BACKGROUND: Implementation of mass colorectal cancer screening, using faecal occult blood test or colonoscopy, is recommended by the European Union in order to increase cancer-specific survival by diagnosing disease in an earlier stage. Post-colonoscopy complications have been addressed by previous systematic reviews, but morbidity of colorectal cancer screening on multiple levels has never been evaluated before.
AIM: To evaluate potential harm as a result of mass colorectal cancer screening in terms of complications after colonoscopy, morbidity and mortality following surgery, psychological

INTRODUCTION: HIV testing and counselling (HTC) interventions are key to controlling the HIV epidemic in East and Southern Africa where HTC is primarily delivered through voluntary counselling and testing (VCT), provider initiated testing and counselling (PITC), and home-based counselling and testing (HBVCT). Decision making processes around uptake of HTC models must be taken into account when designing new interventions. Counselling in HTC aims to reduce post-test risk taking behaviour and to link individuals to care but its efficacy is unclear. This meta-ethnography aims to understand the co

OBJECTIVE: Despite the enormous expansion of HIV testing services (HTS), an estimated 40% of people with HIV infection remain undiagnosed. To enhance the efficiency of HTS, new approaches are needed. The WHO conducted a systematic review on the effectiveness of assisted partner notification in improving HIV test uptake and diagnosis, and the occurrence of adverse events, to inform the development of normative guidelines.
METHODS: We systematically searched five electronic databases through June 2016. We also contacted experts in the field and study authors for additional information where nee

Cystic fibrosis-related diabetes mellitus (CFRD) is the most common endocrine complication of cystic fibrosis (CF), affecting more than 50% of patients by the 4th decade of life. CFRD is often preceded by worsening pulmonary status and nutritional decline. Treatment of CFRD is associated with improvements in body weight and pulmonary function and a reduction in pulmonary exacerbations. Because of the clinical significance of CFRD, diabetes screening with an oral glucose tolerance test (OGTT) is recommended annually for all patients with CF starting at age 10 years. The OGTT detects CFRD with g

Despite the continuing undertreatment, gout has seen something of renaissance in terms of research into epidemiology, genetics, diagnostic criteria, imaging and new treatments. In Metropolitan France, gout prevalence was estimated at 0.9 %. A recent genome-wide association study identified new loci of interest. An easy-to-use diagnostic rule for gout care shows good performance in primary care when joint fluid analysis is not available. Gout is associated with a number of comorbidities, which may have an effect on the development of gout and on the choice of therapeutic agents. Ultrasound allo

Increasing knowledge about the rare disease systemic sclerosis (SSc) and improved diagnostic methods in recent decades has led to the possibility of diagnosing systemic sclerosis in earlier disease stages. In this review, we describe the evolution of diagnostic and classification criteria for SSc, beginning with the preliminary ARA criteria for the classification of SSc in 1980, then presenting the criteria for limited and diffuse cutaneous SSc by LeRoy et al. in 1988 and 2001, and finishing with a discussion of the recently published new ACR-EULAR classification criteria in 2013. In addition,

The sudden demise of an athlete invariably leads to calls for prevention. Although few screening programmes live up to expectations, the 2005 European Society of Cardiology consensus statement contains a recommendation for routine pre-participation screening of asymptomatic young athletes. As the incidence of sudden cardiac death (SCD) in this group is very low (< 2 per 100,000 per year), and no test reliably identifies persons at an increased risk of SCD, the Health Council of the Netherlands advised against mandatory screening. It seems prudent, however, to consider other strategies for redu

Patients with psoriasis attending general practitioner and dermatology clinics may complain about their joints, but it may be difficult for the nonrheumatologist to distinguish psoriatic arthritis (PsA) from other forms of arthritis. A screening tool for PsA would therefore be useful to both general practitioners and dermatologists and help identify patients for further evaluation by a rheumatologist. Although several screening tools have been developed, the Psoriasis Epidemiology Screening Tool (PEST) has the advantage of simplicity and ease of use. This new instrument consists of 5 simple qu

Primary aldosteronism (PA) has a prevalence in the general hypertensive population from 5 to 10%, and is widely recognized as the most frequent form of secondary hypertension. The 2 main PA subtypes are aldosterone producing adenoma (APA) and bilateral adrenal hyperplasia (BAH) that account for 95% of all PA cases. The diagnosis of PA is a 3-step process that comprises screening, confirmatory testing, and subtype differentiation. The different categories of patients at an increased risk of PA who should thus undergo a screening test were described in the first Endocrine Society (ES) Practice G

Splenic rupture is a rare complication after colonoscopy, and to date there are only 46 reported cases in the English-language literature. Presented is a case report of splenic rupture after screening colonoscopy that resulted in laparotomy and splenectomy within 24 hours of the original procedure. The article covers the hypothesized mechanisms of injury, various precautions to take during colonoscopy, suggested diagnostic algorithm, determining factors in treatment, and vaccine regimen. The article concludes by stating that as the number of colonoscopies increase, so will the prevalence of as

Because chronic kidney disease is a growing health concern, family physicians must be equipped to care for this unique patient population. Diabetes mellitus and hypertension, which are commonly addressed in the office setting, are the largest contributors to chronic kidney disease; therefore, these risk factors should be tightly controlled and these patients should be screened closely for signs of renal damage. The National Kidney Foundation recommends that screening include determination of the glomerular filtration rate (GFR) and assessment for the presence of proteinuria. Once the diagnosis
