Renal magnesium wasting associated with therapeutic agents.

Renal magnesium wasting is an important cause of hypomagnesemia observed in hospitalized patients. The purpose of this review is to present an index case of symptomatic hypomagnesemia associated with renal magnesium wasting during capreomycin therapy, and to survey the available literature regarding the various therapeutic agents associated with the causation of this syndrome. Finally, we have considered the pathophysiologic mechanisms that may contribute to the development of the multiple electrolyte abnormalities observed in these patients, and have outlined the current strategies to treat t…

Skeletal buffer function and symptomatic magnesium deficiency.

A major factor relating to the oversight of neurological dysfunction and seizures caused by magnesium (Mg++) depletion, involves the buffer functions of the skeletal system orchestrated primarily by the parathyroid. A Mg++ depleted subject may appear relatively asymptomatic until a short period after homeostatic responses go into effect. The results can be devastating if not recognized promptly and treated appropriately. This series of events can best be demonstrated in veterinary medicine but we propose that analogous syndromes occur in clinical medicine. Evidence is presented to support the …

Blood lymphocyte magnesium in kidney stone formers.

Blood lymphocyte, serum and urinary magnesium analyses were performed in a randomized group of 36 kidney stone-formers with the aim to determine the incidence of intracellular and extracellular magnesium depletion in urolithiasis. Lymphocyte magnesium depletion was found in 10 patients, serum magnesium depletion in 5 and concomitant lymphocyte and serum magnesium depletion in another 3 patients. Thus intracellular and/or extracellular magnesium depletion was found in 15 patients (41.7%). Oral magnesium supplementation (5-10 mmol Mg2+ daily) was found in some patients ineffective in both lympho…

Increased erythrocyte magnesium in untreated essential hypertension.

In the present study we tested the hypothesis of magnesium deficiency and intracellular magnesium depletion in essential hypertension. Atomic absorption was used to determine the erythrocyte content of magnesium in 50-year-old otherwise healthy white men with never-treated, essential hypertension (n = 12, supine blood pressure 155 +/- 4/109 +/- 2 mmHg) and in a group of particularly well-matched normotensive control subjects. The erythrocyte magnesium content was higher in the hypertensive group (P less than 0.001). No significant difference between the groups was detected for serum concentrat…

Magnesium oxide induced metabolic alkalosis in cattle.

A study was designed to compare the metabolic alkalosis produced in cattle from the use of an antacid (magnesium oxide) and a saline cathartic (magnesium sulphate). Six, mature, normal cattle were treated orally with a magnesium oxide (MgO) product and one week later given a comparable cathartic dose of magnesium sulphate (MgSO(4)). The mean percent dry matter content of the cattle feces changed significantly (P<0.001) following administration of both MgO (15.6-8.1) and MgSO(4) (17.0-8.7) but there was no significant difference between treatments. The mean rumen pH values changed significantly…

Review article. Magnesium deficiency provoked by diuretics.

Many diuretics cause hypermagnesiuria which may lead to magnesium deficiency, presenting as hypomagnesaemia, cardiac arrhythmias and tetany. Loop diuretics cause hypermagnesiuria mainly through direct blockade of magnesium reabsorption at the loop of Henle. Distal tubular diuretics block magnesium reabsorption at the distal convoluted tubule and also reduce magnesium reabsorption at the loop of Henle by an indirect mechanism.

Theophylline magnesium salicylate, a new xanthine compound.

Interaction of 1 mole of magnesium salicylate and 2 moles of theophylline in water precipitated a crystalline compound, identified as theophylline magnesium salicylate pentahydrate from analytical and supportive physicochemical data. Similarly, barium salicylate and theophylline produced theophylline barium salicylate. No precipitates were formed with calcium salicylate or strontium salicylate under the same conditions. Theophylline magnesium salicylate is not a mixture of components and differs in composition from the known theophylline calcium salicylate dihydrate. Unlike the latter compound…

Relation between potassium, magnesium and cardiac arrhythmias.

Magnesium may influence the incidence of cardiac arrhythmias by 1) a direct effect 2) an effect on potassium metabolism 3) an effect as a calcium blocking agent. In the event of a magnesium deficiency the cell cannot attract potassium against the transmembrane concentration gradient. The reason may be that a magnesium deficiency interferes with the function of membrane ATPase, and thus the pumping of sodium out from the cell and potassium into the cell is impaired. The interference from a magnesium deficiency on the equilibrium of potassium between the intra- and extracellular spaces may resul…

Magnesium deficiency: brain acetylcholine and motor activity.

Deprivation of magnesium ion in the diet caused a significant decrease in whole brain acetylcholine content. At the reginal level, the most pronounced effect was seen in the corpus striatum, diencephalon and cerebellum. The rate of depletion and recovery varied from region to region, and what appeared to be a return to normal acetylcholine in whole brain was the total of regions either higher or lower than control. The data presented here suggest that a positive relationship exists between the lowered cholinergic activity in the corpus striatum and increased nocturnal spontaneous motor activit…

Hypomagnesemic hypocalcemia secondary to renal magnesium wasting.

Two patients developed severe hypomagnesemia, hypocalcemia, and hypokalemia as a result of renal wasting of magnesium and potassium shortly after being treated with large doses of gentamicin. When therapy with gentamicin was discontinued renal loss of magnesium and potassium ceased, and serum calcium, magnesium, and potassium returned toward normal. Serum immunoreactive parathyroid hormone levels were inappropriately low during the episodes of hypocalcemia. Both patients represent examples of hypomagnesemic hypocalcemia induced by inappropriate magnesuria, possibly caused by gentamicin. These …

Changes in cellular composition during magnesium deficiency.

Mg deficiency increased the water content of the liver, kidney, heart and thigh muscle in the rat and decreased the proportion of nitrogen in the dry matter of the same tissues. Changes in the concentration of metals also occurred. 2. Cellular fractional indicated that the Mg and K depleted in liver occurred primarily in the heavy-mitochondrial and microsomal fractions respectively. The calcification of liver and kidney was due to preferential deposition of Ca in the heavy-mitochondrial fraction. 3. The proportion of cellular nitrogen present in the heavy-mitochondrial and microsomal fractions
