Enhancing Detection and Management of Spontaneous Bilateral Ectopic Pregnancies: Insights From a Rare Case.

BACKGROUND Ectopic pregnancy is a common gynecological emergency occurring when fertilization implants outside the uterine cavity. Bilateral tubal ectopic pregnancy (BTP), in which there are simultaneous pregnancies in both fallopian tubes, is exceedingly rare. BTP diagnosis is challenging as symptoms mimic unilateral cases, and imaging often fails to identify both sites. No standardized follow-up protocols exist, leading to potential clinical oversight and delayed management. While BTP is more commonly associated with assisted reproductive technologies, spontaneous cases are even rarer. CASE

Right Hepatic Lobe Hypoplasia and Gangrenous Cholecystitis: Surgical Implications and Case Report.

BACKGROUND Hypoplasia of the right hepatic lobe is a rare congenital anomaly often discovered incidentally during imaging or surgery. This anomaly can be challenging during surgery, leading to intraoperative complications. Patient with right hepatic hypoplasia usually live normally without symptoms, but it can be associated with liver cirrhosis, portal hypertension, and gallstones. While the exact pathophysiological relationship between congenital hepatic hypoplasia and cholecystitis remains unclear, a few contributing factors have been proposed. These include mutations in the hepatocyte nucle

Turning the operating room into a mixed-reality environment: a prospective clinical investigation for cerebral aneurysm clipping.

OBJECTIVE: The overall aim of this study was to demonstrate the potential benefit of a novel mixed-reality-head-mounted display (MR-HMD) on the spatial orientation of surgeons. METHODS: In a prospective clinical investigation, the authors applied for the first time a new multicamera navigation technology in an operating room setting that allowed them to directly compare MR-HMD navigation to standard monitor navigation. In the study, which included 14 patients with nonruptured middle cerebral artery aneurysms, the authors investigated how intuitively and effectively surgical instruments could

A giant gastric stromal tumor with dizziness as the main complaint: A case report and literature review.

INTRODUCTION: Small gastrointestinal stromal tumors (GISTs) are often asymptomatic. However, large tumors can cause symptoms like abdominal pain and GI bleeding. We experienced a unique case where a giant GIST was incidentally found by CT scanning during emergency treatment for dizziness. PRESENTATION OF CASE: A 61-year-old man presented temporary dizziness after exercising three days ago before his admission. Enhanced CT scan of the abdomen and pelvis revealed a large circular mass in the right upper abdominal cavity, with a maximum cross-sectional size of approximately 132 mm × 155 mm. Biop

Hemodynamic Patterns of Spinal Cord Perfusion in Thoracoabdominal Aortic Aneurysm Repair.

Paraplegia in aortic surgery is due to its impact on spinal cord perfusion whose hemodynamic patterns (SCPHP) are not clearly defined. Detailed morphological analysis of vascular network and collateral network modifications within Monro-Kellie postulate due to the fixed theca confines was performed to identify SCPHP. SCPHP may begin with intraspinal "backflow" (I-BF), that is, hemorrhage from anterior and posterior spinal arteries, backward via the connected anterior and posterior radicular medullary arteries, through the increasing diameter and decreasing resistance of segmental arteries (SAs

Aspects of Clinical Malpractice in Endodontics.

The clinical dentistry and endodontic procedures involve very technique-sensitive procedures, therefore exposing the operator to risks of causes not only damage to patients but also leads to malpractice. Among various disciplines of dentistry, endodontics-related cases witness the most frequently filed malpractice claims. This is due to the fact that the endodontic treatment procedures involve operative and surgical procedures, using a variety of medicaments and techniques. The endodontic procedural errors can be preoperative errors (such as incorrect diagnosis and misinterpretation), intraope

Total joint replacement for neglected posterior knee dislocation following septic arthritis after arthroscopy.

This report presents the first case of a knee dislocation following septic arthritis after arthroscopy. A 65-year-old woman had an arthroscopy with irrigation and debridement (I&D) of the joint and microfracture for the chondral lesions. She had complaints of postarthroscopic infection but non-steroidal anti-inflammatory medication and local ice compression was recommended. She revisited her physician twice and at the last visit she had a large purulent effusion in her knee. The gram stain of the joint fluid aspirate demonstrated gram-positive cocci and the cultures grew methicilline-sensitive

The Weight of the Resected Gland Predicts Rate of Success After Image-Guided Focused Parathyroidectomy.

BACKGROUND: A recent study of focused minimally invasive parathyroidectomy (FPTX) in sporadic primary hyperparathyroidism (pHPT) using intraoperative parathyroid hormone (ioPTH) measurements shows that inadequate ioPTH drop and multiglandular disease are more commonly found when a first gland<200 mg is resected. Our aim was to study if a resected gland that weighed <200 mg was associated with an increased persistence rate after FPTX. METHODS: This is a cohort study of FPTX for pHPT performed in the period 1998-2013. FPTX was performed in patients with pHPT where Sestamibi and Ultrasound imagi

Endosalpingiosis in peritoneal washings in women with benign gynecologic conditions: thirty-eight cases confirmed with paired box-8 immunohistochemical staining and correlation with surgical biopsy findings.

BACKGROUND: To better define the cytomorphologic spectrum of endosalpingiosis in peritoneal washings (PWs) and thereby facilitate their distinction from well differentiated serous carcinoma, the authors examined PWs from women who underwent surgery and pathologic staging of lesions other than Mullerian malignancies and correlated the findings with surgical specimens. METHODS: This was a retrospective review of medical records and PW specimens from 100 consecutive patients who had PWs coded as both "endosalpingiosis" and "negative for carcinoma" between 2002 and 2012. Thirty-eight of these pat

Total intravenous anaesthesia has recently gained more interest in paediatric anaesthesia. However, the global experience with children is l … -controlled pumps. Large aberrations from the predicted values can occur in the individual patient. Intraoperative awareness is possible, however, its incidence is generally underestimated. Paravenous infusion and pump dysfunction are typical complications of an intravenous technique. A reduced incidence of postoperative vomiting and agitation are recognised advantages of …

Safety issues regarding early extubation after coronary artery bypass surgery.

Potential safety issues related to early extubation include the effect of changes in anesthetic management on intraoperative hemodynamics, stress responses and awareness, altered management in the control of pain, shivering and ischemia in the early postoperative period, and the risks of reintubation in patients who might require reoperation for bleeding. The literature does not implicate any technique necessary to facilitate early extubation as being associated with adverse outcome. Definitive outcome studies are only beginning to be presented, but the data so far suggest that early extubatio
