[Physical exercise and insulin use: challenges for people with diabetes mellitus].

Physical exercise has many health benefits, equally so for people with diabetes mellitus. The glycaemic responses to the various types of exercise differ and include an increased risk of late (nocturnal) hypoglycaemia, making physical exercise a challenge for some people with diabetes who are treated with insulin. Insulin treatment interferes with normal physiologic responses to exercise, which are necessary to maintain the blood glucose level within the normal range. During aerobic exercise, the blood glucose concentration usually drops, whereas anaerobic exercise generally causes a rise in g

Cogan syndrome in pregnancy.

BACKGROUND: Cogan syndrome is a rare, multisystem, autoimmune disorder of unknown etiology. Little is known about whether it affects pregnancy or whether pregnancy affects the disease. CASE: A 24-year-old primigravid woman with Cogan syndrome diagnosed 3 years before her pregnancy presented to our clinic for prenatal care. During pregnancy she experienced no worsening of symptoms of her disease but reported subjective improvement in vision and hearing. Cesarean delivery was performed at term because of nonreassuring fetal status. There were no obstetric or postpartum complications. CONCLUSIO

[Physical measurement of the dorsal kyphosis in 14-17-year old patients with Scheuermann's disease and in matched healthy controls].

The modified use of the Debrunner-kyphometer (a special protractor to measure the dorsal kyphosis by an objective physical method) is presented. After proving the validity and reproducibility of the measuring method, the dorsal kyphosis of patients with Scheuermann's disease and of matched healthy controls was measured. The aim of the measurements was to assess the physiologic values of kyphosis and to investigate into the distribution of Scheuermann-patients regarding the grade of their dorsal sagittal curvature. In contrast with the everyday routine (subjective judgement of the sagittal curv

Acute respiratory failure.

Pregnancy entails many unique physiologic changes that make the management of ARDS both challenging and unique. Most of these women are young and in excellent health before their acute injury, and their survival should exceed that of the general population who develop ARDS. We advocate aggressive management of the suspected lung injury including early intubation and the institution of invasive hemodynamic monitoring. This approach ensures the most thorough assessment of the extent of the initial injury and allows rapid assessment of therapeutic maneuvers and their subsequent adjustment. Our go

Disturbances in acid-base metabolism. Recognition in the office setting.

Determination of the appropriateness of the compensatory response is essential in evaluation of any acid-base disturbance. Adequate compensation supports the presence of a single acid-base event. Inadequate compensation may indicate that a second primary acid-base event is present or, in some circumstances, that time has been inadequate for maximal physiologic compensation. When acid-base disturbances are mixed, treatment of one disorder may unmask the full pH-altering potential of the second disorder. Close monitoring of patients at risk for the development of acid-base disorders permits dete

Fluid therapy for gastrointestinal, pancreatic, and hepatic disorders.

Patients with gastrointestinal, hepatic, or pancreatic disease frequently require parenteral fluid therapy. The selection of the type and amount of fluid for administration should be based on a knowledge of physiologic derangements and the controlling mechanisms. Table 4 provides suggested replacement fluids and supplements for each disease. It must be remembered, however, that individual "tailoring" is required for each case. In addition to an expert knowledge of pathophysiology, monitoring of the patient's weight, fluid losses, serum electrolyte, and acid-base status is necessary for success

Predictive factors and mechanism of arrhythmias and myocardial ischaemic changes in elderly patients during barium enema.

The incidence of significant arrhythmias and ST segment changes during barium enema examination (BE) was evaluated by Holter monitoring of 58 unselected patients over the age of 60 years. Forty percent of the group developed new significant arrhythmias of which the most common were frequent and/or multifocal premature ventricular contractions. Seven percent demonstrated ST segment depression. Administration of glucagon did not diminish the incidence of arrhythmias. Predictive factors for ECG abnormalities were advancing age, abnormal pre-BE-ECG, and pre-BE orthostatic hypotension. Analysis of

Exocrine Pancreatic Insufficiency Treated With Pancreatic Replacement Enzyme in a Premature Neonate: A Case Report.

Exocrine pancreatic insufficiency (EPI) in neonates is rare and challenging to diagnose, particularly in extremely preterm infants in whom physiologic pancreatic immaturity may mimic disease. Persistent malabsorption, poor growth, and repeatedly abnormal fecal elastase-1 (FE1) levels can help differentiate true EPI from transient developmental insufficiency. An extremely preterm female born at 25.0 weeks adjusted gestational age (AGA), weighing 775 g, developed feeding intolerance, direct hyperbilirubinemia, and growth failure during her neonatal intensive care unit course. Serial FE1 testing

Goal-directed therapy: what is the goal again?

Recent attention has focused on intraoperative hypotension (IOH) and hemodynamic instability. This enhanced awareness to limit IOH, combined with fluid restriction and increased vasopressor use, has been associated with an increase in postoperative acute kidney injury. Recent literature supports improved intraoperative monitoring of mean arterial pressure (MAP), fluid management, and appropriate use of vasopressors and inotropic support for hemodynamic management. Implementing an algorithm to manage the causes of IOH minimizes iatrogenic harm by guiding anesthesia clinicians to select the appr

Acid Suppression for Gastroesophageal Reflux Disease in Infants.

