to have elevated gastric acid levels and responded to ranitidine therapy. The conditions of both pa

Jejunal inflammatory fibroid polyp presenting as intussusception--a case report with review of the literature.

A 52-year-old woman was presented with intermittent abdominal pain and vomiting for 10 days. Abdominal CT scan disclosed a dilated small bowel loop with a round solid mass in the right anterior supravesical space. The clinical impression was intussusception caused by small bowel tumor. She underwent an exploratory laparotomy. The macroscopic and microscopic findings confirmed an inflammatory fibroid polyp of jejunum causing intussusception. To the best of our knowledge, this was the 5th reported case of such a presentation in English medical literature.

Peutz-Jeghers syndrome. A call for intraoperative enteroscopy.

Peutz-Jeghers syndrome is a disease manifested by a combination of mucocutaneous pigmentation and gastrointestinal (GI) polyposis. The major morbidity results from intussusception, obstruction, and bleeding. Standard surgical management has been to perform enterotomies at the site of palpable polyps. A method of treating Peutz-Jeghers syndrome surgically with combined intraoperative enteroscopy is presented. The patient had eight large polyps that required three enterotomies for removal. Only three of the polyps were palpable. Two intussusceptions were reduced. The advantages of intraoperative

Colonic filiform polyposis and a malignant adenomatous polyp.

We have reported a case of pseudopolyposis in the unusual form of filiform polyps in the transverse colon with an adenomatous polyp in the sigmoid colon that had a focus of malignant degeneration. The patient did not have any significant past history of inflammatory bowel disease.

Invasive balantidiasis presented as chronic colitis and lung involvement.

A unique case of chronic balantidiasis is described, presenting with chronic colitis and inflammatory polyposis of the rectum and sigmoid colon and an intrapulmonary mass. Histology of the colonic polyps showed Balantidium coli, and both Aspergillus and Balantidium coli were found in the aspirate of the pulmonary mass. The patient was treated with doxycycline HCl 100 mg/day for 10 days with complete clinical recovery and marked improvement of the endoscopic appearance of the colonic mucosa.

Ornithine decarboxylase as a marker for colorectal polyps and cancer.

There is as yet no specific chromosomal abnormality or gene marker identified for colorectal polyps and cancer. Thus available markers include only phenotypic markers. Tumor markers that have been studied include tetraploidy and increased colonic mucosal proliferation; and these markers have identified those patients that are at high risk for colon cancer. The current "gold standard" of colorectal cancer markers is the carcinoembryonic antigen (CEA). CEA is best used as a monitor of disease and recurrence, and not as a screening or diagnostic test. Newer carbohydrate markers include CA 19-9, i

Cecal polyp and appendiceal intussusception in a child with recurrent abdominal pain: diagnosis by colonoscopy.

A 7-year-old boy developed recurrent abdominal pain. He was eventually discovered to have an inverted appendix via colonoscopy. At surgery, a polyploid intracecal mass was palpated at the base of the partially invaginated appendix. Resection of the cecal mass (histologically, a juvenile polyp) and appendix was easily accomplished. The types of clinical presentation and treatment of children with appendiceal intussusception are discussed.

Optimum overhead views in double-contrast barium-enema examinations.

On 100 consecutive double-contrast barium-enema examinations demonstrating a benign or malignant mass in the colon, 10 overhead views were analyzed to determine the smallest number of projections that would permit an accurate diagnosis. The combination of six overhead views (anteroposterior, posteroanterior, angled view of the rectosigmoid, lateral view of the rectum, right and left decubitus) and fluoroscopic spot films of the cecum, transverse colon, and flexures was sufficient to demonstrate all colonic masses. In no instance was a lesion seen only on overhead views performed with the patie

Fused inflammatory polyps simulating carcinoma in ulcerative colitis.

A 33-year old female with a 10-year history of ulcerative colitis developed a large polypoid filling defect in the distal transverse colon, suggestive of carcinoma of the colon. Biopsy indicated an inflammatory polyp but subtotal colectomy was performed because of the long history and the radiographic appearance. Pathologic examination revealed an unusual aggregation of filiform polyps entrapping a semisolid fecal mass. The development of this unique lesion is elaborated.

Colonic adenomas--a colonoscopy survey.

A colonoscopy survey of 620 patients with 1049 colon adenomas showed a predominantly left-sided distribution (77%). Of these lesions 97% were amenable to endoscopic removal or ablation. Sixty per cent of patients presented with rectal bleeding as their major symptom. There was agreement between radiology and colonoscopy in only 62% of patients, as many of the studies were single contrast barium enemas, performed before referral. Forty-eight per cent of adenomas in our series were less than 1.0 cm in diameter. Of the larger adenomas (greater than 2.0 cm in diameter) 66% were situated in the sig
