Fusion of lower limbs with severe urogenital malformation in a newborn, a rare congenital clinical syndrome: case report.

BACKGROUND: Fused lower limbs combined with severe urogenital malformation, also known as sirenomelia, is a rare congenital clinical syndrome. The etiology is unknown, and the outcome for the affected fetus is rather uncertain. CASE REPORT: Mrs RB, a Kuwaiti woman primigravida, married to a non-consanguineous husband, had uneventful antenatal care in a private health service, until she was admitted to the Maternity Hospital, Kuwait, at 31 weeks of gestation with a 3-hour history of ruptured membranes. She had a negative family history for diabetes mellitus, and her past surgical/medical/gynec

Safe and Efficient Removal of Massive Head and Neck Vascular Malformations Using Vessel Sealing Energy Devices.

Large vascular malformations (VMs) pose several difficult clinical challenges to the plastic surgeon. Traditionally surgical dissection was labor-intensive and required a significant amount of time and technique. The advent of more advanced vessel sealing devices has resulted in easier vascular and lymphatic control during resection surgery. The authors present 2 cases of large VMs resected using newer energy devices. First, an infant who was born with a large neck AVM that was acutely bleeding requiring immediate control. The authors utilized the Impact Ligasure device to perform a subtotal r

A rare case of isolated rectal laceration during parturition: consideration of the controversial role of the episiotomy and literature review.

BACKGROUND: Rectal laceration without sphincter lesion during childbirth is an unusual presentation. Like all the other severe lacerations during parturition, if left undiagnosed and untreated could hesitate in serious short and long term complications. Episiotomy once considered a routine procedure in nulliparous and a safeguard against severe tissue trauma nowadays is undergoing criticism and his effectiveness reconsidered. Currently, a policy of "selective" use of episiotomy is recommended only when an impending risk of lacerations is identified. When, according to this proposal, in the abs

Prospective study of maternal and neonatal outcome in great-grand multiparous women (≥10 births) and in aged-matched women with lesser parity.

PURPOSE: Evaluation of outcome of labor/delivery in great-grand multiparous (GGMP) women relative to women with lower parity is complicated because of confounding parity with (older) age. Herein, we compare maternal and neonatal outcome in GGMP from a large obstetrics department to that in older women with lesser parity. METHODS: This was a prospective observational study of older gravid women. All laboring women in a 6-month period were included in this study if they were older than 36 years and had had 2-3 previous births (low parity) or 5-6 previous births (medium parity), and also all wom

Bayesian optimization for design parameters of 3D image data analysis.

Deep learning-based segmentation and classification are crucial to large-scale biomedical imaging, particularly for 3D data, where manual analysis is impractical. Although many methods exist, selecting suitable models and tuning parameters remains a major bottleneck in practice, as manually identifying and comparing them requires significant computational and labor effort. Hence, we introduce the 3D data Analysis Optimization Pipeline, a method designed to facilitate the design and parameterization of segmentation and classification using two Bayesian Optimization stages. First, the pipeline s

Predicting asymptomatic cervical dilation in pregnant patients with short mid-trimester cervical length: A secondary analysis of a randomized controlled trial.

INTRODUCTION: Second trimester asymptomatic cervical dilation is a significant risk factor for early preterm birth. The objective of this study is to evaluate whether transvaginal ultrasound cervical length (CL) predicts asymptomatic cervical dilation on physical exam in women with short cervix (CL ≤25 mm) and no prior preterm birth. MATERIAL AND METHODS: Secondary analysis of a randomized trial on pessary in asymptomatic singletons without prior preterm birth diagnosed with CL ≤25 mm between 18+0/7 and 23+6/7  weeks. Participants had transvaginal ultrasound and physical cervical exam and wer

Risk factors for neonatal brachial plexus paralysis.

PURPOSE: The study was aimed to identify risk factors for neonatal brachial plexus paralysis. METHODS: A retrospective case-control study was designed. A comparison was performed between cases of brachial plexus paralysis, with all consecutive deliveries during the same 5 months period, without brachial plexus paralysis. Statistical analysis was performed using the SPSS package. RESULTS: The prevalence of brachial plexus paralysis was 1.62/1,000 (9/5,525) vaginal births. Independent risk factors for brachial plexus paralysis were shoulder dystocia (OR = 525; 95% CI 51-4,977, P < 0.001), vacu

Imaging the dynamics of murine uterine contractions in early pregnancy†.

Uterine muscle contractility is essential for reproductive processes including sperm and embryo transport, and during the uterine cycle to remove menstrual effluent. Even still, uterine contractions have primarily been studied in the context of preterm labor. This is partly due to a lack of methods for studying the uterine muscle contractility in the intact organ. Here, we describe an imaging-based method to evaluate mouse uterine contractility of both the longitudinal and circular muscles in the cycling stages and in early pregnancy. By transforming the image-based data into three-dimensional

Is increased peripheral ligamentous laxity in term pregnant women associated with obstetric anal sphincter injury?

INTRODUCTION AND HYPOTHESIS: Increased ligamentous laxity is associated with pelvic floor distension in pregnant women. This considered, it may also be related to the risk of obstetric anal sphincter injury (OASI). Our objective was to assess the association among increased ligamentous laxity, perineal tear severity, and OASI occurrence. METHODS: This is a prospective study. We assessed ligamentous laxity between the 36th week of pregnancy and the onset of labor, by measuring the passive extension of the nondominant index finger for a 0.26 N.m torque applied to the second metacarpophalangeal

Impact of pushing timing on occult injury of levator ani: secondary analysis of a randomized trial.

BACKGROUND: Evidence of detachment of the levator ani muscle system is seen more frequently in patients with pelvic floor disorders. It has been suggested that passive descent of the fetus before pushing could be used to decrease operative vaginal delivery and levator ani muscle injury. OBJECTIVE: This planned analysis aimed to determine whether immediate or delayed pushing was associated with an increased proportion of injury to the levator ani muscle system after the first delivery among nulliparous women. STUDY DESIGN: The Optimizing Management of the Second Stage study was a multicenter

The importance of early veno-arterial extracorporeal membrane oxygenation in the management of amniotic fluid embolism complicated by cardiac arrest: a case report and literature review.

BACKGROUND: Amniotic fluid embolism (AFE) with cardiac arrest is an obstetric emergency with an exceedingly high maternal mortality. Early application of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) may provide essential cardiopulmonary support, but its clinical value warrants further investigation. CASE REPORT: This report describes the successful management of a 36-year-old parturient with advanced maternal age who sustained sudden cardiac arrest due to AFE during the second stage of labor. Immediately following arrest, the patient received high-quality CPR and achieved retur

A case report of a pregnant woman with compensated liver cirrhosis and pancytopenia.

KEY CLINICAL MESSAGE: Liver cirrhosis may worsen during pregnancy resulting in adverse maternal and fetal outcomes. Proper antenatal evaluation, staging, and variceal screening will facilitate the management. Elective endoscopic variceal ligation (EVL) during the second trimester can prevent unexpected variceal bleeding. A multidisciplinary approach including the planning of delivery and shared decision-making is recommended for favorable pregnancy outcomes. ABSTRACT: Pregnancy in women with liver cirrhosis is relatively uncommon. During pregnancy, liver cirrhosis and portal hypertension may
