The efficacy and safety of autologous blood transfusion drainage in patients undergoing total knee arthroplasty: a meta-analysis of 16 randomized controlled trials.

BACKGROUND: Autologous blood transfusion drainage (ABTD) has been used for many years to reduce blood loss in total knee arthroplasty (TKA). We evaluate the current evidence concerning the efficiency and safety of ABTD used in TKA compared with conventional suction drainage (CSD). METHODS: We performed a systematic literature search of the PubMed, Embase, Cochrane Library and four Chinese databases. All randomized controlled trials (RCTs) that compared the effects of ABTD versus CSD in TKA were included in the meta-analysis. RESULTS: Sixteen RCTs involving 1534 patients who compared the effe

Ruptured abdominal aortic aneurysm: the state of play.

BACKGROUND: Ruptured abdominal aortic aneurysm (rAAA) remains a challenging problem: 2,410 cases were treated in Germany in 2010. Ruptured abdominal aortic aneurysm should be suspected in patients over age 50 who complain of pain in the abdomen or back and in whom examination reveals a pulsatile abdominal mass. The incidence of hospitalization for rAAA is 12 per 100,000 persons over age 65 per year (statistics for Germany, 2010), and rAAA carries an overall mortality of 80%. METHODS: The current state of knowledge of rAAA was surveyed in a selective review of pertinent literature retrieved by

Efficacy and Safety of Incremental Hemodialysis: A Systematic Review and Meta-Analysis.

Hemodialysis, the most commonly employed renal replacement therapy in end-stage kidney disease, prescriptions have long been standardized as thrice-weekly at full-dose; however, HD implementation is not without major complications. Incremental HD modalities are developed to face such one-size-fits-all approach with hopes for better preservation of residual kidney function, better quality of life and patient compliance, lower complication rates, and improvements in morbidity and mortality. A comprehensive literature search was conducted through PubMed, Scopus, the Cochrane Library, Web of Scien

Postoperative Pain Treatment After Pancreatic Surgery. A Protocol for Systematic Review With Meta-Analysis and Trial Sequential Analysis.

BACKGROUND: Pancreatic surgical procedures are technically challenging and associated with a high level of surgical stress and postoperative pain. The most common surgical intervention in the pancreas is a pancreatoduodenectomy seconded by a distal pancreas resection and total pancreatectomy. Optimal pain management is crucial for ensuring early mobilization and has been shown to reduce the length of hospital stay and the incidence of postoperative complications. The optimal strategy, however, remains a matter of controversy, and the advantages and harms related to the use of different analges

The Association Between Postoperative Fluid Balance and Postoperative Complications in Adults Undergoing Gastrointestinal Surgery: Protocol for a Scoping Review.

BACKGROUND: Fluid balance in patients who have undergone gastrointestinal surgery is known to be challenging to monitor, resulting in imprecise and inaccurate measurements. Despite this knowledge, any association between fluid balance accuracy and the development of postoperative complications in the early postoperative period is limited. This scoping review aims to map the existing knowledge between fluid monitoring and the development of postoperative morbidity, mortality, and length of hospital stays (LoS) in patients who have undergone gastrointestinal surgery. METHODS: The review will fo

Atrioventricular Junction Ablation for Cardiac Resynchronization Therapy in Atrial Fibrillation: An Updated Systematic Review, Meta-Analysis, and Meta-Regression Including Contemporary Randomized Trials.

Atrial fibrillation is a common cause of suboptimal cardiac resynchronization therapy (CRT) delivery because intrinsic atrioventricular conduction reduces effective biventricular capture. We performed an updated systematic review and meta-analysis evaluating atrioventricular junction ablation (AVJA) versus no AVJA or pharmacological rate control in CRT recipients with atrial fibrillation. PubMed, Scopus, and Cochrane Library were searched through February 16, 2026. Sixteen studies were included, comprising 14 observational cohorts and 2 randomized trials. AVJA significantly increased biventric

Comparative Effects of Antimicrobial Stewardship Interventions on Pediatric Antibiotic Prescribing: A Systematic Review of Randomized Controlled Trials.

Acute respiratory tract infections (ARTIs) are a major driver of antibiotic prescribing in pediatric outpatient care, much of which is inappropriate. Multiple randomized trials have evaluated antimicrobial stewardship interventions; however, the consistency of effects across different intervention mechanisms remains unclear. We conducted a systematic review of randomized and cluster-randomized controlled trials evaluating interventions aimed at reducing antibiotic prescribing for pediatric ARTIs in outpatient and primary care settings. Searches were performed in PubMed, Embase, the Cochrane Li

Cardiac Rehabilitation in Heart Failure: Evidence, Coverage, and Implementation.

Cardiac rehabilitation (CR) is a multicomponent secondary prevention intervention that improves functional capacity and quality of life across cardiac diagnoses, yet remains markedly underused in heart failure (HF). Existing reviews treat efficacy, coverage, delivery, and equity separately; this narrative review integrates them to clarify why uptake in HF remains low despite an established evidence base. We searched PubMed/MEDLINE and the Cochrane Library from January 2009 through December 2025, prioritizing clinical practice guidelines, landmark trials, systematic reviews, registry analyses,

[By the 150th anniversary of famous quote by Austrian surgeon G. Wegner about peritonitis: "I and my generation were raised in fear of God and peritonitis". Evolution of study and treatment of peritonitis].

The authors analyzed evolution of therapeutic approaches for acute peritonitis with accent on abdominal drainage. A systematic review enrolled the PubMed, Scopus, Web of Science, eLibrary and Cochrane Library databases for the period 1980-2024. Methods of abdominal sanitation in acute peritonitis underwent through a long way of evolution from simple passive drainage to complex programmed interventions. Historically, various approaches have been used (from closed systems to open abdominal management with laparostomy). The middle of the 20th century was marked by revolution of programmed relapar

Outcomes of extended versus standard lymphadenectomy in pancreatoduodenectomy for pancreatic cancer: systematic review and meta-analysis.

BACKGROUND: Pancreatic cancer has a poor prognosis, and surgical resection is the only curative option. Extended lymphadenectomy (EPD) during pancreatoduodenectomy may improve staging and reduce recurrence, but its survival benefits over standard lymphadenectomy (SPD) remain controversial. METHODS: A systematic search of PubMed, Embase, Web of Science, and the Cochrane Library was conducted on March 25, 2025. All studies that met the inclusion criteria were subjected to quality assessment and subsequently analyzed by meta-analytical methods. RESULTS: Nine RCTs involving 1382 patients were an

A comprehensive proteomic analysis uncovers novel molecular subtypes of gastric signet ring cell carcinoma: Identification of potential prognostic biomarkers and therapeutic targets.

Gastric signet ring cell carcinoma (GSRCC) is a distinct subtype of gastric cancer with unique epidemiological and pathogenic characteristics. However, its prognostic features and molecular landscape remain poorly understood, limiting the development of targeted therapies. In this study, we analyzed clinical data from over 10,000 patients with gastric cancer treated at Zhejiang Cancer Hospital between January 2010 and December 2019. A comprehensive proteomic analysis was conducted on 112 GSRCC patients with a signet ring cell content exceeding 70%, identifying 7322 proteins. This study establi
