Patellar instability with recurrent dislocation due to patellofemoral dysplasia. Results after surgical treatment.

Twenty-one patients (25 knees) with recurrent dislocation of the patella due to patellofemoral dysplasia were treated with a combined distal and proximal realignment of the extensor mechanism of the knee. In addition to traditional medial transfer of the tibial tuberosity, lateral release, and medial joint capsule duplication, vastus medialis transposition and deepening of the intercondylar sulcus on the proximal joint surface of the femur were done. A follow-up ranging from 1-14 years (mean 6 years) found the results excellent or good in 22 knees, fair or poor in three. One redislocation occu

Congenital dislocation of patella with ipsilateral hip flexion-abduction deformity: a case report.

Iliotibial band tightness and its abnormal insertion on patella is one of the contributing factors in congenital dislocation of patella. Although hip flexion-abduction deformity is caused by a tight iliotibial band, as frequently seen in postpolio residual paralysis, an association of hip flexion-abduction deformity with congenital dislocation of patella has never been reported previously. Such a case was presented to us, and surgical repositioning of the patella was carried out by means of release of the tight iliotibial band from the patella besides other tight lateral structures along with

The Position of the Patella and Extensor Mechanism Affects Intraoperative Compartmental Loads During Total Knee Arthroplasty: A Pilot Study Using Intraoperative Sensing to Guide Soft Tissue Balance.

UNLABELLED: The achievement of a well-balanced total knee arthroplasty is necessary for long-term success. We hypothesize that the dislocation of the patella during surgery affects the distribution of loads in the medial and lateral compartments. Intraoperative load sensors were used to record medial and lateral compartment loads in 56 well-balanced TKAs. Loads were recorded in full extension, relaxed extension, at 45 and 90° of flexion at full gravity-assisted flexion, with the patella in four different positions: dislocated (everted and not), located, and located and secured with two retinac

Recent advances in the prevention, early diagnosis, and treatment of congenital dislocation of the hip in Japan.

Results of animal experiments and then of clinical study of newborn infants suggested that the main mechanical factor causing hip dislocation is prolonged tension on the hamstrings and iliopsoas and that each of these muscles has a synergistic effect in the production of hip dislocation during the perinatal period. It was, therefore, presumed that the high incidence of congenital dislocation of the hip (CDH) in Japan might be due mainly to the Japanese tradition of forcibly maintaining the legs of infants in an extended position with a "swathing diaper." The incidence of CDH in Japanese infant

Medial Patellofemoral Ligament Reconstruction: Indications, Technique, and Outcomes.

The medial patellofemoral ligament is the primary static restraint to lateral patellar translation. It is injured in 96% to 100% of patellar dislocations that affect approximately 6 to 29 of 100,000 patients and is more common in patients younger than 20 years of age. Risk factors for patellar dislocation include patella alta, trochlear dysplasia, genu valgus, increased Q angle, and hyperlaxity. The treatment for patellar instability depends on the clinical and radiographic findings and can be nonoperative for first-time dislocations (bracing, proximal strengthening, and progressive return to

Radiographic and computed tomographic analysis of the position of the tibial tubercle in recurrent dislocation and subluxation of the patella.

Among the factors that influence the patellofemoral alignment and stability, the position of the tibial tubercle was evaluated using plain radiographs and computed tomography (CT). Radiographs of the Q-angle with the knee in full extension and quadriceps muscle relaxed showed that mean Q-angles in the dislocation or subluxation group and control group were 13.8 degrees and 14.3 degrees, respectively. The major reason for this deceptive measurement was a patella positioned laterally in the dislocation or subluxation group. Better estimation was made with the modified Q-angle measurement by usin

Mosaicplasty for femoral osteochondritis dissecans.

The authors describe a surgical mosaicplasty technique, with an anterior surgical dislocation approach without trochanterotomy, for osteochondritis dissecans of the hip. A graft was taken from the lateral condyle of the knee. Two adolescents underwent this procedure with good results. No osteonecrosis was observed at the longest follow-up. Mosaicplasty is a useful treatment method for small osteochondritis dissecans (<2cm(2)).

Intramedullary rod and cement static spacer construct in chronically infected total knee arthroplasty.