Gastroesophageal reflux is a normal physiologic occurrence that is common throughout infancy and usually resolves on its own. Infrequently, reflux causes complications and turns into gastroesophageal reflux disease (GERD), which may warrant intervention. Available interventions vary in invasiveness and supporting data may be lacking for efficacy and safety. Nonpharmacologic interventions are first-line therapy for GERD in infants, whereas pharmacologic and surgical approaches are controversial. Efficacy data are limited for pharmacologic strategies for infantile GERD and safety data have demon

Doppler ultrasound in all three trimesters of pregnancy.

Doppler ultrasound velocimetry of uteroplacental, umbilical, and fetal vessels provides the clinician with important information on the hemodynamics of the vascular area under study. Transvaginal color Doppler is a new technique that allows precise investigation of fetomaternal circulation in early pregnancy. Doppler can be used to follow physiologic hemodynamic changes in uteroplacental and fetal vessels in this period of gestation, and to measure circulatory disturbances in some pathologic conditions. In late pregnancy, Doppler is a useful diagnostic tool for monitoring fetuses with intraute

Special considerations in the use of NSAIDs in the elderly.

As the number of elderly patients with rheumatic conditions continues to increase, rheumatologists are faced with the challenge of providing more effective but safer therapy to their geriatric patients. In 1991, nonsteroidal anti-inflammatory drugs (NSAIDs) accounted for more than 29 million prescriptions worldwide. The elderly are at increased risk for NSAID-associated toxicity, particularly gastrointestinal side effects. Predisposing factors include age-related physiologic changes, underlying conditions, and polypharmacy. Clinical experience shows etodolac, 300 mg twice daily, given to elder

The Relationships of Cerebral and Somatic Oxygen Saturation with Physiological Parameters in Pediatric Cardiac Surgery with Cardiopulmonary Bypass: Analysis Using the Random-Effects Model.

Recently, tissue oxygenation in pediatric heart surgery is measured by using near-infrared spectroscopy. Monitoring of cerebral oxygen saturation (ScO2) is most common but that of somatic tissue oxygen saturation (SrO2) is also gradually becoming widespread. However, the value of their monitoring is not well established. One of the reasons for this may be that the physiological factors affecting ScO2 and SrO2 have not been sufficiently clarified. Accordingly, we prospectively observed the changes in ScO2 and SrO2 simultaneously throughout cardiac surgery with cardiopulmonary bypass (CPB) in ch

Acute Kidney Injury After Pediatric Heart Transplant: The Impact of Intraoperative Hemodynamics Using High-Fidelity Data.

Acute kidney injury is a frequent complication after pediatric heart transplant and is associated with long-term renal dysfunction and mortality. Factors associated with acute kidney injury have been studied; however, the role of intraoperative physiologic monitoring, particularly renal near infrared spectroscopy, remains incompletely understood. This study aimed to evaluate the association between intraoperative hemodynamic markers with the development of postoperative acute kidney injury in pediatric heart transplant recipients. This was a single-center retrospective study of pediatric patie

Beyond the Snapshot: Overcoming GES Limitations to Characterize Gastric Phenotypes and Therapeutic Responses Using a 6-Day Ambulatory Wearable Monitor.

BACKGROUND: Gastric emptying scintigraphy (GES) is considered the standard for differentiating gastroparesis from functional dyspepsia and other foregut disorders characterized by nausea and vomiting but provides only a brief physiologic snapshot that may not accurately reflect daily function. We utilized a non-invasive wireless patch system (WPS) to characterize multiday gastric myoelectrical activity, evaluate its relationship with GES, and assess changes following a therapeutic intervention. METHODS: Adults referred for GES for symptoms thought to represent gastroparesis were enrolled in a

Emerging trends and clinical challenges in AI-enhanced emotion diagnosis using physiological data.

This review explores the relationships between physiological parameters and emotions, as well as the potential value and applications of the use of machine learning to facilitate emotion recognition. First, the relationships between physiological parameters (such as heart rate, respiration, blood pressure, galvanic skin response, electroencephalography, and heart rate variability [HRV]) and emotions are discussed. The impacts of emotional states on these physiological parameters represent a crucial aspect of emotion research. For example, the increased heart rates and faster breathing resultin

Physiological correlates of beat-to-beat, ambulatory, and day-to-day home blood pressure variability after transient ischemic attack or minor stroke.

BACKGROUND AND PURPOSE: Visit-to-visit and day-to-day variability in systolic blood pressure (SBP) are associated with an increased risk of stroke, more strongly than variability on 24-hour ambulatory BP monitoring, but underlying physiological mechanisms are unclear. We related potentially relevant physiological characteristics to beat-to-beat, ambulatory, and day-to-day BP variability to identify underlying mechanisms and potential therapeutic targets. METHODS: BP variability (coefficient of variation [CV]) on 1-month home BP monitoring (3 sitting readings, 3× daily), on 24-hour ambulatory

Artificial Intelligence and Sensing Technologies for Vascular Access in Hemodialysis: A Narrative Review.

Vascular access performance determines the adequacy, safety, and cost of hemodialysis. Stenosis, thrombosis, cannulation difficulty, infection, and access-related cardiac stress remain major sources of morbidity. This narrative review examines artificial intelligence and sensing technologies for vascular access care, based on PubMed and ClinicalTrials.gov searches through 22 August 2026 with citation chaining. Existing reviews have examined artificial intelligence in dialysis broadly or prediction models in isolation. In this review, the vascular access itself is kept as the unit of analysis,
