Motorist's vestibular disorientation syndrome.

Six patients are described who experienced difficulty in driving a motor car. Four had illusions that the car was turning, which occurred particularly on open, featureless roads or the brows of hills and caused the driver to stop. All patients had peripheral or central neurootological abnormalities, but the only finding consistent with the directionality of the symptoms was an unpleasantly increased sense of circularvection during optokinetic stimulation in the direction of the illusion. These problems occur because of a false sense of orientation arising either from inappropriate signals from

Labyrinthine ossification secondary to childhood bacterial meningitis: implications for cochlear implant surgery.

Of 20 children who underwent cochlear implantation for profound sensorineural hearing loss secondary to bacterial meningitis, 14 had round-window and cochlear ossification at surgery. Preoperative polytomography demonstrated ossification in 11 of these. The incidence of ossification was highest after meningitis secondary to pneumococcal pneumonia. In only one of four children with severe ossification of the labyrinth was implant surgery unsuccessful. Preliminary results indicate that mild labyrinthine ossification is not a contraindication to cochlear implantation.

Labyrinthitis ossificans.

Three cases with postinflammatory inner ear sequelae are presented to illustrate unusual histopathologic changes. Endolymphatic hydrops without changes in the perilymphatic system was present in one ear following "influenza" meningitis and labyrinthitis ossificans in the contralateral ear. The characteristic histopathological changes of the temporal bones with hematogenic bacterial infection were an extensive labyrinthine ossification associated with a generalized sclerotic change of the whole periotic bone. Bony fixation of the stapedial footplate occurred with the generalized inflammatory pr

Molecular analysis of the Pendred's syndrome gene and magnetic resonance imaging studies of the inner ear are essential for the diagnosis of true Pendred's syndrome.

Pendred's syndrome is a combination of congenital sensorineural hearing loss and iodine organification defect leading to a positive perchlorate test and goiter. Although it is the commonest form of syndromic hearing loss, the variable clinical presentation contributes to the difficulty in securing a diagnosis. The identification of the disease gene (PDS) prompts the need to reevaluate the syndrome to identify possible clues for the diagnosis. To this purpose, in three Italian families presenting with the clinical features of Pendred's syndrome, the molecular analysis was accompanied by full cl

Acute labyrinthine disorders.

A patient seeking emergency treatment for a labyrinthine disorder is usually complaining of dizziness. The task at hand in the emergency room is, first, to rule out the truly life threatening emergent disorders, and then by means of a rapid but adequate history and physical examination to aid in the differentiation between central and peripheral etiologies. If the problem is central, and especially if there are signs of a true emergency, there should be no hesitation in sharing the responsibility for the care of this patient with a neurologist or neurosurgeon. If one is convinced that the prob

Decompensated labyrinthine weakness presenting as de novo peripheral vertigo: a discrete clinical entity?

OBJECTIVE: A distinct subgroup of patients, presenting with apparently spontaneous onset of vertigo, is described. RESULTS: Although vestibular evaluation revealed caloric weakness, the proximate cause of vertigo was not labyrinthine dysfunction, but rather the loss of vestibular compensation for an older and previously compensated labyrinthine injury. CONCLUSION: Instead of addressing the vestibular weakness, effective management needs to focus on the condition that has caused the loss of compensation.

[Hearing rehabilitation in vestibular and inner ear schwannomas - Hearing aids, bone conduction Implants and CI treatment].

Patients with vestibular schwannoma (VS) in the internal auditory canal and/or cerebellopontine angle often experience hearing loss in the affected ear, which is attributable to the disease or its treatment. If conventional hearing aids do not provide sufficient speech comprehension, cochlear implantation (CI) can be an effective means of hearing rehabilitation after microsurgical tumour resection, radiotherapy or observation, if patients are carefully selected. However, the audiological results are more variable compared to the more common CI indications. A prerequisite for good hearing resul

Intraoperative use of cone-beam computed tomography in a cadaveric ossified cochlea model.

