The use of cone beam CT for the removal of wisdom teeth changes the surgical approach compared with panoramic radiography: a pilot study.

This prospective study evaluated the role of cone beam computed tomography (CBCT) in the treatment of patients with impacted mandibular third molars at increased risk of inferior alveolar nerve (IAN) injury. Subjects with an increased risk of IAN injury, as diagnosed on panoramic radiographs, were enrolled in this study and underwent additional CBCT imaging. Two oral maxillofacial surgeons independently planned the surgical technique and estimated the risk of IAN injury on panoramic radiographs and on CBCT images. A test of symmetry and the McNemar test were executed to calculate the differenc

An ultra-late maxillofacial cavity following multimodal treatment for adult head and neck rhabdomyosarcoma: a case report with a 16-year follow-up.

OBJECTIVE: To report a rare case of a patient with adult rhabdomyosarcoma (RMS) in the craniofacial region who developed a very late-stage left maxillofacial cavity after 16 years of multimodal treatment, and to systematically investigate the potential causal relationship between this complication and previous radiotherapy. METHODS: A detailed report is presented on the diagnosis and treatment process of a 46-year-old female patient. A comprehensive evaluation framework was employed to analyze causality, encompassing temporal correlation, spatial alignment with the original radiation field, e

The complete anatomy of the lingual nerve: A meta-analysis with implications for oral and maxillofacial surgery.

Lingual nerve (LN) injury during surgical procedures in the third molar region warrants a detailed study of its common pathway and important variations. Therefore, the objective of this study was to analyze and compile the multiple anatomical variations of the LN for use in oral and maxillofacial surgery. It is anticipated that the results of the present meta-analysis may help to minimize the possible complications when performing procedures associated with this anatomical entity. Major online databases such as PubMed, Web of Science, Scopus, Embase were used to gather all relevant studies reg

Videolaryngoscopy As the Primary Approach to Emergency Airway Management in Facial Trauma: A Case Report.

Managing the airway in maxillofacial trauma poses significant challenges. The distorted anatomy often complicates face mask ventilation and intubation, necessitating specialized skills in emergency settings. Successful management hinges on prompt planning and patient cooperation. Here, we describe the airway management of a bullhorn fascial injury patient requiring wound exploration and repair under general anesthesia. Timed clinical judgment and an adaptable approach ensured effective airway management using videolaryngoscopy as the primary approach in this case. In addition, we have describe

[Postoperative antero-external tibial compartment syndrome: co-responsibility of the operating table].

A 26-year-old, ASA1 patient underwent maxillofacial surgery under general anaesthesia, of 12-hour duration in the supine position. Postoperatively he developed rhabdomyolysis and acute renal failure. In the subsequent days, a bilateral leg compartment syndrome occurred with anterior tibial motor nerve injury requiring fasciotomies and excision of necrotic muscles. Several aetiological factors may have contributed to this accident: a long-lasting procedure, controlled hypotension and inappropriate position of the lower limbs. A laboratory study showed that the hardness of some new operating tab

Developing a craniomaxillofacial and cervical equipment module for surgeons in the austere environment: a systematic review.

The treatment of craniomaxillofacial and cervical wounds in a disaster relief setting is done by clinicians from local medical treatment facilities, non-governmental organisations (NGO), or the military. Although each group and individual surgeon will need specific equipment, this will be restricted by weight, portability and interoperability. We systematically reviewed scientific and commercial publications according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The papers we identified described the portable equipment that is required to treat

Solitary Frontal Sinus Fractures Compared to Multiple Facial Fractures, Energy Impact Dependency.

Frontal sinus fractures account for 2% to 15% of maxillofacial injuries. Up to 66% to 87% of the patients with frontal sinus fractures experience associated facial fractures. The majority of classifications used today categorize frontal sinus fractures depending on the integrity of the anterior table, posterior table, and the nasofrontal outflow. A retrospective study was performed, which included 24 patients diagnosed with frontal sinus fractures. Treatment in all patients consisted of open reduction and bone fixation. We analyzed population variables, injury etiology, fracture site, associat…

Incidence, aetiology and pattern of mandibular fractures in central Switzerland.

PURPOSE: The two major causative factors for mandibular fractures, as stated in the literature, are either interpersonal violence or motor vehicle accidents. The purpose of this study was to describe epidemiological trends of mandibular fractures in Switzerland. A special emphasis was directed towards the potential impact of socio-economic standards on the mechanism and pattern of mandible fractures. PATIENTS AND METHODS: A database of patients aged over 16 years who had been diagnosed with a mandibular fracture between January 2000 and December 2007 at the University Hospital of Bern, Switze

Malpractice claims for permanent nerve injuries related to third molar removals.

On the basis of the register of the Finnish Patient Insurance Association, the aim of this study was to examine malpractice claims for nerve injuries associated with third molar removals and determine whether they are concentrated among specialists, among less experienced dentists, or in certain geographic areas. During 1987-93 there were 139 claims for permanent sensory or motor disturbances related to removal of lower third molars in Finland. The lingual nerve was injured in 54% and the inferior alveolar nerve in 41% of the claims. In 91% of the cases the injury occurred in relation to surgi

Reconstruction of Extensive Composite Parotid Region Oncologic Defects with Immediate Facial Nerve Reconstruction Using a Chimeric Scapulodorsal Vascularized Nerve Free Flap.

BACKGROUND: Cancer involving the parotid gland region may originates from parotid parenchyma itself or from locoregional organs and in rare cases, the facial nerve (FN) has to be sacrificed during tumor resection. In these cases, cancer extension often goes beyond the parotid compartment and requires extensive local resection responsible for complex multitissular defects. The goals of reconstruction may be summarized in the following two components: (1) restoration of the volumetric tissue defect and (2) FN reconstruction. The aim of this study is to describe our surgical technique and our cos

US/ROS-responsive ALN-modified polymer nanoparticles enhance the efficacy of infiltration anesthesia through improved bone targeting and permeation.

Complex maxillofacial bone anatomy compromises the efficacy of local anesthetics. This study develops an ultrasound (US)/reactive oxygen species (ROS) cascade-responsive polylactic-coglycolic acid (PLGA) nanoparticle system for enhanced dental infiltration anesthesia and perioperative pain management. The nanoparticles, which were surface-modified with alendronate (ALN) via an ROS-cleavable thioketal (TK) linker and coloaded with bupivacaine (BUPI) and perfluorohexane (PFH), enabled enhanced bone targeting and penetration. Under US stimulation, PFH undergoes a liquid-gas phase transition, gene

Epidemiological analysis of the facial fractures pattern in relation to motorcycle helmet type: A retrospective study on 282 patients.

Most maxillofacial traumas are caused by road traffic collisions and, in particular, by motorcycle accidents. Helmets represent an efficient protective tool in these traumas but they do not provide complete protection. The aim of this study was to perform an epidemiological analysis of facial fractures pattern in relation to the helmet type worn during the accident. The study was a retrospective analysis of 282 patients with a diagnosis of maxillofacial fracture caused by a motorcycle accident. The patients were divided in three groups based on helmet type (Group A: full-face helmet; Group B:
