Contralateral bone conducted sound wave propagation on the skull bones in fresh frozen cadaver.

The study aimed to investigate the efficient pathway for BC sound transmission by measuring vibrations on the opposite side of the skull bone, referred to as the mastoid position. The realistic contralateral transmission pathway of bone conduction (BC) vibrations is investigated through each osseous structure in the midlines of the fresh-frozen whole head. BC stimulation is applied to the mastoid using a bone vibrator, and acceleration responses are observed on the contralateral mastoid bone and seven midline points of skull bones using triaxial accelerometers. The study finds that the range s…

Clinician-Guided Deep Learning Segmentation of Skull Base Pneumatization on Computed Tomography Using 3D Slicer and MONAI Label.

Skull base pneumatization is anatomically variable and clinically relevant to temporal bone and transsphenoidal surgical corridors, but manual volumetric segmentation is time-consuming. This retrospective pilot study evaluated a clinician-guided deep learning workflow for mastoid and sphenoid sinus compartment segmentation on bone computed tomography. Images were curated and annotated in 3D Slicer using MONAI Label and separate three-dimensional SegResNet models. The mastoid development dataset comprised 122 side-cases, with 28 reserved side-cases; the sphenoid dataset comprised 117 developmen…

Eustachian Tube Dysfunction Due to Medial Pterygoid Nerve Impairment After Trigeminal Schwannoma Surgery.

BACKGROUND AND IMPORTANCE: Eustachian tube dysfunction due to medial pterygoid nerve impairment is a rare complication after trigeminal schwannoma surgery. Eustachian tube dysfunction leads to fluid inside the mastoid air cell, which requires differential diagnosis with cerebrospinal fluid leakage. CLINICAL PRESENTATION: We present a 49-year-old male patient of trigeminal schwannoma. The extradural middle fossa rhomboid approach was performed. The mastoid air cell was opened during the drilling of the middle skull base. After surgery, mastoid air cell fluid was observed. The fluid volume did …

Mastoidectomy for chronic serous otitis media.

The purpose of this paper is to show that satisfactory results following mastoidectomy can be obtained for the problem of chronic serous otitis. The success of the simple mastoid operation or mastoidectomy with facial recess approach and the subsequent increased aeration of the mastoid-antral-attic-middle-ear complex is to be suggested when the repeated use of myringotomy tubes alone is not the answer to chronic serous otitis. It is chronic negative pressure due to eustachian tube dysfunction that leads to irreversible mastoid disease. Seven cases of mastoidectomy for chronic serous otitis med…

Utilizing temporal imaging to enhance early diagnosis of eosinophilic otitis media in clinical practice.

PURPOSE: The diagnosis of eosinophilic otitis media (EOM) remains a major challenge. Current diagnostic methods rely primarily on accurate samples of middle ear tissue or secretions, which can be complex. In this study, we sought to explore the diagnostic value of mastoid pneumatization combined with other temporal radiological features for EOM. METHODS: This study included 28 patients with EOM and 32 patients with ordinary OM. All participants underwent the temporal bone HRCT, and middle ear status was assessed using four sections: mesotympanum, attic, antrum, and mastoid air cells. Quantifi…

Which is mightier, the tuning fork or the bone oscillator?

INTRODUCTION: It is important to differentiate cochlear implant candidates with profound sensorineural hearing loss from those with profound mixed hearing loss. The latter include patients with far advanced otosclerosis and chronic otitis media who may be better treated with ossiculoplasty and conventional amplification than with cochlear implantation. Otologists have observed that a dentally placed tuning fork can be heard by some patients whose sensorineural thresholds are beyond the limits of a bone oscillator placed on the mastoid. We hypothesized that tuning forks may be able to deliver a…

Measurement of middle ear volume using the impedance audiometer.

Ears with chronic otitis media are characteristically associated with small mastoid air cell systems. This also indicates a small air volume in these ears, which may influence the pathophysiology and course. Therefore, it may be important to get a measure of the air volume in the middle ear mastoid air cell system. Conventional mastoid X-ray investigations have so far been used for estimating this volume. As impedance audiometry has gained in popularity during recent years, it has been postulated that this tool may be useful for measuring the volume in the middle ear and the mastoid system. In…

Otomastoiditis Versus Otomastoid Effusion.

On imaging reports, mastoiditis can span clinical entities ranging from a benign incidental finding of mastoid effusion to fulminant coalescent mastoiditis. This range of entities has different levels of medical urgency and appropriate therapy. Differentiating among possibilities requires consideration of the entire clinical picture. A review of the images themselves, often in conversation with the interpreting radiologist or otolaryngology consultant, is helpful in resolving the ambiguity created when encountering the nonspecific term mastoiditis.

Otomastoiditis-related facial nerve palsy.

A 9-year-old girl with persistent otitis media, despite antibiotic therapy developed a facial nerve palsy. Computed tomography (CT) scan revealed ipsilateral mastoiditis, prompting admission for intravenous antibiotic and steroid therapies. Acute mastoiditis, uncommon in the post-antibiotic era, is usually diagnosed on physical examination findings, but two variants, masked mastoiditis or silent mastoiditis, may be difficult to appreciate clinically. Patients who present with facial nerve palsy in the setting of persistent otitis media should undergo CT scan for evaluation of intracerebral or …

Postauricular myocutaneous flap in otologic surgery.

Management of a large mastoid defect resulting from skull base operations or extensive surgical procedures because of chronic ear disease continues to challenge the otologic surgeon. Various local muscle or periosteal rotation flaps have been used to help reduce the size of the postoperative mastoid cavity. With these techniques there are problems with flap retraction and epithelization that may result in delayed healing or chronic drainage. Closure of the ear canal and tissue obliteration of the mastoid results in a maximal conductive hearing loss. A postauricular myocutaneous flap based on t…

Transmastoid encephaloceles. A case report.

Encephaloceles are uncommon abnormalities occurring in the pediatric age group. For the otolaryngologist they are usually encountered in the nasopharynx or nose, but may also be found in other sites adjacent to the brain. Encephaloceles usually develop as a result of prior mastoid surgery, trauma, or infections; but it is extremely rare for large encephaloceles to occur congenitally in the mastoid. It is even more uncommon for there to be a second mastoid defect which would allow the brain to present as a subcutaneous postauricular mass. The case history of a child with congenital subcutaneous…

Mastoidectomy update.

The long-term results of 375 primary mastoidectomy operations for chronic otitis media and chronic mastoiditis are described and discussed. Two-thirds of these cases were open cavity mastoidectomies and 1/3 closed cavity mastoidectomies. Primary pathology was cholesteatoma in 1/3 and granulation tissue in 2/3 of the cases. The method of reducing the size of the mastoid cavity by sculpturing mastoid cavity by sculpturing mastoid cortical bone is most important in avoiding large postoperative cavity problems. Principles and methods are described. Healing (dry ear) is a more important management …