OBJECTIVES: To describe a cadaveric temporal bone model of labyrinthitis ossificans and investigate the utility of intraoperative cone-beam computed tomography (CBCT) in the facilitating cochlear implantation. DESIGN: Cadaveric temporal bone study. METHODS: Five cadaveric heads had cement introduced into the 10 cochleas. CBCT and a conventional CT scan were compared to assess the extent of cochlear obliteration. The cement was drilled-out (under CBCT guidance, if required) and cochlear implant electrode arrays (from 3 different manufacturers) inserted. RESULTS: CBCT images demonstrated temp

[Enlarged middle fossa approach to the cerebello-pontine angle. Technique and indications].

Our enlarged middle fossa approach, first published in 1982, aims to expose the internal auditory meatus and cerebello-pontine angle from above, with preservation of the cochlea and labyrinth. The steps of the operation are described, especially certain technical details: the skin incision, resection of the squamous temporal bone, extradural exposure of the petrous temporal bone, subsequent resection of the middle meningeal artery and superior petrosal sinus, extensive bone removal behind and in front of the internal auditory meatus, opening of the dura mater to expose the brain stem, and plas

The pharmacological treatment of Menière's disease.

The management of Menière's disease often provides a formidable clinical challenge largely because its precise aetiology is unknown. There is no known 'cure' once the condition is established, but drugs may be helpful in treating both the acute attacks of vertigo, and in more long-term management, particularly in the earlier fluctuant stage. These remedies are largely symptomatic and there are few if any properly controlled studies of their efficacy. Suppressant drugs may act centrally at neurotransmitter sites, or peripherally on the labyrinth. Conventional diuretics and osmotically acting ag

Retrosigmoid-internal auditory canal approach vs. retrolabyrinthine approach for vestibular neurectomy.

A new procedure, the retrosigmoid-internal auditory canal (RSG-IAC) vestibular neurectomy, has been developed, presented, and compared with the retrolabyrinthine vestibular neurectomy (RVN). The RSG-IAC involves a 3 cm retrosigmoid craniotomy removal of the posterior wall of the internal auditory canal (IAC) to the singular canal, with transection of the superior vestibular nerve and the posterior ampullary nerve. This effects a complete denervation of the vestibular labyrinth and preserves the patient's hearing. All 11 patients with Meniere's disease were cured of vertigo. Hearing was preserv

[Acute mountain sickness and Menière disease].

A professional mountain guide experienced an attack of vertigo at high altitude (4,000 m). Clinical data and the results of functional tests, were consistent with an attack of Meniere's disease. The physiopathological changes noted at high altitudes, and known as acute mountain sickness, are described. A hypothesis is suggested concerning the mechanism triggering this attack of Meniere's disease at high altitude.

Dizziness in childhood.

Dizziness in childhood is not an infrequent symptom. Accurate history taking and close co-operation between otologist, paediatrician and neurologist are necessary in the approach to the dizzy child. Most cases of childhood dizziness settle in time and investigations should be carefully selected; those with severe and persistent dizziness or ataxia should be thoroughly investigated including: EEG, ENG, calorics and CT scan. The conditions causing dizziness in children are discussed and are illustrated with case histories from our series of 27 children. Dizziness of unknown aetiology, serous oti

Perilymphatic fistula caused by Eustachian tube air inflation.

We present a case of a perilymphatic fistula (PLF) caused by Eustachian tube air inflation (ETAI) that was diagnosed using cochlin-tomoprotein (CTP) testing and successfully treated using transcanal endoscopic ear surgery to seal the inner ear window. A 77-year-old woman developed hearing loss and dizziness after undergoing ETAI at a local ear, nose, and throat clinic. Despite initial bed rest and steroid pulse therapy, the hearing did not improve, and transcanal endoscopic ear surgery was performed to repair the PLF. The CTP test confirmed the diagnosis of PLF, leading to a significant improv
