Blood-CSF barrier integrity in multiple sclerosis.

INTRODUCTION: In about 20% of MS patients an increased CSF/serum albumin quotient (QAlb) has been observed. The reason for this blood-CSF barrier dysfunction is yet unclear. SUBJECTS AND METHODS: QAlb values from 48 MS patients in relapse were correlated with parameters of active CNS lesions as measured by gadolinium-DTPA MRIs. QAlb values from 20 MS patients without relapse served as controls. RESULTS: Mean QAlb values (x 10(3) of a group with spinal cord lesions (7.6 +/- 3.6; n = 16) differed significantly from those of a control group (4.6 +/- 1.5; n = 20; p < 0.005) as well as from those

Periodic lateralized epileptiform discharges in multiple sclerosis.

We report an unusual cause of periodic lateralized epileptiform discharges (PLEDs) in a young man with a long history of multiple sclerosis. Two exacerbations of multiple sclerosis, 3 years apart, were complicated by complex partial status epilepticus. After each episode, serial electroencephalograms revealed PLEDs (right frontal PLEDs in the first episode and right frontotemporal and bifrontal PLEDs in the second one), which resolved within 2 weeks. With the first episode, magnetic resonance imaging revealed an enhancing white matter lesion in the right frontal area, which improved after 3 we…

Binaural interference in multiple sclerosis: case study.

A case report of a young, adult male with multiple sclerosis who demonstrates binaural interference is presented. Binaural interference was demonstrated on behavioral and physiologic measures during the active stage. Binaural interference was present, although reduced, during the stage of remission. During remission, binaural interference occurred despite the absence of interaural asymmetry in the audiometric configuration or suprathreshold speech-recognition score. A trial period of left ear amplification was introduced. A possible explanation for the improvement in soundfield suprathreshold …

Case study: mania associated with multiple sclerosis.

Mania occurs sometimes in association with an organic condition affecting the brain. A case of a 15-year-old girl with secondary mania related to multiple sclerosis is described, along with a selective review of the subject. The English-language literature published from 1986 to 1994 was reviewed, but geriatric and mentally retarded cases were excluded. This case points out that organic causes, often multifactorial, have to be kept in mind when treating patients who have mania.

[Dissociated sensory loss syndrome in multiple sclerosis].

Spatial and temporal dissemination of demyelinating lesions continue to provide the basis for diagnosing multiple sclerosis (MS). We describe 20 patients (from a series of 234 with MS) who experienced flare-ups consistent with sensory suspension syndrome (SSS). The presence of syringomyelic cavities (non communicating syringomyelia) was ruled out by nuclear magnetic resonance imaging (NMR). We discuss the possible locations of lesions responsible for this syndrome: the trigeminus, dorsal root entry zones, anterior medullary white matter, and the mid-lateral portion of the spinothalamic tract. …

Lower urinary tract dysfunction in multiple sclerosis.

Previous investigators have shown that in multiple sclerosis failure to empty the bladder was secondary to detrusor-distal sphincter dyssynergia or areflexia. However, our urodynamic evaluation of 46 female and 43 male patients with multiple sclerosis revealed that 63 percent of patients failed to empty their bladders because of a hypocontractile detrusor, and only 6 percent had areflexia. Detrusor-distal sphincter dyssynergia (6%) and bladder neck obstruction (6%) were present in only 12 percent of patients. Hyperreflexia was common (78%) and was associated with hypocontractility in 63 percen…

Immunological differentiation between neuroborreliosis and multiple sclerosis.

Neuroborreliosis, a tick-borne spirochaetosis of the central nervous system, is diagnosed by the presence of intrathecally synthesized Borrelia burgdorferi-specific antibodies. Multiple sclerosis and neuroborreliosis can show similarities in clinical symptoms as well as lymphocytic cell reactions and oligoclonal bands in the isoelectric focusing of cerebrospinal fluid. To differentiate between multiple sclerosis and neuroborreliosis we tested intrathecally synthesized IgM and virus antibodies. The IgM indices were higher for most of the neuroborreliosis patients studied than for those with mul…

Multiple sclerosis following optic neuritis in Chile.

Twenty three unselected cases of optic neuritis were re-examined between 2 and 18 years after the onset of the disease, with a mean of 9.7 years. Only 8/23 (35%) had unilateral non-recurrent disease. One patient only (4.3%) went on to develop multiple sclerosis. These findings support the impression that demyelinating disease varies considerably in Latin America compared with northern industrialised countries.