Two-stage reimplantation, with interval antibiotic-impregnated cement spacer, is the preferred treatment of prosthetic knee joint infections. In medically compromised hosts with prior failed surgeries, the outcomes are poor. Articulating spacers in such patients render the knee unstable; static spacers have risks of dislocation and extensor mechanism injury. We examined 58 infected total knee arthroplasties with extensive bone and soft tissue loss, treated with resection arthroplasty and intramedullary tibiofemoral rod and antibiotic-laden cement spacer. Thirty-seven patients underwent delayed

Prevalence and patterns of anatomical risk factors in patients after patellar dislocation: a case control study using MRI.

OBJECTIVE: To assess anatomical risk factors in patients after lateral patellar dislocation (LPD) and controls using MRI. METHODS: MR images of 186 knees after LPD and of 186 age- and gender-matched controls were analysed. The presence of trochlear dysplasia was assessed by evaluation of trochlear inclination, facet asymmetry, and trochlear depth; patella alta was evaluated by the Insall-Salvati index and Caton-Deschamps index; the lateralised force vector was measured by the tibial tuberosity-trochlear groove (TT-TG) distance. RESULTS: Compared with controls, dislocators had significantly l

Specific complications of the mobile-bearing total knee prosthesis.

Total knee arthroplasty (TKA) is considered as a standard procedure for the treatment of the severe degenerative knee joint. The outcomes of TKA have improved over the last few decades because of a better understanding of design features and surgical techniques. Contemporary tibial component designs in TKA can be subdivided into two groups (fixed or mobile-bearing) on the basis of different fundamental principles. The mobile-bearing knee is designed with better biomechanical concepts than the fixed-bearing knee, in terms of providing congruity and mobility of tibial bearing. It is expected tha

10- to 20-year followup of total knee arthroplasty for valgus deformities.

One hundred eight knees in 83 patients with a valgus alignment of greater than 10 degrees underwent total joint replacement performed by a single surgeon using the same technique for ligament balancing, which involved releasing the lateral retinaculum and iliotibial band, followed when necessary by detaching the lateral collateral ligament and popliteus tendon from the femur. Sixty knees in 46 patients had followup of at least 10 years and were the focus of study. At an average followup of 14.1 years, the mean Knee Society knee score was 88.7 and the mean functional score was 69.2. Postoperati

Intramedullary total femoral replacement for salvage of the compromised femur associated with hip and knee arthroplasty.

Severely compromised femora because of prosthetic loosening, osteolysis, or periprosthetic fracture around total hip and knee arthroplasties are increasing. Two approaches that create an intramedullary total femoral (IMTF) replacement are reported. Twenty-three IMTF replacements in 22 patients were performed at 2 institutions. Seven revision total knee arthroplasties with a stemmed component were linked to a well-fixed hip stem with a custom intramedullary sleeve. Sixteen IMTF replacements involved revision of both hip and knee arthroplasties which were connected via an intercalary segment wit

Medial patellofemoral ligament reconstruction appears to be a better treatment than repair, proximal realignment, or conservative management for primary patellar dislocation: A network meta-analysis.

BACKGROUND: The purpose of this study was to compare the functional outcomes and re-dislocation rates of medial patellofemoral ligament (MPFL) reconstruction, MPFL repair, combined proximal realignment (CPR), and conservative management for primary patellar dislocation by conducting a systematic literature search of the available studies. The hypothesis was that MPFL repair and MPFL reconstruction would be better options for treating primary patellar dislocation. METHODS: Randomized controlled trials or prospective studies of primary patellar dislocation treated with MPFL reconstruction, MPFL

Excellent clinical and radiological outcomes after both open flake refixation and autologous chondrocyte implantation following acute patella dislocation and concomitant flake fractures.

PURPOSE: To investigate clinical and magnetic resonance (MR) imaging results of patients undergoing patella stabilization with either open flake refixation (oFR) or autologous chondrocyte implantation (ACI) and concomitant soft tissue patella stabilization after sustaining primary, acute patella dislocation with confirmed chondral and/or osteochondral flake fractures. It was hypothesized that refixation will lead to better results than ACI at mid-term follow-up. METHODS: A retrospective chart review was conducted to identify all patients undergoing oFR or ACI after sustaining (osteo-)chondral