Cognitive event-related potentials in multiple sclerosis.

Twenty-three patients with multiple sclerosis (MS) and 1 patient with isolated optic neuritis (mean age 37.5 yrs, average duration of disease 10 yrs) were tested with auditory and visual cognitive event-related potentials (ERPs) using an oddball paradigm. The results were compared with data from 19 age-matched healthy controls. All patients had magnetic resonance imaging (MRI) head scans and 19 had comprehensive neuropsychological testing. The stimulus-related components of the ERPs were normal for nearly all patients, but 10 showed abnormalities of the later cognitive-related components and a…

Ocular inflammatory changes in established multiple sclerosis.

Fifty consecutive patients with clinically definite multiple sclerosis were studied to assess the prevalence of concomitant uveitis. Asymptomatic ocular inflammatory changes were found in nine patients (18%) and appeared to show a positive correlation with severe and progressive disease. Conversely uveitis was uncommon in the presence of established optic atrophy which suggests a negative influence on its pathogenesis. In the absence of optic atrophy inflammatory changes in the eye may be a valuable index of disease activity.

Multiple sclerosis presenting with progressive visual failure.

Progressive visual failure as the presenting feature of multiple sclerosis is described in five patients. The clinical features did not permit a distinction from visual loss due to compression. The finding of oligoclonal bands in the CSF at presentation is a useful pointer to the diagnosis, but is not specific and full investigation to exclude treatable causes of visual loss is essential.

[Asymptomatic multiple sclerosis - 3 cases (author's transl)].

Multiple Sclerosis (MS) cases found at autopsy in patients who had died from other diseases and in whom no sign or symptom could be related to MS are called "asymptomatic". Three cases are reported. The first patient was a 62 year old man who presented with a slowly progressive disturbance of gait, incontinence and deterioration of intellectual function. A falx meningioma was surgically removed. The patient died 3 years later with an acute respiratory illness. Examination of the brain disclosed evidence of the operation and numerous old plaques disseminated through the cerebral hemispheres (ce…

Occult fifth nerve dysfunction in multiple sclerosis.

Somatosensory evoked potentials (SEPs) were recorded following trigeminal nerve stimulation in 25 normal subjects. Mucosal stimulation of the lip resulted in a reduced stimulus artefact. The three initial peaks, N13, P19, and N30, measured respectively 12.8 +/- 0.9 S-3, 19.3 +/- 1.4 S-3, and 28.6 +/- 1.7 S-3. Blink reflex studies were also performed in most of these subjects. In 41.4% of 29 patients with established or suspected multiple sclerosis, the trigeminal SEP was abnormal. Additional use of the blink reflex raised the overall incidence of trigeminal nerve dysfunction to 51.7%. None of …

Pattern visual evoked responses in multiple sclerosis.

The latency of the first major positive component of the visual evoked response to pattern reversal was measured from a midoccipital electrode for each eye in 30 patients with a definite diagnosis of multiple sclerosis (MS). Their latencies were compared with those of a group of 18 normal subjects and 18 patients with other neurological diseases. Of 26 MS patients with a measurable response, 77% gave values that were outside the ranges of the two control groups. A total of 75% of the MS patients without history or findings of optic neuritis had an abnormal response. The procedure appears to be…

[Paroxysmal syndromes in multiple sclerosis (author's transl)].

Paroxysmal syndromes do not occur frequently in the course of multiple sclerosis, but require diagnostic considerations of particular nature. The pathogenesis and clinical aspects of a) cerebral convulsions, b) (usually appearing unilaterally) tonic brain stem seizures, c) narcoleptic attacks, d) hemiballismus, e) acute attacks of vertigo, f) paroxysmal dysarthria, g) trigeminal neuralgia are discussed.

Multiple sclerosis: some epidemiological clues to etiology.

The diagnosis of multiple sclerosis is frequently made with undue haste and without a firm basis. A false positive diagnosis is made in about 20 to 30% of the cases originally labeled as multiple sclerosis. The proportion of false positive diagnosis is probably still higher in countries where this disease is less frequent. An exhaustive investigation (neurological, clinical, neurorradiological, isotopic, etc.) is necessary before accepting such a diagnosis. This is particularly important because many of the lesions which can masquerade as multiple sclerosis are amenable to medical or surgical …
